after having been sworn to in accordance with law, depose and say:
1. At the meeting of the Members of the Board of Directors of
Opportunity Kauswagan Bank, Inc, held on _________________ at which quorum
was present and acting throughout, the following resolutions were adopted:
Resolved that Opportunity Kauswagan Bank, Inc. files as
it hereby files an appeal to the Department of Justice in
connection with resolution dated 15 October 2010 and
Resolution dated 24 November 2010 of the office of the City
Prosecutor of Antipolo City, NPS Docket No. XV-01-INV-10G00954 to 00973, entitled Gerardo A. Anit vs. Noel M. Alcaide et.
al., and/or in the event it is needed files appropriate petition to
the Court of Appeals or Supreme Court;
Resolved Further, to authorize GERARDO A. ANIT to file
said appeal and/or petition or any appropriate proceedings in
the Department of Justice, Court of Appeals or Supreme Court
and for this purpose also sign, make, execute therein in behalf
of Opportunity Kauswagn Bank, Inc. the required sworn
Certification Against Forum Shopping and Verification as
required by law;
Resolved Finally, to authorize said GERARDO A. ANIT full
power and authority to do and perform any and every act and
thing whatsoever requisite or necessary to be done in and
about the premises.
2. I certify that the foregoing excerpt of the minutes of the meeting of
the Members of the Board of Directors is part of the records of Opportunity
Kauswagan Bank, Inc and has not been amended or repealed.
