Whats the Shubitz Family Clinic?


The Shubitz Family Clinic is a student-run clinic. Under the supervision of volunteer attending physicians,
medical students coordinate and provide culturally appropriate preventive care, primary care, referrals and
diagnostic tests to patients with no other source of healthcare.

What do I bring?
MS1/MS2 (Intakes): Laptop (iPad not acceptable), Stethoscope, Hospital ID Badge
Attire: Professional or Scrubs (only if lab volunteer)
MS3/MS4 (Caregivers): Laptop or iPad, Stethoscope, Hospital ID Badge
Attire: Professional or Scrubs
Other- Triage, Translator: Laptop or iPad, Stethoscope, Hospital ID Badge, Watch (optional)
Attire: Professional

What do I do?

Upon arriving at 5:30pm, please sign-in on the volunteer sign-in form and wait in the room designated for
medical students in the UAMC Satellite Clinic.
The student-in-charge will give a brief introduction to all volunteers. At this point, you will transition into
your positions roles for the evening (see below).

INTAKES AND CAREGIVERS:


1. Following the volunteer introduction, the student-in-charge will help MS1/MS2s log into practice fusion, at
the following site: https://static.practicefusion.com/apps/ehr/#/login
2. The student-in-charge will pair a MS1/MS2 (Intake) with a MS3/MS4 (Caregiver). This is a good time to
introduce yourself to your assigned partner, and review the format of a SOAP note.
3. At 6:00pm, patients will begin signing in at the front desk. The vitals are then taken by the triage volunteer. 
4. The student-in-charge will assign patients to a caregiver/intake pair when they arrive. 
5. Each pair will review the patients chart and vitals prior to their interaction with the patient, while the Triage
person measures vitals. If the patient is new to the clinic, the pair will review the patients chief complaint. 
6. The student pair will enter the room designated for their patient and conduct a focused history and physical
examination (H&P). 
7. After performing the H&P, intakes will present to the attending physician in a separate room where they can
order lab tests, or write prescriptions. 
8. Intake/caregiver pair and attending will enter the patient room for follow-up. 
9. If lab tests are needed, intake/caregiver pair will walk over the lab form (filled with patients information
and the attendings signature) with the patient to the lab. 
12. The student pair will write a SOAP note in practice fusion upon completing the H&P where the attending
will approve and sign upon completion. 
13. BEFORE patient leaves, intake/caregiver pair should make sure the student-in-charge made copies of
any forms related to the patients care (i.e. laboratory form, prescriptions).

TRIAGE:
The student-in-charge will give a brief training prior to start of the clinic. Youll take vitals on each patient and
document in Practice Fusion:
Height
Weight
Blood pressure
Heart rate
**Write down the measurements on the pink slip of paper (the SIC will provide you with this) and
include the patients room number and initials (no DOB). Hand off to designated caregiver/intake pair
after you have escorted patients to the assigned room.
After vitals have been taken, you will assist the Student-in-Charge with clinical tasks, such as paperwork,
dispensing medications, and collaborating with the lab.


LAB:
Responsibilities of the lab volunteer include:
Draw blood
Administer vaccines
Lab organization and restocking
**All lab tests will be performed under the supervision of the lab coordinator. Training or review will be
provided and conducted by the lab coordinator.
