I have purchased acer switch 10 model bearing the seal number LITMX253179C ,

on 9th September 2015 at reliance digital limited.The invoice number of cash bill is
(52).The is purchased at a cost 29,990.
At purchase itself I was given the national insurance company certificate which
covers theft and accidental damage certificate. The certificate valid up to 9th
September 2016.
Unfortunately while working with the laptop in home address: P2 Andhra university
Quarters Shivaji Palem, Opp. Rama Laxmi Apartments, Visakhapatnam.At 9 o'clock
in the evening on 15th April 2016.The laptop has fallen on floor and damage took
place .It is noticed that the screen is damaged. Immediately I brought it to the
notice to insurance company on 16th April 2016 ,for possible disbursement of the
claim.
As per the directions of the insurance I got it to the authorized service center
charged an amount of Rs 13000/- in anticipation of the claim I paid the amount to
the authorized service center through cash bill number.
