Duties and Responsibilities:
Actual duties and responsibilities in the Unit (Ward, SCU, AS/ER)
1. ADMISSION of patient
a. Receiving admitted patients from AS/ER & SCU
b. Checking the IVF for patency, accuracy (IV tag) & IVF level
c. Assessing the general condition of the patient
d. Checking the identification of patient (patient tag)
e. Promoting safety of patient (use of side rails/restraints)
f. Instructing patients watcher regarding hospital policies and doctors orders.
g. Carry out doctors order
g.1. Checking and signing of doctors order
g.2. Transcription in the Kardex
g.3. Preparation of medication card
g.4. Preparation and administration of medication & treatment (observe 10 rights)
g.4.1. Oral
g.4.2. Injection (IV or IM)
g.4.3. Venoclysis/blood transfusion
g.5. Preparation of patients for special procedures
g.6. Arrangement for ancillary/diagnostic/therapeutic procedures.
h. Assistance to doctors
h.1. During rounds
h.2. During procedures
i. Forwarding prescriptions/Physician Drug Order (PDO) form
j. Collecting of specimen (urine and stool)
k. Conduct health teachings
l. Taking and recording of vital signs, intake and output
m. Insertion and reinsertion of IVF (IV Therapist)
n. Performs chest physiotherapy and turning of patient (bed ridden patient)
o. Performs suctioning procedures
p. Administers osterized feeding
q. Documentation/charting of nursing care
r. Preparation for the discharge of patient
r.1. Prepares clearance slip
r.2. Prepares home instruction form
r.3. Forwarding of turn in slip (excess medicines & IVF)
r.4. Conduct discharge instruction
