SYNTHESIS

For 6 days duty, we were assigned at the Upper Arroyo, Matina Pangi, Davao City
for our community health nursing under Mr, Leonardo Molina R.N., M.N. We were
assigned to different patients with fluid and electrolyte imbalances. The patient we chose
was Al.P., a 54 yr old male diagnosed with diabetes mellitus type 2. Mr. Al.P.s family is a
nuclear type of family. He is married to Mrs. An.P. and has only one child, S.P., who is
currently in attending at Emar Learning Centre as a grade 7 student. The couple both
decides for the family and fortunately, the couple have no signs or presence of conflicts or
problems.
Their house is really spacious for the three of them. The parents and the child have
their own respective rooms. Their house is made mostly of wood which makes it prone to
fire hazard. Located outside their house is their sari-sari store which sells different goods
and even viands/meals. They have loose wirings around the house and a toilet facility
located outside their house. They also have a motorcycle that they use whenever they go
out. Their drainage system consists of a pipe that leads to the river while their garbage is
collected twice and is not segregated.
The couple is really concerned when it comes to their health status. Mr Al.P. was
diagnosed with diabetes at the age of 47. Mr Al.P. strictly drinks his maintenance medicine
daily. The immunisation status of their son is still incomplete even though he is already 12
yrs old. He still lacks a Hepa B vaccine until now. They use the health centre in their
community whenever health problems arise.
Health threats are present within the vicinity of their home. Their kitchen uses fire,
gasoline or charcoal for cooking with smokes that can cause respiratory problems.
Different breeding sites and vectors are present inside their house. Health teachings about
proper garbage segregation, pest control, proper hygiene, compliance of medicine, proper
diet and exercise were given to the family during the time of interview. The family agreed
to follow the given health teachings given to them.
We have performed physical assessment, traced their venogram and obtained
informations about their family history. The possible occurrence of the disease is acquired
to the gene pool of the mother of Mr. Al.P. The group identified actual and potential health
problems of the family, enumerated the drugs that was given to the patient, reviewed the
anatomy and physiology of the affected organs and system, comprehended and traced the
pathophysiology, evaluated the effectiveness of the interventions performed and the care
rendered, recognized the course in the community and appropriate nursing interventions to

be done, provided a nursing theory and recommendations regarding with patients case,
and evaluated the objectives of the study in the end.
