Submission of Certified True Copies of School Sites'

Ownership Titles of Public Elementary and Secondary Schoots

April 13,2016

The Sites Titling Office (STO) is requesting the submission of the School
Sites' Ownership Titles which include Survey Plans and Cadastral Maps for updating
and lmproving their database.
These ownership of titles may either be under the following:

1. Original Certificate of Title (OCT)

2. Transfer Certificate of Title (TCT)
3. Special Patents

4.

Presidential Proclamations

5. Tax Declarations

6. Deed of Donations (DODs)

7. Deed of Sale
8. Usufruct Agreements (UA)
9. Expropriation; and
10. Other similar documents

The foregoing documents must be clear, legible and mar:ked.as Gertified

True Copies by the appropriate issuing agency (i.e. LRA, Assessor's Office).

The Division Physical Facilities Coordinator, Division Engineer and Legal

Officer are tasked to facilitate the consolidation of the abovementioned documents
and the same be submitted to this Office (Attention: Legal Secfion) on or before
April 25,2016.
Further, the documents must be submitted inside a short white folder (wellfastened), see illustration bel'ow.
