1.0 PROJECT SCOPE, REQUIREMENTS AND OCCUPATIONAL HEALTH AND SAFETY


OBJECTIVES
1.1 Scope of Works
The site work activities which will require the oversight by a LFD GENCON, INC.
Engineer include the following scope and will include the completion of:

Installation of an External Partition and Scaffolding for the entrance of workers in


terms of Safety, Quality and Delivery
Conversion and improvisation of the existing Dryer 1 Control Room into a new Dryer
1 & 2 PPU Office and Testing Room in terms of Safety, Quality and Delivery
Installation of Internal Partitions to be use for isolation of D1 and D2 in Safety,
Quality and Delivery

Details of the scopes of work to be completed in each of the work areas for this
project are provided within the CSA Project No. 1801.10 Plan.
1.2 Occupational Health and Safety Objectives
This Contractors Safety Implementation Plan (CSIP) addresses foreseeable
activities associated with the site work activities to be conducted at WYETH Nutrition,
Cabuyao, Laguna. This CSIP establishes personnel protection standards and mandatory
safety practices and procedures.
LFD GENCON, INC. personnel involved with inspection of site work activities
shall comply with the requirements of this CSIP. All LFD GENCON, INC. personnel
engaged in onsite activities will read this document carefully. Contractors and
subcontractors conducting construction-related activities are solely responsible for their
own workers Health and Safety and providing a safe working environment in accordance
with all applicable federal, state and local requirements. Each Subcontractor will have a
designated Site Health and Safety Coordinator who will be responsible for ensuring that
the designated procedures are implemented in the field. Personnel who have any
questions or concerns regarding implementation of this plan are encouraged to request
clarification from the LFD GENCON, INC. Project Manager. Field personnel must follow
the designated health and safety procedures, be alert to the hazards associated with
working close to vehicles and equipment, and use common sense and exercise
reasonable caution at all times.
The level of protection and the procedures specified in this CSIP represent the
minimum health and safety requirements to be observed by site personnel engaged in
the referenced inspection of construction related activities. In the event of an
immediately dangerous or life threatening situation, the employee always has "stop
work" authority.
THE ULTIMATE RESPONSIBILITY FOR THE HEALTH AND SAFETY OF THE
INDIVIDUAL EMPLOYEE RESTS WITH THE EMPLOYEE AND HIS OR HER
COLLEAGUES. Each employee is responsible for exercising the utmost care and good

judgment in protecting his or her own health and safety and that of fellow employees.
Should any employee observe a potentially unsafe condition or situation, it is the
responsibility of that employee to immediately bring the observed condition to the
attention of the appropriate health and safety personnel as designated above and to
follow-up the verbal notification. "Extenuating" circumstances such as budget or time
constraints, equipment breakdown, changing or unexpected conditions, never justify
unsafe work practices or procedures. In fact, the opposite is true. Under stressful
circumstances all project personnel must be mindful of the potential to consciously or
unconsciously compromise health and safety standards, and be especially safety
conscious. ALL SITE PERSONNEL ARE EXPECTED TO CONSIDER "SAFETY FIRST"
AT ALL TIMES.
2.0 OCCUPATIONAL HEALTH AND SAFETY POLICY
An uncompromising commitment to safety, environmental and regulatory compliance
from LFD GENCON, Inc. management and each employee to be assigned to WYETH Nutrition
Philippines project is the key to achieving our mission and goal.
All employees have the right and responsibility to immediately halt and report unsafe or
non-compliant equipment, acts or practices. LFD GENCON, Inc. Management support of this
policy is total and absolute.
LFD GENCON, Inc. strives to keep all employees aware of required safety, health and
environmental procedures and provides the resources for each employee to ensure his/her
safety.
Everyone is responsible for their own safety and for the safety of those around them.
We will strive to meet or exceed WYETH Nutrition Philippines safety regulations. A measure of
performance and commitment to safety standards will be an integral part of every employees
job and performance evaluation.
The Occupational Health and Safety Policy apply to all employees, temporary
employees, contractors, sub-contractors and members of the public who are or may be affected
by our activities. It is the policy of the Company to provide and maintain, as far as reasonably
practicable, a working environment that is safe and without risk to health. LFD GENCON aims to
achieve Zero Harm through effective management of health and safety. This includes the
provision of:

Safe and healthy working conditions;


Arrangements for the operation, design and maintenance of safe systems of work;
Properly maintained and guarded machinery;
Information, instruction, training and supervision appropriate to the Companys activities;
Implementation of emergency procedures evacuation in case of fire or other significant
incident; and
Arrangements for regular engagement and consultation with employees on day-to-day
health and safety conditions and provide advice and supervision on occupational health.

The Company will communicate the Policy to its employees, contractors and suppliers to
ensure that they are aware of the Companys commitments and their responsibilities. The

Company will provide staff training to promote good practice as well as awareness of the
policys requirements. The Company expects all its contractors to take similar action.
2.1 Provisioned Uniforms
All employees and workers are required to wear uniforms provisioned by Wyeth
Nutrition. If employees clothing is do not conformed to the prescribed uniform, the
Safety Officer / Supervisor will instruct the employees to change the clothing before they
will be allowed to enter in the plant. Appropriate clothing must be worn at all times by all
workers/employees.
3.0 CONTROL OF OCCUPATIONAL HEALTH AND SAFETY PLAN
The Company requires all employees to recognize their own individual responsibility
regarding occupational health and safety and to comply with all health and safety requirements
relevant to their activities, including but not limited to:

Following established standard operation procedures, guidelines and instructions;


Where appropriate always wearing their Personal Protective Equipment (PPE) during
working hours;
Accurately reporting all incidents, accidents and hazards in a timely fashion and in line
with Company procedure;
Not beginning any task without first considering the health and safety aspects;
Not taking unnecessary risks which could affect their health or wellbeing, or that of any
other person; and
Asking questions of their supervisor when unsure of the safety requirements.

5.0 SAFETY ORGANIZATION AND RESPONSIBILITIES
This section specifies the LFD GENCON, INC. Project Organization. LFD GENCON, Inc.
philosophy of safety Management is that each employee has the key role in maintaining an
incident free workplace. This is accomplished through training and awareness.
5.1 Project Manager
Provides review of all site activities and directs site activities via the Site Safety Officer
reports to Client. Has authority to direct response operations.
Responsibilities:

Obtains permission for site access and coordinates activities with appropriate
officials.
Ensures that the Gantt chart is executed and on schedule.

Briefs the field team on their specific assignments.


Coordinates with the Site Safety Officer (SSO) to ensure that health and safety
requirements are met.
Prepares the final report and support files on the response activities.
Assists SSO in conducting Jobsite Safety Inspections and assists with the
correction of shortcomings found.
Ensures training requirements are satisfied in a timely manner.
Ensures medical evaluations of LFD GENCON, INC. personnel are current.
Maintains all records on personnel (medical evaluation results, training and
certifications, accident investigation results, etc.).
Prepare any Root Cause Investigation Reports/Preventative Action Plans for any
incidents and or Close Calls.

5.2 Site Safety Officer


Advises the Project Manager on all aspects of health and safety on site. Stops work if
any operation threatens worker or public health or safety.
Responsibilities:

Manages field operations.


Executes the Plans and schedule.
Enforces safety procedures.
Enforces site control.
Documents field activities and sample collection.
Periodically inspects protective clothing and equipment.
Ensures that protective clothing and equipment are properly stored and
maintained.
Controls entry and exit at the Access Control points.
Knows emergency procedures, evacuation routes, and the telephone numbers of
the ambulance, local hospital, poison control center, fire department, and police
department.
Coordinates emergency medical care.
Controls the decontamination of all equipment, personnel, and samples from the
contaminated areas.
Ensures that all required equipment is available.
Advises medical personnel of potential exposures and consequences.
Notifies emergency response personnel by telephone or radio in the event of an
emergency.
Assist in the preparation of all Root Cause Investigation Reports/ Preventative
Action Plans for any incidents and or Close Calls.

A Safety Officer will be assigned who shall coordinate safety matters and acts as
liaison between LFD GENCON, INC. Management and WYETH Nutrition Philippines
Project management, Safety and Security Team. He will also conduct safety training and
awareness among all LFD GENCON, Inc. employees.

A safety committee will be established which will be headed by the Safety Officer
and all supervisors and leadsman will be member. Their charter is to promote safety and
security through communications and awareness.
6.0 SUBCONTRACTOR OCCUPATIONAL HEALTH AND SAFETY
The objective of LFD GENCON Subcontractor Safety Plan is to achieve and maintain
the highest possible level of safety performance for all Subcontractor personnel working on LFD
managed projects. The success of this objective requires the commitment and dedication of
every worker to his own personal safety and the safety of fellow workers. All Subcontractors and
workers are to become thoroughly familiar with the contents of LFD GENCONs Company
Safety Program and use it for discussion at project and regular safety meetings. With the
cooperation of all, LFD GENCON and Subcontractors will succeed in improving safety on the
job.
6.1 General Safety Requirements

Subcontractors must report to the Power Project Team any safety concerns,
observed conditions or violations of job safety, regardless of whether they are
within the observer's power or responsibility to correct.
Subcontractors shall assure that supervisory employees have a working
knowledge of applicable safety requirements as they pertain to their areas, and
encourage all employees to improve their accident prevention awareness.
All subcontractors must be familiar and comply at all times with the requirement
of LFD's Company Safety Program and must ensure that they and their workers
have the skill and ability to perform the work;

6.1 Subcontractor Compliance


Subcontractors shall hold each of their agents, vendors, tier subcontractors and
suppliers responsible for compliance with these safety requirements. Subcontractors
shall include these safety requirements in contracts with all tier subcontractors, vendors
and suppliers. Entry onto project, property, or the job site constitutes acknowledgement
by the subcontractor, subcontractor employees or invitee of their obligation to adhere to
these safety requirements. Each subcontractor shall establish and maintain an effective
safety and health program that addresses the requirements herein. The subcontractor
shall be solely responsible for implementing the safety program and shall have sole
responsibility for monitoring the work of its employees, subcontractors, agents, vendors
and suppliers to ensure compliance.
6.3 Subcontractor Safety Representation
The subcontractors designated Safety Representative is expected to make a
jobsite visits to audit implementation of the subcontractors safety and health plan and
the safety requirements. This representative will also be required to attend regular on
site safety meetings, as determined by LFD. Additional onsite safety and health staffing
will be defined in the individual trade scopes. The subcontractor will provide a copy of all
documented inspections, observations, or reports to LFD.

Use good judgment and experience to anticipate and identify job hazards in the
planning stage of all job assignments;
Ensure that all workers are aware of their rights and responsibilities
Ensure that proper equipment is available to do the job safely;
Stop work being performed in an unsafe manner;
Report all incidents/accidents to LFD as soon as possible, and conduct or
participate in the investigation of all serious accidents on their projects including
preparing an incident/accident investigation report and providing LFD with a copy
of that report;
Attend and participate in safety start-up meetings; and
Complete daily worksite hazard assessments for scope of work.

7.0 METHOD STATEMENTS/CONSTRUCTION METHODOLOGY


7.1 SCAFFOLDING
Scaffolds shall be erected, moved, dismantled, or altered only under the supervision
of a competent person and will have guardrails and toeboards installed. When scaffolding
hazards exist that cannot be eliminated, then engineering practices, administrative
practices, safe work practices, Personal Protective Equipment (PPE), and proper training
regarding Scaffolds will be implemented. These measures will be implemented to minimize
those hazards to ensure the safety of employees and the public.
7.1.1 Scaffolding Safety Policy
To insure safety and serviceability the following general precautions concerning the
care and use of Scaffolding will be observed:

The maximum work level height will not exceed four (4) times the minimum or
least base dimensions of any mobile scaffold. Where the basic mobile unit does
not meet this requirement, suitable outrigger frames will be employed to achieve
this least base dimension, or provisions will be made to guy or brace the unit
against tipping.

Only trained and authorized employees of this company will supervise the erection of

The three main areas of inspection are for rust, straightness of members, and welds.
Only trained employees of this company will conduct the inspection. The following as a
minimum apply:
7.1.4

Rust

Heavily rusted scaffolding equipment is a possible sign of abuse or neglect. Severely


rusted components should be thoroughly inspected and cleaned before approved for
use.
7.1.5

Straightness of members.
Mishandling, trucking and storing may cause damage to scaffolding
equipment. All members or parts of all steel scaffolding components should be
straight and free from bends, kinks or dents.

7.1.6

Welds
Scaffolding equipment should be checked before use for damaged welds and
any piece of equipment showing damaged welds or rewelding beyond the original
factory weld should not be used. The factory weld reference pertains to location and
quality of rewelds.

Check serviceability of locking devices.

Check alignment of coupling pins and braces.

8.0 RISK ASSESSMENTS AND JOB HAZARD ANALYSIS


This section presents all assessment of the general, chemical, physical and biological
hazards that may be encountered during the tasks specified under this CSIP.
8.1 General Hazard Assessment
A general hazard assessment was conducted for the required field work and the
following potential hazards have been identified:
Physical hazards associated with the use of heavy equipment;
Tripping hazards;
Noise exposure;
Heat stress (depending on weather conditions);
Electrical hazards; and
Use of personal protective equipment
Mitigation and controls will include as needed work procedures, work/rest regiment,
dust control measures, personal protective equipment, and respiratory protection as
appropriate.
8.2 Physical Hazards

8.2.1 Temperature Extremes


Hot Temperatures Heat stress is a significant potential hazard, which is greatly
exacerbated with the use of PPE, in hot environments. The potential hazards of working in
hot environments include dehydration, cramps, heat rash, heat exhaustion, and heat stroke.
If onsite workers exhibit the signs of heat exhaustion or heat stroke, they should seek
immediate medical attention.
8.2.2 Noise and Air Resources
Noise is a potential hazard associated with the operation of heavy equipment, power
tools, pumps and generators. Hearing protection is required and shall be used in designated
areas of the site as indicated by the posted signs.
8.2.3 Hand and Power Tools
In order to complete the various tasks for the project, personnel will utilize hand and
power tools. The use of hand and power tools can present a variety of hazards, including
physical harm from being struck by flying objects, being cut or struck by the tool, fire, and
electrocution. Proper personal protective equipment shall be worn while utilizing hand and
power tools.
8.2.4 Slips, Trips, and Falls
Working in and around the site will pose slip, trip and fall hazards due to equipment,
piping, slippery surfaces that may be oil covered, or from surfaces that are wet from rain or
ice. Potential adverse health effects include falling to the ground and becoming injured or
twisting an ankle. Good housekeeping at the site must be maintained at all times.
8.2.5 Material Handling
Manual lifting of heavy objects may be required. Failure to follow proper lifting
techniques can result in back injuries and strains. Back injuries are a serious concern as
they are the most common workplace injury, often resulting in lost or restricted work time,
and long treatment and recovery periods. Whenever possible, heavy objects must be lifted
and moved by mechanical devices rather than by manual effort. The mechanical devices will
be appropriate for the lifting or moving task and will be operated only by trained and
authorized personnel. Objects that require special handling or rigging will only be moved
under the guidance of a person who has been specifically trained to move such objects,
such as a Master Rigger or equivalent. Lifting devices, including equipment, slings, ropes,
chains, and straps, will be inspected, certified, and labeled to confirm their weight capacities.
Defective equipment will be taken out of service immediately and repaired or destroyed.
All reciprocating, rotating, or other moving parts will be guarded at all times.
Accessible fire extinguishers will be made available in all mechanical lifting devices. All
material must be stored in tiers, racked, blocked, or otherwise secure to prevent sliding,
falling, or collapse. All loads/material will be verified to be secure before transportation.
8.2.6 Electrical Safety

Although not anticipated, personnel may utilize hand and power tools. The use of
hand and power tools can present a variety of hazards, including physical harm from being
struck by flying objects, being cut or struck by the tool, fire, and electrocution.
9.0 NON-CONFORMANCE, CORRECTIVE AND PREVENTATIVE ACTION
9.1 Emergency Preparedness
Being prepared for emergencies means making sure that the necessary equipment
and supplies are readily available and that employees shall be trained and know what to
when something unplanned happens such as release, spill, electrocution, fire or injury.
Employees will be provided with written procedure to have the opportunity to practice
emergency response skills regularly.
9.1.1 Emergency Site Evacuation Routes and Procedures
All project personnel will be instructed on proper emergency response procedures
and locations of emergency telephone numbers during the initial site safety meeting. If an
emergency occurs as a result of the site investigation activities, including but not limited to
fire, explosion or significant release of toxic gas into the atmosphere, the Langan Project
Manager will be verbally notified immediately. All heavy equipment will be shut down and all
personnel will evacuate the work areas and assemble at the nearest intersection to be
accounted for and to receive further instructions.
9.2 Fire Prevention and Protection
In the event of a fire or explosion, procedures will include immediately evacuating the
site and notification of the LFD GENCON Project Manager of the investigation activities.
Portable fire extinguishers will be provided at the work zone. The extinguishers located in
the various locations should also be identified prior to the start of work. No personnel will
fight a fire beyond the stage where it can be put out with a portable extinguisher (incipient
stage).
10.0 INCIDENT REPORTING
Once first aid and/or emergency response needs have been met, the following parties
are to be contacted:

The employer of any injured worker who is not a LFD GENCON employee for
emergencies involving personal injury and/or exposure including near misses, the SSO or
designee will complete and submit an Incident Report form within 48 hours. If the employee
involved is not a LFD GENCON employee, his employer shall receive a copy of the report.

The general contractor/construction manager shall conduct and document regular safety
inspections (minimum of informal daily and formal weekly) of their work areas and practices,
and those of their subcontractors. Documentation of inspections shall be readily available for
review on the project site. The project manager/supervisor will immediately correct any
hazardous or otherwise noncompliant conditions identified and maintain documentation of the
corrective action. In addition to the regular inspections indicated above, a documented audit
conducted by corporate or safety representatives shall be done at least once a week.
12.0 TRAINING
12.1 General Health and Safety Training
With LFD GENCON, Inc. corporate policy, site workers shall, at the time of job
assignment, have received an initial health and safety training for operations unless
otherwise noted in the above reference. At a minimum, the training shall have consisted of
instruction in the topics outlined in the standard. Personnel who have not satisfied the
requirements for initial training shall not be allowed to work in any site activities.
12.2 Site-Specific Training
Prior to commencement of site activities, all field personnel assigned to the project
will have completed training that will specifically address the activities, procedures,
monitoring, and equipment used in the site operations. Should any new employees arrive
on-site, they will also be given a documented full review or one that address the
appropriate tasks that remain at the time of the new employees arrival.
12.3 Hazard/Safety Training Programs

12.3.1 Electrical Safety Related Work Practices

Before working with electricity, all employees will be trained and/or demonstrate
their proficiency in safely working around electricity and electrical devices. Only authorized
electrician will be allowed to work on any temporary electrical power connections.

12.3.2 Fall Protection / Ladder Safety

All employees who work in elevated areas or use ladders will be trained and
instructed in fall protection. This includes how to safely work under those conditions and how
to inspect the equipment.

12.3.3 Industrial Vehicle / Driver Safety

All employees who will operate handling equipment such as forklift, trucks or
company vehicles will be trained in pre-orientation inspections and the safe use of such
vehicles.

12.3.4 Fire Extinguisher

Fire extinguisher training is provided for all employees. All personnel will be
trained in the basic operation of the fire extinguishers at the facility.

12.3.5 Personal Protective Equipment

All LFD GENCON, Inc. employees utilize personal protective equipment (PPE)
(e.g. protective clothing such as gloves, face shields, eye protection, and safety shoes) at
one time and another, the policy of LFD GENCON, Inc. to provide with the required PPE and
to train all personnel on its use prescription safety glasses, if required, are provided. Safety
shoes and Plant approved shoes are supplied according to location policy. The Safety
Officer and Supervisor can answer any questions each personnel may have on personal
protective equipment.

12.3.6 Lockout/Tagout

Employees who service, maintain, or install equipment will be trained in


lockout/tag-out procedures. These are the only employees authorized to lockout or tagged
out; it cannot be started except by the employee who locked it out.

All other employees need to be aware that if a piece of equipment is shut off and
locked out or tagged out, it cannot be started except by the employee who locked it out.

It is the policy of LFD GENCON INC. to monitor both the hazard and the control
method to make sure that the control is working effectively and that exposure to the hazard is
reduced or eliminated.

The management shall conduct inspection, testing, exposure assessment,


observations, injury or illness tracking, employee feedback/input, occupational health
assessment and other method.

A comprehensive review shall identify areas that may need more improvement or
refinement. It is important to know if all workplace hazards are identified, assessed and
appropriately controlled. Communication about the hazards and how to control them must be
done for all employees.

14.0 SAFETY MEETINGS

Project personnel and visitors will participate in documented daily on-site health
and safety briefings led by the Safety Security Officer to assist site personnel in safely
conducting their work activities. The briefings will include information on operations to be
conducted that shift, changes in work practices or changes in the site's environmental
conditions, as well as periodic reinforcement of previously discussed topics. The briefings will
also provide a forum to facilitate conformance with safety requirements and to identify
performance deficiencies related to safety during daily activities or as a result of safety
inspections. The meetings will also be an opportunity for the work crews to be updated on
monitoring results. Prior to starting any new activity, a training session will be held for crew
members involved in the activity.
