I. Waskita Policy

Waskita Policy
PT Waskita Karya (Persero) Tbk as the Agency for Construction Services always
control the risk to Safety - Occupational Health, Environment, Quality and
Security Management System by implementing Waskita to the satisfaction of
stakeholders.
As part of this commitment, management always:
1. Comply with laws and regulations and other applicable requirements.
2. Improving performance on an ongoing basis.
3. Prevent injury, occupational illness, environmental pollution and the
occurrence of security incidents that affect the company's business
processes.
4. Providing training, providing a healthy and working facilities, safe and
convenient to all stakeholders.

II. Executive Summary


This report records the Health and Safety activities of Waskita Karya (Persero) Tbk. Especially
Unpgrading of Existing Suai Airport Project for the months of December 2014.

III.General
In carrying out a job, health and safety issues are important factors that should be the
primary concern of all parties. Our success in executing the work not only measured from
the completion of the job. Many things can be used as a parameter assessment of the
success of a job. The work was considered successful if the health and safety of all existing
resources is assured, can be completed on time or even earlier than the time specified,
provide benefits to the company, to give satisfaction to all parties (management, employees
and employers).
Health and safety issues are very important, since the establishment of health and safety
at work means can reduce operating costs. If in performing the work of an accident, it will
increase the cost of expenditure, which finally reduces the profits of the company. In the
case of a severe accident, the losses are not only related to financial aspects (funding), but it
can cause defects in the worker or even death.
Construction work contains a lot of potential to pose a risk of occupational accidents,
occupational diseases, environmental pollution, and the discrepancy in the quality of work.
And not least in the Upgrading of Existing Project Suai Airport.

Health Performance
a. Complaining of Disease
Increased rainfall in the December can increase the risk of the disease.
complaining of pain that will increase still around influenza, cought and
fatigue. medications has been available on site.
b. Occupational Disease
There is no Occupational diseases during PT. Waskita Karya (Persero) Tbk.
carrying out construction and campaigns on the prevention and control will
continue to be gradual when the health and safety talk.
c. Referral Case
There is no referral case on December 2014

VII. Safety Performance


a. Man power
Total Waskita Karya (Persero) Tbk. There is no increased from the
previous period as many as 26 people. PT. WTJ (Subcontractor) with rental
equipment, additional manpower has increase 4 personell for this month.
Especially in the foreman as a whole will continue to increase with the
progress of the work. Total employees working in Waskita Suai Airport
Project site as many as 144 people overall.

b. Man Hours
The number of manhours until December 15th 2014 amounted to
24.290 hours of work and working hours Increase compared to the previous
period. due to along with increasing intensity of work on site and there is no
time lost working hours due to an accident on this month, total cumulative
hours of work until now of 104.954 hours and no Loss Time Injury and
fatality (Start from May 1st). but there were incidents were not caused from
the job (riot), which cause the victim does not want to work anymore

There is 1 incidents on this month. No victim. This Project is still
ZERO ACCIDENT fatality.

Occupational Health and Safety Performance

* Incident rate becomes large because the number of working hours in use only 8 months

d. Hazard report
Based on the report hazards recorded in HSE unit on December period,
there is no hazards findings occurred in the area of basecamp . From the last
month finding of hazard, the corrective actions taken are still in progress and
late. because majority of worker are focus to erection batching plant and
AMP. For inspection results classified into 2 types of hazards, unsafe action
and unsafe condition. From the results of the inspection period December
( Nov 24 - Dec 15) resulted in findings.

Inspection activities in December 2014 carried out at the airport and
the basecamp area. Every inspection finding will be record on CAR document
(Corrective & Preventive Action Request) and will be signed & follow up by
the person in charge of the area.

h. Flight Report

Suai Airport Flight Report.

Flight report started in September, and in December there is 1 flight


in Suai. No flight incidents related to existing work in the field. Take off and
Landing went smoothly without distubed the work. To avoid the accident in
the airside area, will be applied siren system if there is about flight to Suai
Airport. So before the aircraft will be landing, the run way area will clearance
from all activity and equipment.

Clearance area for


emergency landing and Take
off minimum 700 meters

1. Clearance Area along the "Runway", which counted 60 meters from the

center line of the width of "Runway". This area should be completely free
from all outside activities on the flight activity of the airport operating hours.
2. If there is a plane that will land, the contractor must ensure the runway is
free of any obstacles (clearance area), at least 700 meters.

Existing runway lenght remaining 829m due to demolished work, so


clearance area are still on safe distance for emergency landing and take off .

HSE meeting
HSE meeting is an activity to remind workers of the importance of
safety in the workplace, hazards in the workplace, the incidents that occurred
in the workplace and safety related issues. In this case we usually call safety
morning. HSE meeting attended by all personnel in the project, and also
subcon foreman. in the November has been done 2 times a safety morning.

LANDSLIDE INCIDENT

Chronology :
On Thursday afternoon at 14:00 happen very heavy rain in the area basecamp. Because of
heavy rain, the production process is stopped and workers in the field stopped. After 1 hour
the rain, suddenly, gabion retaining not strong enough to hold the load increase due to
rainwater and finally collapse on the side of the house generators. Landslides are not get to
generator house, no work activity at the site, and none of the victims.

Conclusion :
A. At the time of manufacture of is not done with vibro compaction, only by using the
excavator, so it is not in getting the maximum density to hold the load.
B. Gabion retaining the height > 5m only installed one layer and a width of 1 m, so that
the strength is not maximum.
C. In the event of heavy rain, the water that is in the top pile into the soil compacting
gravel are less than the maximum. Thereby increasing the pressure on the gabion.
And finally gabion not able to hold the pressure.
Corrective Action :
A. The access road will be repaired, and its design will a little away from the Electrical
transmission tower.
B. At the end of the access road will be installed gabion Minimum 2 layers.
C. Most of the embankment will be made the slope, so that the water in the top can
sliding down.

There were no victims in the incident, and there was no damage to property, it's just we lost
production process for 7 days for a period of repair.
