Miss Vita is a 20 years old patient, come to the hospital accompanied by her Sister.
She come to the hospital with a symptoms which the problem has about her pain on her
fracture trace in the right upper arm since three weeks ago after she got an accident. She
didnt go to the hospital when she got the accident three weeks ago, she just come to the
massage therapist to solve her problem, she has been drinking some medicine to reduce her
pain, but it doesnt work at all. She just lies down on her bed with grimacing face and holding
her arm. In the afternoon her condition was getting worst so that she asked her sister to
accompany her to come to the hospital.
Miss Vita and his Sister arrived in the hospital and went to the receptionist. But
unfortunately they didnt know about the direction to the registration place because it was too
many change with that place since two years ago, so that they asked to the nurse that walks
over them.
Sister

: Excuse me nurse, may I ask you about direction?

Nurse2

: Yeah of course, where do you want to go miss?

Sister

: I want to go to the registration place, but I dont know where.

Nurse2
: Oh sure, first you should go a straight, then you will find the intersection,
then you turn right. The registration place was on your left side, beside the emergency room.
Is that clear miss?
Sister

: Ok, Its clear enough. Thanks for your help.

Nurse2

: Ok.
After they arrived in the registration place, her sister admitting Miss Vita and make an

appointment to her doctor. She asks Miss Vita to wait for her in the waiting room.
Receptionist

: Hello good afternoon, may I help you?

Sister

: good afternoon, I want to see dr. Elisa for my sister today.

Receptionist

: Do you have an appointment with her?

Sister

: No, but usually today is her schedule to practice in this hospital right?

Receptionist

: Im sorry, but dr. Elisa schedule is completely booked for today.

Sister

: Ok, no problem, actually I want to admit my sister here.

Receptionist

: Ok miss, is she has been going or hospitalized in here before?

Sister

: Yes I think, but it was two years ago.

Receptionist

: That is no problem, I will check her data. Sorry miss, may I know her

medical record number?

Sister

: Yes sure, it is 240-95-07

Receptionist

: I will check, just wait in a view minute.

The Receptionist has been checking the data of his sister.

Receptionist

: I got it, let me compare on you, may I?

Sister

: Of course.

Receptionist

: Her name is Miss Vita Agustin Erlyana, is that right?

Sister

: Yes, with v actually.

Receptionist

: Ok, and her home address?

Sister

: Graha Prasetya street number 73 Semarang.

Receptionist

: And her phone number?

Sister

: it 081-229-253-272.

Receptionist

: Very well, you may take your queue number and we will call you according

to the number, you may wait on the waiting room, thank you.
After waiting in a view minutes, the receptionist call her queue number and invite her
to consulting room, to meet the nurse.
Receptionist

: Miss. Vita Agustin.

Miss Vita

: Yes me.

Receptionist

: This is your turn.

Miss Vita

: Ok, thanks.

Miss Vita has been admitted for investigation to find out more about the pain
promblem on her fracture trace. The nurse has about to examine her complain and check her
condition.
Nurse

: Hello, good afternoon Im Nurse Fifi that will take care of you today.

Miss Vita

: Good afternoon nurse fifi.

Nurse

: Ok, Is that true with Miss Vita Agustin Erlyana, 20 years old?

Miss Vita

: Yes, Im.

Nurse

: Ok miss, may I take your time for we talk about your feeling now and check

your condition?, it wont be long maybe just 10 until 15 minutes.

Miss Vita

: Hmm oke, of course.

Nurse

: How are you this morning, you look so painful, is it something wrong.

Miss Vita

: Actually Im not sure with my condition, its so annoying.

Nurse

: Ok Miss, before we continue, may I take your time for we talk about your

feeling now and check your condition, it wont be long maybe just 10 until 15 minutes.
Miss Vita

: Ok Nurse, but how should I start?

Nurse

: Now just a little about why youve come into hospital. Can you tell me why

you have been admitted today?

Miss Vita

: Well, Ive had so much trouble with my fracture trace. And its not clear

why. Youre going to run some examination I think, and see if you can find out whats going
on.
Nurse

: Thats right. So tell me a little bit more about the problem.

Miss Vita

: Well, its been going on for several weeks now, and after I hit by a car my

right upper arm was injured, but it got a lot worse this morning, I thought Id better do
something about it, so that I went here.
Nurse

: Yes, youre doing the right thing, and did you get any medicine before you

went here miss?

Miss Vita

: Hmm, I dont know but my family just brought me to the massage therapist

to solve the problem and thats not too bad I guess. And when the pain is relapse usually I
used to take asam mefenamat to reduce the pain.
Nurse

: So what kind of symptoms do you get, Miss Vita?

Miss Vita

: Well, you know, quite a lot of pain, especially after Ive did some

movement, I feel like there is a needle that punctured, so that I cant do more activity as
usual.
Nurse

: a needle that punctured? Hmm did you hitting from something after your

better condition before?

Miss Vita

: No, not at all Im very preserve my fracture trace.

Nurse

: Well, so If I give you a scale from 1 until 10, how much you rate your

pain?
Miss Vita

: It will be on 6 scale nurse.

Nurse

: Ok, do you mind If I see your fracture trace on your right upper arm?

Miss Vita

: No, not at all, but softly please.

Nurse

: Of course, Hmm you said that you cant do more activity,so how about your

daily activity?
Miss Vita

: Surely,I was helped by my family.

Nurse

: So how about taking a bath? You helped to?

Miss Vita

: Yes, my family also helped me to clean my body and helped me when Im

Nurse

: Hmm, and for your daily, are you just stay on your bed?

Miss Vita
: Actually no, I can walk as usual, but for did some activity with hand I use to
be done with my left hand and if I cant I just asking for help.

Nurse
: Ok I see, do you have any problem with your nutrition? And how much you
eat in each day?
Miss Vita

: No, Im eating three times a day as usual, and I eat up all my portion of the

: Oh good, do you have any problem for breathing?

Miss Vita

: no, my breathing is normal.

Nurse

: And then how about communication with others? Do you have any

Miss Vita

: no, I feel comfort to talk with other people

Nurse 2

: Ok good, so do you feel comfort to sleep and take a rest miss?

Miss Vita
: I felt difficult to sleep because of my pain, sometimes it punctuated me
when I just sleep.
Nurse 2

: Oh, and then how about your religion activity?

Miss Vita

: I just pray as usual but for some movement that I cant, I just do it as I can.

Nurse

: Ok. Ill just write that down. Now Ill take your blood pressure, body

temperature, respiratory rate, and your pulse, may I?

Miss Vita

: Oh yeah, sure.

Nurse has about to take a blood pressure, body temperature, respiratory rate, and pulse
of the Miss Vita.
Nurse

: Well, you got 110/80 mmHg in a blood pressure, 22 rate per minutes, 37,2

C body temperature, and 57 beat per minutes.

Miss Vita

: Thank you nurse, am I well at all?

Nurse

: Yes, your vital sign is normal.Ok, I think is enough for this section, and it

also almost end of our time contract. After this Ill back to the nurse station and
coNurse2ulting your condition with your doctor to served the best medication for you.
Miss Vita

: Ok nurse, thankyou

Nurse Fifi had come back to the nurse station to recapitulate the data of the patient
and consulting with the doctor. After consulting in a view hour, doctor decided to take further
examination for Miss Vita including the x-ray test, so that she will be hospitalized in a few
days because her conditioNurse2.
On the following day Nurse Lina come to her ward to check up her condition and give
the pain managemet program.

Nurse 2

: Hello, good afternoon Im Nurse Lina that will take care of you today.

Miss Vita

: Good morning nurse.

Nurse 2

: Ok, Is that true with Miss Vita Agustin Erlyana, 20 years old?

Miss Vita

: Yes, Im.

Nurse 2

: How are you this morning?

Miss Vita

: Im not to bad nurse.

Nurse 2

: Ok miss, in this morning I will check up your condition and give some

therapies to solve your pain problem. It wont be long maybe just 10 until 15 minutes.
Miss Vita

: oke, sure.

Nurse 2
: Okay Miss, firstly let me clarified. Is that true if you dont feel comfortable
because your pain on your right upper arm because of your fracture trace?
Miss Vita

: Yeah right nurse Lina, Ive told to nurse Fifi before.

Nurse 2

: How about your feelings today? Do you get any problem for sleep last

Miss Vita

: Yes, there was no change, my sleep was distrubing by my pain last night.

Nurse 2

: Ok miss, before I give you some therapies let me checked your vital sign.

(Nurse Lina has about to check a blood pressure, body temperature, respiratory rate,
and pulse of the Miss Vita)
Nurse 2

: Your blood pressure is 120/80 mmHg, and you got 36,7 body temperature,

21 rpm respiratory rate, and your pulse is 56 bpm, and overall your vital sign is in the normal
range.
Nurse 2
: Well actually, Im here to give you some therapies to reduce your pain. And
there are many therapies to manage your pain.
Miss Vita

: Really? May I have one of them? Is that difficult to do?

Nurse 2
: Absolutely no Miss, all of the therapy is simple. There are many therapies,
but I will give you two optioNurse2 to choose, that is music therapy, and aromatherapy.
Miss Vita

: So, What is the music therapy?

Nurse 2
: The music therapy is one of therapies method by using the select music
indeed, you just have to listen some music that can make you feel comfortable with it and you
have to choose the genre of the music by your own.
Miss Vita

: and then, how about the aromatherapy?

Nurse 2
: Yeah, for the aromatherapy I will give you the aromatic fragrance in your
room that will relaxing you and make you feel comfortable.
Miss Vita
: Fragrance? Hmm, I think Im not too interested with that one. I think Music
therapy is the good choice for me.
Nurse 2
: Yeah, I think that is a good selection miss. So what about the genre of the
music that you want to hear for this therapy can you mention it?
Miss Vita

: Hmm, how about classic music? Do you think thats good?

Nurse 2
: Yeah... thats a good choice miss. Classic music is the one genre of music
that can make you feel relax and comfortable.
Miss Vita

: So, when you will give that therapies for me?

Nurse 2
: Actually started from now, but Im sorry miss, Do you mind if I will prepare
some equipment of music therapy first? It will be occur in a 10 minutes?
Miss Vita

: no, of course not. I will waiting for you here.

Nurse 2

: thank you miss. Ill be right back.

Just whitin a minutes, nurse came back to Miss Vitas ward and bring some
equipments for the music therapy.
Nurse 2

: Hello,excuse me miss. Im back.

Miss Vita

: Ok nurse, please come in

Nurse 2
: Ok miss, Now the equipment is ready for you, we will start the therapy. I
will help you have to put the headphone till it was fit on tour head and ear. And if you are
ready I will play the classic music. And you can enjoy it.
Miss Vita

: OK

(The Miss Vita enjoy the music)

Nurse 2

: Miss Vita, can you hear me?

Miss Vita

: Yes I still can hear you voice.

Nurse2

: okay miss, how do you feel after you got this therapy?

Miss Vita
: Actually I feel better nurse, I also feel more comfort and it was easy to
practice and so helped. Then I feel happy because I can talk with you, because I can express
my complaint.
Nurse 2
: yes miss, you can do this therapy by your own, especially when you take a
rest or sleep it may help you to reduce your pain.

Miss Vita

: Thankyou nurse. Im happy to see you.

Nurse2
: Im happy to see you too. So do you want me to come again? You have to
talk with me again and I then will check your condition. Do you permits me?
Miss Vita

: yes of course. I really want to do.

Nurse 2

: So, how about the time?

Miss Vita

: how about the afternoon in next day?

Nurse 2
: ok miss. I will come here at the same time in the next day, and I think it is
enough. Do you have any question or complaint?
Miss Vita

: no, I dont

Nurse 2
: Ok, I have to go now, if you need any help, please press the button, and the
nurse will come to you, thankyou for your cooperation mom, good morning.
Miss Vita

: Ok, nurse, thanks you for your coming, nice to see you.

Nurse 2

: Ok, nice to see you too. Good bye.

Miss Vita

: Bye.

After hospitalized in a view day finally Miss Vita was permitted by her doctor to back
home. In the end Miss Vita got back her activity and she is more carefully.
