Ensure that personnel not involved in operations is kept clear of
working area (OIM)
Personnel training (PM)
Define roles of personnel on deck and establish positioning during
operations (OIm)
Supervised operations (PDS)
Certified Equipment (PDS)
Sprv to ensure relevant Rig/Vessel personnel are aware of the tasks to
be conducted (OIM)
Sprv to ensure communication with relevant Rig/Vessel personnel
(PDS)
Sprv to ensure the ladder is checked every 6 months by competent
person (PDS)
Sprv to ensure that ladder is visually check before use (operator; PDS)
in good condition, rungs are clear of slippery substances- oil, mud
etc.
Consideration given to environmental conditions prior to starting work
No work to be undertaken in adverse weather conditions ( wind, rain
etc.) and during night hours (only day-light use of ladder)
Plan for the use of safety harness (PDS&operator):
o
existing approved anchorage points,
o
adequate harness (full body harness, minimum length
lanyard, shock absorber, connecting hardware, I-beam strap
etc.)
o
the clearance or distance to fall into (include lanyard length,
deceleration distance of 1 m, your height, one foot of harness
slack, elongation factor, and a safety factor)
o
obstacles between operator and the ground or floor below
o
how would operator be rescued if falls and suspended in the
harness
Ensure harness fits comfortably- can be adjusted so that it is
comfortable- and has the attachment points appropriate for its use
(positioning, fall arrest, etc.) (operator)
Inspect harness prior to use for the following:
o
Operation of snap-hooks or any other means of attachment
o
Condition of webbing, straps and buckles
o
Operation of brakes and ratchets in self-retracting lanyards
and winches
Damaged, defective, or in any way questionable equipment is removed
from use
Assess loads to be taken up/ down the ladder
If loads too heavy/ bulky for operators safety- consider the use of a
pulley
Ensure safety devices are maintained as operational and that operators
know the location of these devices and how to use them.
Protective equipment to be worn as per the above (operator)

Minimize the number of ascends / descends adequate preparation
For multiple users of ladder: only one person at a time on the ladder;
wait until first person reaches the roof; wait near the ladder NOT under
it
Team work; NO access/egress on the ladder alone; person assisting
must be certified first-aider; keep constant watch on operator; keep all
unauthorized personnel out of safety area; give clearance to operator
for lowering equipment; must hold bottom of the ladder during ops to
secure safe position (if needed)
Maintain good communications (operator & assistant)
Enforce the manual handling procedure (PDS)
Ensure safety devices are maintained as operational and that operators
know the location of these devices and how to use them (PDS)Safety
