PREPARING THE INDIVIDUAL PERFORMANCE COMMITMENT AND REVIEW FORM

MFO (Major Final Output)- goods or services provided to external clients to achieve a common outcome.
In DepEd, What outputs are we providing to external clients/ stakeholders to achieve our mandate?
(Organizational outcomes)
Our organizational outcomes are reflected in our VMVGO. (Vision, Mission, Values, Goals Objectives)
MFOs should .. simply describe goods and services to be delivered (avoid using adjectives)
measurable, manageable, auditable
within dept./agency control
PERFORMANCE INDICATORS (PIs)are the measures that are used to judge success in achieving outcomes or in
delivering the outputs.
PIs characterize performance in terms of Quantity (volume or output), Quality (how well output is
delivered),
Timeliness (rate at which output is delivered to clients) and Cost (amount or budget allocation
used to produce an output).
