1.2 HSE Responsibilities of Quippo Staff


What
The purpose of this HSE procedure is to define the Health, Safety & Environment responsibilities of
Quippo Oil & Gas Infrastructure Ltd & to ensure that staff is made fully aware of the HSE
responsibilities applicable to each individual within their scope of work as defined in their respective
Job Description.
Who
It is responsibility of each member of staff to carry out his/ her work activities within the spirit of
companys HSE policy statement, in full compliance with his/ her HSE responsibilities & companys
HSE principles and standards. The details are given below.
The President
He shall be responsible for developing companys overall HSE policy & he shall be accountable to
ensure that the policy is effectively implemented. He shall also ensure that there are adequate staff,
equipment & funds available to meet the requirements of the HSE policy.
He shall regularly review the effectiveness of the policy & of personnel under his control to which
responsibilities for the various aspects of HSE protection have been assigned.
He will carry out rig surveillance inspection in accordance with annual HSE improvement programme,
participate in Rig HSE meetings during his visits, initiate corrective measures on all findings observed
& ensure appropriate close out of all corrective action initiated.
VP (Drilling Operations)
He shall be responsible for liaising with Rig & QSM with reference to HSE affairs.
He will make regular visits to assigned as per HSE improvement programme & RSI calendar focusing
on all critical areas in well control.
He shall be responsible for all operational HSE Matters affecting Staff, other Persons and equipment
and of rig(s). He will familiarize himself with all HSE procedures manual and DOP requirements and
will closely monitor compliance of HSE and DOP performance of rig(s).
He will initiate appropriate action following the receipt of reports and records related to companys
HSE procedures manual and will monitor the timely completion of all corrective measures taken.
He will evaluate suggestions for HSE improvements and reports on HSE improvement compliances
within Companys ICA and other reporting systems, bring such matters to the attention of Management
Team and initiate and follow-up all actions decided upon.

He will ensure a constant flow of HSE communications within his operations, train Tool Pusher in the
appropriate application of all HSE procedures manual and DOP Procedures and Instructions, execute
Rig Surveillance Inspection, participate in Rig HSE Meetings, participate in MDSVs, initiate corrective
actions on all findings observed and ensure timely close out on all corrective and preventive measures
initiated.
He shall liaise with respective Client Representatives in all operational HSE affairs.
He shall chair the daily & monthly operations & quarterly review meetings & ensure that appropriate
actions are initiated on all HSE aspects which are in need of improvement.
He shall evaluate all safety communications with Authorities, Clients, third parties & internal
operations & direct special request for follow up to relevant departments.

Drilling Superintendent
He shall be responsible to carry out in-house training with rig supervisors on Drilling procedures
especially focusing on hole problems and ensuring all field staff are thoroughly trained on Prevention of
stuck pipe and associated down hole problems, equipment use and its Proper function.
He shall be responsible for Subcontractors compliance with Companys HSE requirements.
He will liaise with VP (DO) with reference to Safety and Quality administration of assigned rig.
He will monitor and control service contractors to ensure that service providers meet Quippo service
requirements and HSE standards.
He shall carry out Rig Surveillance Inspection as per Audit Calendar. He shall for follow up and submit
status on progress of yearly HSE Improvement Program to VP (DO) on a monthly basis.
He shall attend Rig HSE Meetings regularly and provide feed back to contract Manager. Monitor and
review monthly stop performance charts for each rig.
He will ensure that the in-house procedure training and HSE training of personnel is carried out on the
assigned Rig as per Company policy.
He shall liaise with VP (DO) on all aspects of HSE in co-ordination with HSE department.
He is responsible to support the TP during any emergency following Quippos emergency response
Procedures.

Department Head
Shall be responsible for all HSE aspects affecting employees and other Persons in his Department and
the safety and integrity of Tool, Equipment and Plant under his control. He will receive all health and
safety reports pertaining to his Department and monitor all aspects of HSE Performance of his staff and
assets.
He/ She will initiate appropriate action following the receipt of reports and records related to
Companys HSE procedures manual and will monitor the timely completion of all rectifying actions
initiated by him.
He/ She will be responsible to evaluate suggestions for safety improvement and reports on safety short
comings and bring them to the attention of Management Team for initiation of corrective/ preventive
actions. He/ She will monitor the progress of such actions and ensure timely close out of all matter
concerning his department.
He/ She will ensure that any vendor/ supplier associated with his work comply with Quippos HSE
procedures manual requirements for delivering the service(s).
The Tool Pusher & Night pusher
He shall be responsible for the HSE standards of his rig and must ensure that all operations on his rig
are carried out in compliance with Procedures and Instructions of Companys HSE procedures manual,
Drilling Operating Practice System and good oilfield Practice Requirements. He is also responsible for
the Safety and integrity of all Tool, Equipment and Plant on the rig.
He will ensure that the Supervising Staff is trained in the appropriate application of all Procedures and
Instructions of Companys HSE procedures manual and DOP. He will initiate the timely
implementation of new and revised Safety Requirements and monitor closely that such requirements are
fully complied with.
He is responsible to evaluate all hazards reported and take action as appropriate to eliminate them and
inspect all work carried out under PTW.
He is responsible to disseminate learning points from accidents made known to him and ensure that all
practical steps are taken to prevent reoccurrence of such accidents on his rig.
He shall chair the fortnightly HSE Meeting and ensure that all provisions made in HSE procedures
manual are fully complied with and will carry out TBTs and STOP audits in compliance with
documented requirements.
The Night Pusher will assist the Tool Pusher in attending to his responsibilities and will act on his
behalf when ever necessary.
The Night Pusher is responsible to act for the Driller in assuming the responsibilities for PCM as
defined in procedure 2.4

Supervisor
He shall be responsible to his Tool Pusher for the safety of all personnel working in or passing through
his section and for the safety and integrity of all tool, equipment and plant under his control.
He will ensure that all Employees have received induction training (general and job-orientated) and are
properly instructed in the safe use of tool, equipment and plant and that they are aware of their duties
and responsibilities as defined in Quippos HSE procedures manual and DOPM.
He will ensure that Toolbox Talks are held with his team members at the beginning of his shift and
during the shift as and when required, and the team members are explained about the hazards involved
with the task(s).
He is responsible to carry out Physical Condition Monitoring fortnightly and STOP Audit weekly using
Checklists/Report provided to record all findings, safe/unsafe acts/ conditions and rectifying all
shortcomings as appropriate.
He is responsible to inspect & identify hazards, initiate remedial action and report to his Tool Pusher.
He is responsible for close supervision for all work carried out under Permit to work system as Permit
Holder to ensure full compliance with PTW and inspect work site before start of work and after
completion of same to ensure that safety of work site and Plant is restored.
He is responsible for reporting all accidents to his Tool Pusher without delay, participate in accident
investigations and bring such occurrences to the attention of his crew within daily held TBTs.
He will participate actively in the fortnightly held safety meetings by bringing to the attention of all
personnel safety hazards which have arisen in his section and put forward suggestions for rectification.
The Assistant Driller
He is to assume the responsibilities of supervisors for their respective scope of work under the category
of responsibilities of supervisors at rig location, and will report to their Driller when executing these
responsibilities.
Employee
He/ She shall be responsible for carrying out his/ her work in compliance with statutory obligations as
established for Quippo. He/ She will execute his/ her operations in compliance with all provisions made
in companys HSE procedures manual and DOP which are relevant to him/ her.
He/ she is responsible to cooperate with his/ her Supervisor/ Manager to enable company to discharge
its Health, Safety and Environmental Responsibilities.

He/ she is responsible to take reasonable care of his/her own health and safety and that of other persons
who may be affected by his/ her acts and omissions at work.
He/ she is responsible to bring to the immediate attention of his/ her Supervisor/ Manager any hazard to
health and safety and report all accidents, including Near Misses as soon as possible to his/ her
Supervisor/ Manager.
He/ she is responsible for familiarize himself/ herself with all information on health and safety provided
by the company which is relevant to him/ her and acts as requested.
He/ she is responsible for not intentionally or recklessly interfere with or misuse anything provided in
the interest of health, safety or welfare in compliance with any statutory, Client or Company provisions.
HSE Manager
He shall be responsible to assist in the preparation and implementation of Companys HSE Policy and
monitors the HSE performance of all Operations and Departments. He will evaluate monthly safety
returns, including the evaluation of any action on communications received from Authorities and
Clients.
He will ensure that management is kept informed of all provisions of relevant new HSE Legislation,
alterations and amendments of existing regulations including all safety requirements of Client.
He is responsible for the documenting and upkeep of Companys HSE procedures manual, for
monitoring of the effectiveness of all provisions made and for the initiation of any action required in
order to enhance the existing system.
He is responsible to assist Line Management in maintaining adequate HSE Standards, including the
identification, evaluation and elimination of hazards as they arise.
He is responsible to establish the HSE procedures Audit Schedule at the beginning of the year execute
HSE Audits and participate in external audits and analyze the results of audits for Management Review
Reports and Meetings.
He is responsible to prepare the annual HSE improvement programme and review same in monthly
intervals including performance data and statistical data as required by Company and Client.
He is responsible for control compliance with Companys training requirement schedule and administer
the training need system.
He is responsible for executing induction training and control In-House HSE Training as appropriate
and monitor compliance with other In-House training requirements.

HSE Officer
He is responsible for assisting the HSE Manager in attending his responsibilities and will act on his
behalf as instructed.
Procurement Manager
He will ensure that machinery, plant, articles and substances purchased conform to Quippos process
management system quality and safety requirements and that all relevant information supplied by
manufacturer or designer on hazards, dangers, operational restrictions and safety precautions to be
obtained are passed on to the ultimate users and to HSE department.
He will ensure that any quality and safety critical information supplied to them from vendors or other
persons which can affect the health, safety and welfare of employees is forwarded to HSE department
so that employees, likely to be affected, can be informed accordingly.
He will ensure that such technical information and certification, that is affecting health, safety and
welfare of employees, is requested from Suppliers if not received on delivery of materials.
The HSE Steering Committee
Will comprise of the following Department Heads located at Head/ Base Office: OM, DSs, QSM,
HRM, PME, MM, FNM etc.
The steering committee members will be responsible to review Quippo's HSE performance on a
monthly interval and address the improvement needs. This monthly review meeting will as a minimum
include HSE Programme Progress, Accident Analysis, Incidents and Occupational Illness Frequency
review and Rig HSE Performance Review.
The steering committee members will be responsible to participate in the Annual HSE Performance
review meeting and agree on improvement action items for the current year.
The steering committee members will be responsible to participate in the quarterly meeting with Subcontractors to discuss existing issues.
How and When

As soon as any employee joins the company, the job description of individual will be handed over;
the detailed HSE responsibilities are included in job description.

During induction training programme individual HSE responsibilities will be highlighted & records
of induction training programme will be maintained in personal folder at HR department.

Time to time HSE responsibilities will be discussed during HSE meetings, Tool box talk at
respective work location.

This responsibility lies with line management from Top management to the lowest level. This
procedure is prepared as a part of HSE strategy.

Senior Management has to establish & maintain the HSE policy and to develop & coordinate the
safety organization that is needed to implement that policy, but safety, being a line responsibility,
must be included in the existing management organisation & must be part of each managers and
supervisors duties & responsibilities within the context of his/her employment & therefore be part
of his/her job description & daily duties

However, responsibility for safety does not stop at supervisory levels. All employees have to be
involved in the safety effort & be aware of their role in the Safety organisation & what their own
responsibilities are.

Finally, if these basic principles are not understood or not applied in the company then there is very
little chance to set up & maintain a well functioning HSE procedures Manual & acceptable HSE
performance will always be a lucky strike & not an achievement of good management.

As far as the organization for HSE concerned, it should be clear to everyone by now that there is no
other organization necessary for managing HSE in the Company. The organization for the
management of the company is pictured in the Organinogram contained in the process management
system.
