Abstra
ct
Introduction: The use of panoramic
radiography has shown a remarkable
expansion over the past 20 years.
Accurate positioning and preparation
of patients is needed to avoid
image distortion. The aim of this study
was to evaluate the common errors on
panoramic radiographs taken in the
radiology department of Rasht dental
school.
Materials and Methods: The study
sample included
390
digital
panoramic radiographs taken in Rasht
faculty of dentistry by a direct digital
panoramic unit. Frequency of common
errors including errors related to
patient
positioning
and
patient
preparation in male and females were
evaluated.
Results: There were 314 panoramic
radiographs (81.5%) with errors. Errors
related to the improper patient
positioning were observed in
306 cases (78.5%). The most common
error was palatoglossal air space
(56.2%). There was no significant
difference between males and females.
Conclusion: The errors seen in
panoramic
radiographs were relatively high, thus
training the operators is essential to
maximize the quality of panoramic
radiographs.

Introducti

panoramic errors in the above studies
reveals that preparation of error-free
panoramic radiographs is complicated. Thus,
the aim of this study was to evaluate the
frequency of common errors on panoramic
radiographs taken in the radiology
department of Guilan dental school by
trained assistants, in order to prevent further
occurrence of them.

Panoramic imaging is a technique for


producing a single tomographic image of the
facial structures that includes both the
maxillary and mandibular dental arches, and

their supporting structures.

image is a useful tool for diagnosis and

complementary dental imaging in recent

The quality of any radiograph depends on an
accurate technique and careful processing of
the image. Correct positioning of the patient
is essential for a sharp, accurate and

A total of 390 digital panoramic radiographs


of patients above 18 years old taken from
November to February 2012 with a DR
panoramic unit (Cranex D, Sordex, Helsinki,
Finland) in the radiology department of
Rasht faculty of dentistry were considered in
this study. The radiographs were taken by
only two trained technicians with the same
amount of experience. Radiographs related
to patients with mental or physical disorders
were not included in this study. All
radiographs were printed by Kodak (5700,
Germany) printer. The radiographs were
viewed under identical condition, using a
standard viewing box in a room with
subdued lighting.
Radiographs were evaluated by one oral and
maxillofacial radiologist, and frequency of
errors was recorded according to the criteria

described by Langland et al .
Patient positioning errors:
Antero-posterior positioning error (Patient is
positioned too forward or backward)
The patients head is twisted.
The chin is tipped too low or high.
The patient is slumped.
The chin is not positioned on the chin-rest.
The tongue is not on the palate.
The patients movements.

undistorted image. The operator must pay


careful attention to the patient positioning
and film processing. But there is no film
processing in digital imaging.
Some of the technical errors may cause
radiographs to be of minimal or inadequate
diagnostic quality. In cases without enough
diagnostic quality, radiographs have to be
retaken which results in receiving
unnecessary radiation absorbed dose of the
patient.
A number of studies evaluated the frequency
rate of errors seen on panoramic
radiographs. Schiff et al. reported that out of
1000 panoramic radiographs, 79.7% had

errors and Brezden et al. examined 500
radiographs and found only one error-free

radiograph. Kaviani et al. reported that out
of 250 panoramic radiographs, 92.4% had
errors. Glass et al. examined 75 panoramic
radiographs of edentulous patients and found

that 89.3% of radiographs had errors.

Granlund et al. evaluated the frequency of
errors in panoramic radiographs of young
orthodontic patients and reported that 96%

Technical errors:
The patients wearing prostheses,orthodontic
appliances or jewelry.
Apron/thyroid shield artifact.
Error related to film radiographs, such as
static electricity or exposure and darkroom
problems were not evaluated in our study.
Because our study focused on digital
panoramic radiographs, and exposure
problems can easily be dealt with by
changing brightness. The frequencies of
individual errors on all radiographs were
evaluated. Chi-Square analysis was carried
out to test for the differences between
genders. The level of significance was less
than 0.05.
Results
Panoramic radiographs of 390 patients
(female: 199 and male: 191) were evaluated.

There were 76 radiographs (19.5%) which


were error free and 314 radiographs (80.5%)
with errors. The most common positioning
error was found to be palatoglossal air space
over the roots of the maxillary teeth
(56.2%). Frequency distribution of common
errors observed in this study is shown in
tables 1&2. Some radiographs had more
than one error, so the percentages of
technical and positioning errors add up to
more than 100 percent. There was no
significant differences between male and
females by using chi-square statistical

(51.7%) radiographs had one error, 83


radiographs(21.3%) had two errors, 28
(7.2%) radiographs had three errors, and
only one (0.3%) radiograph had four errors.

Panoramic radiographs have long been one

In our dental school, training in panoramic


radiography is only given to radiology
assistants and technicians, but not the dental
students. The radiographs in this study were
taken by two trained dental assistants with
about 5 years of experiences. The rate of
error free radiographs was found to be

19.5%. Kaviani et al. reported that 7.6% of

radiographs were error free. Schiff et al.

and Akarslan et al. found that 20.3% and


37.61% were error free, respectively.
Dhillon and coworkers reported Out of
1,782 panoramic radiographs, 196 (11%)

were error free.

On the other hand, Brezden et al. and

Rushton et al. reported that only 0.5% and
0.8% of radiographs had no errors,
respectively.
In all above mentioned studies, conventional
panoramic machines were only used in our

study and Peretz et al.


study digital
panoramic radiographs were used. Thus
there were no errors related to chemical film
processing.
The most common error was found to be the
presence of palatoglossal air space. This
(3,5,8,10,11)

result is similar to other studies.


This error happens when the patient does not
raise the tongue against the palate. The
resulting air in the mouth is visible on the
radiograph as a band of radiolucency over
the maxillary teeth. This can reduce the
diagnostic quality of the roots and its
surrounding structures. According to our
experiences, patients find it difficult, or
become a little confused, when asked to
raise their tongue against the palate. This
might suggest the reason for such a high rate
of error. Taking radiographs with older
machines tends to increase error rates
because with extended use, the location of
the focal trough could change.
In this study, radiographs were taken with a
only a couple of months old machine. This
may be reason for presence of more error
free radiographs than the other studies.

But generally the error rates seem to


be high when the biological effects of
ionizing radiation is considered. Sometimes

errors could occur beyond the operators


control in patients having facial asymmetry,
short and heavy neck, very over weight,
unusually tall and the inability of following

the instructions .These situations make it
difficult to position the patient properly in
the X-ray machine. The operator must be
careful in positioning these patients.
Although errors beyond the operators
control may occur, most errors are within
the control of the operator and could be
reduced by paying more attention to taking
the radiographs. Thus, the errors seen on
panoramic radiographs were relatively high
and the errors in patient positioning were the
most frequent errors. In most cases, we
accept radiographs to reduce absorbed
patient dose. But its ideal to take standard
radiographs. It seems that the operator skill
can decrease the frequency of errors and
help produce high quality radiographs. It is
important to monitor the panoramic images
regularly and identify errors and suggest
methods to avoid these errors.
Conclusi
on
From the results of the study, the frequency
of errors seen on panoramic radiographs are
relatively high, thus training the operators
discussing the technical errors in case of
occurrence are highly essential to maximize
the quality of panoramic radiographs.

Abstrak
Pendahuluan: Penggunaan radiografi panoramik telah menunjukkan perubahan yang luar biasa
selama 20 tahun. Posisi yang akurat dan persiapan pasien diperlukan untuk menghindari distorsi gambar.
Tujuan dari penelitian ini adalah untuk mengevaluasi kesalahan umum pada radiografi panoramik yang
diambil di departemen radiologi fakultas kedokteran gigi Rasht.
Bahan dan Metode: Sampel penelitian adalah 390 radiografi panoramik digital diambil di fakultas
kedokteran gigi Rasht langsung oleh Unit panoramik digital. Frekuensi umum kesalahan termasuk
kesalahan yang berhubungan dengan posisi pasien dan persiapan pasien pada pria dan wanita dievaluasi.
Hasil: Ada 314 radiografi panoramik (81,5%) dengan kesalahan. Kesalahan yang terkait dengan posisi
pasien yang tidak benar diamati pada 306 kasus (78,5%). Kesalahan yang paling umum adalah ruang
udara palatoglossal (56,2%). Tidak ada perbedaan yang signifikan antara laki-laki dan wanita.
Kesimpulan: Kesalahan yang terlihat di radiografi panoramik relatif tinggi, sehingga pelatihan operator
sangat penting untuk memaksimalkan kualitas radiografi panoramik.

Pendahuluan
Pencitraan panoramik adalah teknik untuk menghasilkan gambar tomografi tunggal dari struktur wajah
yang meliputi baik rahang atas dan lengkung gigi rahang bawah, dan struktur pendukungnya. Gambar
Panoramik adalah alat yang berguna untuk diagnosis dan perencanaan perawatan penyakit dan
merupakan salah satu jenis gambar pencitraan gigi pelengkap di dalam beberapa tahun terakhir.
Kualitas radiografi apapun tergantung pada teknik yang akurat dan pengolahan gambar yang hati-hati.
Posisi yang benar dari pasien sangat penting untuk gambar yang tajam, akurat dan tidak terdistorsi.
Operator harus memberikan perhatian terhadap posisi pasien dan pemrosesan film. Tetapi tidak ada
pengolahan film dalam pencitraan digital.
Beberapa kesalahan teknis dapat menyebabkan radiografi untuk menjadi minimal atau kualitas yang tidak
memadai untuk keperluan diagnostik. Dalam kasus kualitas diagnostik tidak memadai, radiografi harus
dilakukan kembali yang menghasilkan penerimaan dosis radiasi yang tidak perlu oleh pasien.
Sejumlah penelitian mengevaluasi frekuensi tingkat kesalahan yang terlihat di radiografi panoramik.
Schiff dkk. melaporkan bahwa dari 1000 radiografi panoramik, 79.7% memiliki kesalahan dan Brezden
dkk meneliti 500 radiografi dan hanya menemukan satu foto radiografi yang bebas kesalahan. Kaviani
dkk. melaporkan bahwa dari 250 radiografi panoramik, 92,4% memiliki kesalahan. Kaca dkk. memeriksa
75 radiografi panoramik pasien edentulous dan menemukan bahwa 89,3% dari radiografi memiliki
kesalahan. Granlund dkk. mengevaluasi frekuensi kesalahan dalam radiografi panoramik pasien
ortodontik muda dan melaporkan bahwa 96% memiliki kesalahan. Sebuah pemaparan dari frekuensi
kesalahan panorama dalam studi di atas mengungkapkan bahwa persiapan radiografi panoramik bebas
kesalahan sangat rumit. Dengan demikian, tujuan dari penelitian ini adalah untuk mengevaluasi frekuensi
kesalahan umum pada radiografi panoramik yang diambil dalam departemen radiologi fakultas
kedokteran gigi Guilan oleh asisten yang telah dilatih, untuk mencegah lebih lanjut terjadinya kesalahan.
Bahan dan Metode

Sebanyak 390 radiografi panoramik digital pasien di atas 18 tahun yang diambil dari November-Februari
2012 dengan 11 dalam Penelitian ini. Radiografi diambil oleh hanya dua teknisi terlatih dengan jumlah
pengalaman yang sama. Radiografi terkait untuk pasien dengan gangguan mental atau fisik tidak
dimasukkan dalam penelitian ini. Semua radiografi dicetak oleh printer Kodak (5700, Jerman).
Radiografi yang
dilihat di bawah kondisi yang sama, menggunakan view box di sebuah ruangan dengan lampu redup.
Radiografi dievaluasi oleh satu radiologist oral dan maksilofasial, dan frekuensi kesalahan dicatat sesuai
dengan kriteria yang dijelaskan oleh Langland dkk.
Kesalahan posisi pasien:

Kesalahan posisi antero-posterior (Pasien diposisikan terlalu maju atau mundur)


Kepala pasien dipelintir.
Ujung dagu terlalu rendah atau tinggi.
Pasien merosot.
Dagu tidak diposisikan di chin-rest.
Lidah tidak di palatum.
Gerakan pasien.

Kesalahan teknis:

pasien yang tetap memakai protesa, piranti ortodontik atau perhiasan.

Kesalahan yang berkaitan dengan radiografi film seperti listrik statis atau paparan dan masalah kamar
gelap tidak dievaluasi dalam penelitian kami. Karena penelitian kami terfokus pada radiografi panoramik
digital, dan masalah paparan dapat dengan mudah ditangani dengan mengubah tingkat kecerahan.
Frekuensi kesalahan individu pada semua radiografi dievaluasi. Analisis Chi-Square dilakukan untuk tes
perbedaan antara jenis kelamin. Tingkat signifikansi kurang dari 0,05.
Hasil
Radiografi panoramik dari 390 pasien (perempuan: 199 dan laki-laki: 191) dievaluasi. Ada 76 radiografi
(19,5%) yang bebas dari kesalahan dan 314 radiografi (80,5%) dengan kesalahan. Posisi kesalahan yang
paling umum ditemukan ruang udara palatoglossal atas akar gigi rahang atas (56,2%). Distribusi
frekuensi umum kesalahan yang diamati dalam penelitian ini ditunjukkan pada tabel 1 dan 2.

Beberapa radiografi memiliki lebih dari satu kesalahan, sehingga persentase kesalahan teknis dan posisi
hingga jumlah kesalahan lebih dari 100 persen. Tidak ada perbedaan yang signifikan antara laki-laki dan
wanita dengan menggunakan analisis statistik chi-square (P-value = 0,824). Dalam studi ini, 202 (51,7%)
radiografi memiliki satu kesalahan, 83 radiografi (21,3%) memiliki dua kesalahan, 28 (7.2%) radiografi
memiliki tiga kesalahan, dan hanya satu (0,3%) radiografi yang memiliki empat kesalahan.
Diskusi
Radiografi panoramik telah lama menjadi salah satu cara yang paling umum untuk pencitraan struktur
gigi. fokus dari mesin panorama adalah zona tiga dimensi yang melengkung dan penting untuk
mendapatkan gambar berkualitas tinggi. Terbatas dimensi fokus, kecerobohan operator dan usia unit
panoramik mempengaruhi terjadinya beberapa kesalahan .
Dalam fakultas kedokteran gigi kami, pelatihan radiografi panoramik hanya diberikan kepada asisten
dan teknisi radiologi, bukan mahasiswa. Radiografi dalam penelitian ini adalah diambil oleh dua asisten
gigi yang dilatih dan memiliki pengalaman sekitar 5 tahun. Tingkat radiografi bebas kesalahan yang
ditemukan 19,5%. Kaviani dkk. melaporkan bahwa 7,6% dari radiografi yang bebas dari kesalahan.
Schiff dkk. dan Akarslan dkk menemukan bahwa 20,3% dan 37,61% yang bebas dari kesalahan, masingmasing. Dhillon dkk melaporkan Dari 1,782 radiografi panoramik, 196 (11%) yang bebas dari kesalahan.
Di sisi lain, Brezden dkk dan Rushton dkk. melaporkan bahwa hanya 0,5% dan 0,8% dari radiografi tidak
memiliki kesalahan, masing-masing.
Dalam semua studi yang disebutkan di atas, mesin panoramik konvensional hanya digunakan di
penelitian kami dan dalam studi Peretz dkk menggunakan radiografi panoramik digital. Dengan demikian
tidak ada kesalahan yang berhubungan dengan pengolahan film.
Kesalahan yang paling umum ditemukan adalah kehadiran ruang udara palatoglossal. Hasil ini mirip
dengan penelitian lain. Kesalahan ini terjadi ketika pasien tidakmengangkat lidah ke palatum. Hal ini
mengakibatkan udara di mulut terlihat pada radiografi sebagai radiolusen diatas gigi rahang atas. Hal ini
dapat mengurangi kualitas diagnostik akar dan struktur sekitarnya. Menurut pengalaman kami, pasien
sulit, atau menjadi sedikit bingung, ketika diminta untuk mengangkat lidah ke palatum. Kondisi ini
mungkin menjadi alasan tingkat kesalahan yang tinggi. Mengambil foto radiografi dengan mesin yang

lebih tua cenderung meningkatkan tingkat kesalahan karena dengan penggunaan yang berkepanjangan,
lokasi fokus bisa berubah.
Dalam penelitian ini, radiografi diambil dengan mesin yang berusia hanya beberapa bulan. Ini mungkin
menjadi alasan untuk kehadiran lebih radiografi bebas kesalahan dari penelitian lain. Tapi umumnya
tingkat kesalahan tampaknya tinggi ketika efek biologis radiasi pengion diperhatikan. Kadang-kadang

Meskipun kesalahan di luar kontrol operator dapat terjadi, sebagian besar kesalahan berada dalam kontrol
operator dan bisa dikurangi dengan lebih memperhatikan teknik pengambilan foto
radiografi. Dengan demikian, kesalahan yang terlihat pada radiografi panoramik yang relatif tinggi dan
kesalahan dalam posisi pasien adalah kesalahan yang paling sering ditemukan. Dalam kebanyakan kasus,
kami menerima hasil radiografi untuk mengurangi dosis radiasi yang diserap pasien. Tapi ideal untuk
mengambil foto radiografi standar. Tampaknya keterampilan operator dapat mengurangi frekuensi
kesalahan dan membantu menghasilkan radiografi berkualitas tinggi. Penting untuk memantau gambar
panoramik secara teratur dan mengidentifikasi kesalahan dan menyarankan metode untuk menghindari
kesalahan ini.
Kesimpulan
Dari hasil penelitian dapat disimpulkan bahwa frekuensi kesalahan yang terlihat pada radiografi
panoramik relatif tinggi, sehingga pelatihan pada operator dan kesalahan teknis dalam suatu kejadian
kasus sangat penting untuk memaksimalkan kualitas radiografi panoramik.
