UNIQUE IDENTIFYING INFORMATION in the possession of the Wireless Service Provider


listed above with regard to the target cell phone listed above, specifically: The Electronic Serial
Number (ESN) or Mobile Equipment Identifier (MEID) in both decimal and hexadecimal
formats, Mobile Subscriber Identification Number (MSID), International Mobile
Equipment Identity Number (FMEI), and/or International Mobile Subscriber Identity
(IMSI) for the target cell phone.

HISTORICAL INFORMATION in the possession of the Wireless Service Provider listed


above with regard to the Target Cell Phone listed above in the Historical Target Period (the
Historical Target Period is between December 1, 2014, 00:00 and June 13, 2015. 23:59. These
historical records include:
o Subscriber and billing information including any unlisted or unpublished phone numbers
and any account comments,
o Call Detail Records including both inbound and outbound call detail records and/or call
origination or termination records and/or other data which could reveal the location of the
target cell phone. These records include:
Cell site tower information, in other words, information which would show the towers
this cell phone interacted with electronically and the manner in which that interaction
occurred. Using the industry terminology, this would include switch and sector
information as well as Registration Zone and Base Station Identification [BSID]
information as well as range data, orientation, sector, azimuth, beam width and any
available historical RTT (Round Trip Delay Time) information associated with voice
calls, text messages, and/or data sessions.

An engineering map or maps and/or a list of the locations (physical addresses)


of all towers or cell sites directly accessed by the target cell phone for the calls in
the call detail records and the same information for all towers or cell sites in the
market(s) in which the target cell phone was used
Stored communications orfiles including voice messages, email messages, digital images,
buddy lists, and/or SMS text message data and SMS text message content and any other files
associated with the account.

Upon oath and/or affirmation, I hereby swear that the facts establishing the grounds for issuance of a
Search Warrant / Order are as set forth in the attachments made a part of this application.

PROBABLE CAUSE FOR THE TARGET C E L L PHONE AND SUBJECT:

On June 12 , 2015 at approximately 19:30 hours, officers were dispatched to the Coral Ridge
Mall, 1451 Coral Ridge Ave., Coralville, Iowa, for a shooting. On arrival near the information
desk inside the mall, officers found one gunshot victim who was transported to UIHC and later
died. Multiple witnesses on scene identified the shooter as mall security employee Alex.
Coralville Police Officer Josh VanBrocklin, familiar with the employee, gave his last name as
Kozak. Witnesses stated Kozak ran from the scene. In addition, one witness saw the shooting and
saw Kozak flee in a blue Kia Optima bearing the Texas registration DNF6582.
Mall security records showed Kozak's address as 240 Sadler Ln. Apt. 201, North Liberty, Iowa.
Officers arrived at that address and made entry into the apartment for a welfare check on
Kozak's wife, Kellie Kozak. The apartment was empty. Shortly after, officers made contact with
Kellie Kozak outside the apartment as she arrived home from work. She confirmed 240 Sadler
Ln. Apt. 201, North Liberty, Iowa, as their address. Kellie Kozak confirmed that Alex owned at
least one gun and described it as a 9mm Glock, as well as multiple swords.
On 6/12/2015 at approximately 2023 hrs., Trooper Engstler was parked in the median on 180 at
the 285 mile marker when he observed a blue passenger vehicle, later identified as a Kia, pass by
with a Texas registration. Trooper Engstler found this vehicle to match the vehicle description
from a shooting at Coral Ridge Mall. Trooper Engstler exited the median and proceeded to catch

up to the Kia. Trooper Engstler ran the Texas plate of DNF6582 with Cedar Rapids State Radio
and a dispatcher soon advised the registration matched the suspect vehicle. Trooper Engstler
proceeded to follow the Kia until he was backed up by Scott County Sergeant Lage. Trooper
Engstler and Sgt Lage initiated a traffic stop at the 294 mile marker on 180 and the driver, later
identified as Kozak, stopped the Kia. Kozak was taken into custody and handcuffs were placed
on him. Kozak was placed in the back of Sergeant Lage's squad car. Trooper Engstler soon read
Kozak his Miranda rights, to which he said that he understood.
Trooper Engstler assisted Special Agent Rick Rahn with the transport of Kozak back to the
Coralville Police Department. Trooper Engstler drove SA Rahn's vehicle with Kozak and Rahn
being the passengers. During this transport, Kozak advised that he was working at the mall
today when he left the mall at approximately 5:20 pm to go back to his residence. Kozak
advised the victim had sent a text and broke off their relationship. Alexander Kozak and the
victim had been conversing by cell phone text messaging or calling each other using a cell
phone. Kozak advised that he snapped and went back to his residence in North Liberty to get his
gun (Glock) and packed a bag of clothes. Kozak said that he then returned to the mall and
proceeded to shoot the victim 3 times in the back. Kozak entered his vehicle and drove onto 180
eastbound. While traveling eastbound, he threw his cell phone out onto the interstate.
On June 13,2015, S/A Craig Mackaman ofthe Iowa DCI located Alexander Kozak's cell phone,
An LG cell phone with a green case, on 180 at the 257.7 mile marker. S/A Mackaman seized the
cell phone and transported it to the Coralville Police Department.
Investigators located a white Iphone with a maroon or pink case lying next to the victim after the
shooting. The Iphone was covered in blood and investigators believe that the Iphone belonged to
Andrea Farrington. On June 13,2015,1 called Connor Sirovy. Sirovy was in a relationship with
Farrington and he described the cell phone he had seen Farrington recently in possession of.
Sirovy stated that Farrrington's cell phone was a white Iphone with a purple or maroon case.
Sirovy also stated that Farrington's cell number is 641-990-6776. This cell number is subscribed
to US Cellular.
It is probable to believe that information regarding the intentions and/or motive of Alexander
Kozak could be contained in text messaging content stored in Farrington's Iphone. The text
messaging content along with the incoming/outgoing call history from US Cellular would assist
investigators in the murder investigation.
The Wireless Service Provider, United States Cellular, has confirmed that the Target Cell Phone
is a valid phone number on their network.
This Target Cell Phone Number is a United States Cellular phone number. I knowgfrom
my training and experience that cell phone providers, such as United States C e l l a r in
this case, keep data such as that set out in the "Items to be Produced and Seized^s^ctio^
of this warrant application. So there is probable cause to believe these items aje^i thj^location.

SPECIFIC PROBABLE CAUSE FOR ITEMS SOUGHT:

Unique Identifying Information, as set out in this application, is relevant to this criminal investigation
because, in part, it will help to locate the target cell phone and upon any seizure of the phone, will
confirm that the correct phone was seized, that the other records obtained are pertinent to this specific
phone, suspect and therefore this specific investigation.
Historical Information, set out in this application, is relevant because: it will likely lead to locations
where the suspect has regularly stopped or stayed in the past and numbers and persons the suspect has
routinely contacted in the past. Such historical information can also help to confirm (or refute)
investigative information that the suspect was at the scene of the crime (to the extent the suspect was in
possession of the target phone at the time of the crime). Historical information can further help to
establish relationships and prior contacts between the suspect and other witnesses and/or the victim of the
crime at issue which can help in re-creating the events surrounding the offense and can lead to additional
witnesses with information about the crime at issue.
WIRELESS DATA INFORMATION:

General Cell Phone Data: I knowfrommy training and experience andfromspeaking with other peace
officers and wireless service providers (hereinafter cell phone companies), that cell phone companies
collect and store a wide variety of data about their customers and the cell phone usage of their customers.
Cell phone companies keep administrative, subscriber and billing records. When a cell phone call is
made or received, data about the call is captured and stored. By way of example, the date, time and
length of the call, whether the call was incoming or outgoing, whether the call connected or went to voice
mail, as well as incoming and outgoing phone numbers are all often captured and stored. Such
information is relevant to your affiant's ongoing criminal investigation.
Conclusion: The affiant is not seeking information regarding the content of any contemporaneous
communication. To the extent this application seeks communications, it only seeks stored historical
communications accessiblefromthe cellular phone provider.
