(1)Overview:
Theliverisaveryimportantorganinthehumanbody.Itisplacedunderrightribs
justundertherightlung.Themostimportantfunctionoftheliveristoconvert
foodintoenergyandfilteringandstoringblood.Theliverplaysapivotalroleinthe
detoxificationofblood,i.e.removaloftoxinsproducedinthebodyduetometabolic
activityofthebody.Theliverreceivesbloodfromtwosources,thehepaticartery
suppliestheliverwithbloodfromtheheartthatisrichinoxygenandtheportal
veincarriesnutrientrichbloodfromthedigestiveorgans.Liveralsoproduces
clottingagentsnecessaryforclottingthebloodwhenthebodyisinjured.Itsecrets
bileintotheintestines,tohelpabsorbnutrients.Theliverismadeupofseveral
differenttypesofcells.Thisiswhythereareseveraltypesofmalignant(cancerous)
andbenign(noncancerous)tumorsthatcanformintheliver.
Benigntumors:
(i)HemangiomaThemostcommontypeofbenigntumoroftheliver,startsin
bloodvessles.Becausemosthemangiomasofthelivercausenosymptoms,theydo
notneedtreatment.Somehowevermayneedtobesurgicallyremoved.
(ii)HepaticadenomasThesearebenigntumorsofhepatocytes(themaintypeof
livercell).Mostcausenosymptomsanddonotneedtreatment.However,some

eventuallycausesymptoms,suchasabdominalpain,amassintheabdomen,or
bloodloss.Becausethereisariskthatthetumorcouldrupture,andasmallrisk
thatitwouldeventuallydevelopintolivercancer,mostexpertsusuallyrecommend
surgicalremovalifpossible.
(iii)FocalnodularhyperplasiaThisisatumorgrowthofseveralcelltypes.
AlthoughFNHtumorsarebenign,itcanbedifficulttotellthemapartfromtrue
livercancers,andtheysometimesaresurgicallyremovedwhendiagnosisis
unclear.
Malignanttumors:
(i)Hepatocellularcarcinoma:Itisthemostcommonformoflivercancer
occurring.Itisonnumerousoccasionscalledhepatomabecauseitcomesfromthe
hepatocytes.Itaccountsforabout75%ofprimarylivercancers.
(ii)Cholangeocarcinomas:Thisaccountsfor10to20%ofprimarylivercancers.
Thesearealsocalledintrahepatic(startingwiththeliver)Cholangeocarcinomas.
Thesecancerstartsinthesmallbileductswithintheliver.
(iii)Angiosarcomasandhemangiosarcomas:Thesearerarecancersthatbeginin
thebloodvesslesoftheliver.Exposuretovariouschemicalssuchasvinylchlorides
orthoriumoxide,andradioactiveelementslikeradiumorpoisonslikearsenicare
knowntocausethesetypesofcancers.Angiosarcomasgrowrapidly,bythetime
theyaredetectedthegrowthistoolargetobesurgicallyremoved.
(iv)Hepatoblastoma:Thisisaveryrarekindofcancerthatdevelopesinchildren,
usuallyuptofiveyearsofage.Ifdetectedearlythiscancerishighlytreatable.

(2)Symptoms:
Signsandsymptomsinlivercancersarenotpresentuntilthelaststagewhencancer
hasspread.Manysignsandsymptomsarerelativelynonspecific,theycanbe
causedbyothercancersornoncancerousdiseases.Howeverthefollowing
symptomsmayrequireattention.
(i)Unexplained,unintentionalweightloss.
(ii)Anorexiapersistentlackofappetite.
(iii)Liverenlargementoramassthatcanbefeltintheareaofliver.
(iv)Persistentabdominalpain.
(v)Jaundice.
(vi)HyperglycemiaSomeliverhormonesmaycausehyperglycemia(highblood
calciumlevels).Thiscanleadtoweakness.
(vii)HypoglycemiaLowbloodsugarlevels,whichcanleadtofainting.
(viii)GynecomastiaEnlargementofbreastsinmen,andshrinkingoftestis.
(ix)AseitisWaterretentionintheabdominalarea.Manypatientsareseenwith
thesesymptoms.

(3)Riskfactors
(i)GenderHepatocellularcarcinomaispredominantlymorecommoninmales

thaninfemales.
(ii)ChronicviralhepatitisChronic(longterm)infectionwithhepatitisBvirusor
HepatitisCvirusisanimportantlivercancerriskfactor.Theseinfectionsare
responsibleformakinglivercancerthemostcommontypeofcancerintheworld.
(iii)CirrhosisCirrhosisistheresultofscartissueformationintheliver.Thiscan
oftenleadtocancer.Mostlivercirrhosisoccursinpeoplewhoabusealcohol.But,
HepatitisBandCarealsomajorcausesoflivercirrhosis.Anothercauseis
excessiveironinliver.
(iv)TobaccouseAlinkhasbeenestablishedbetweentobaccouseandlivercancer.
Combinedwithalcoholtobaccoisverypotentincausingcancer.
(v)InheritedmetabolicdiseasesCertainmetabolicdiseasescanalsoleadto
cirrhosis.Peoplewithhemochromatosis,absorbtoomuchironfromtheirfood.
Theyaremorelikelytodevelopcirrhosisbecauseofthehighlevelsofironintheir
liver.
(vi)AflatoxinsThesecancercausingsubstancesareproducedbyafungusthat
contaminatescrops.LongtermexposuremaycauseHepatocellularcarcinoma.
(vii)ArsenicChronicexposuretodrinkingwatercontaminatedwithnaturally
occurringarsenicsuchasthatfromwells,increasestheriskoflivercancer.

(4)Metastasis:
Mostlywhencancerisfoundintheliver,itdidnotstarttherebutspread,or
metastasizedfromacancerthatdevelopedintheotherorgansofthebodysuchas
pancreas,colon,stomach,breast,orlung,andmetastasizedtotheliver.These
tumorsarenamedaftertheirprimarysiteofoccurrencesandarefurtherknownas
metastasis.Forexample,cancerthatstartedinthelungandspreadtotheliveris
calledmetastaticlungcancerwithspreadtotheliver.

(5)Diagnosis:
Thefollowingmethodsareusedbymodernscienceinthediagnsisoflivercancer.

(ii)Computedtomography(CT)
(iii)Magneticresonanceimaging(MRI)
(iv)Angiography
(v)Leproscopy
(vi)Biopsy
(vii)Alphafetoproteinbloodtest.

(6)Staging:
(i)Stage1Thetumormaybesmallorlargebuthasnotaffectedbloodvessles.
(ii)Stage2Theremaybemanytumorsoflargesizebutbloodvesslesarenotyet
affected.

INTRODUCTION:
Liveristhelargestglandinthebodyandperformsmorethan500
differentfunctions.PrimaryliverCancerisrelativelyrarebut
metastaticlivercancer(spreadfromadistantcancerousorgan)isat
least20timesmorethanprimarylivercancer.Livermetastasisisa
terminaleventinmanycancers.Advancedlivermetastaticcancerisa
significantproblemwithlimitedtreatmentoptionsandhasverylittle
chancesofsurvival.Chronicliverdiseasesincludesviralhepatitis,
Jaundice,,Alcoholiccirrhosisorprimarybilliarycirrhosis,hepatitisB,
alltheseareriskfactorsofhepatocellularcarcinoma.Maleshavehigher
riskforHCCthanfemales.Themetastasiscanbeeithersolitaryor
multiple.Incaseofmultiplemetastasis,liverfunctioncanbecome
altered,leadingtofurtherdeteriorationinapatient'scondition.The
treatmentoptionsinsuchascenarioarelimitedwithausualfatal
outcome.Commonsymptomsfoundinthesepatientsarehepatomegaly,
weightloss,abnormalelevationofliverenzymes,elevationofAFP,
Anemia,massormultipleliverlesions,obstructivejaundice...etc.For
thedetectionoflivercarcinoma,elevationofAFP,USGAbdomen,CT
scanorMRIofabdomen,needlebiopsyorlaproscopicliverbiopsyis
helpful.

researchcommitteemembers,alongwiththeirassistantresearch,
doctorsandscientistconductedthewholeproject.
AIMSANDOBJECTIVES:
1.Theaimistostudytheeffectsofthejuiceofthe"Citrusmedica'",
popularlyknownas"Bijora",inthetreatmentofpatientshaving
primaryormetastaticlivercancerswithimprovingliverfunctions.
2.Oneofthemost"ancient"medicalsciencesoftheworldis
"Ayurveda".Ayurvedicmedicinehasnosideeffects.Since1978Vaidya
ShreeShreeMaaAnantanandTirdhjitreatsalltypesofadvanced
cancerswithdifferenttypesofanticancertreatments,somrofthemare
panchkarmatherapyandherbaljuicetherapy.Thesetreatmentsfind
excellentresults.Theinstituteisworkingtowardsprovidingthepatients
withacureforthisterribledisease.

Methods:Allpatientsregisteredat`DivyajyotAyurvedicResearch
Foundation'weregivenspeciallypreparedtreatment.
Theirrespectivefindingswereanalyzedforstatisticalsignificance.
Patientswithliverdiseasesunderwentbaselineinvestigationsinthe
formofliverfunctiontest,tumormarkerstudies,Ultrasound
examinationofliveranddetaileddocumentationofclinicalstatus.The
patientareadministeredaglassofsingleBijorafruitjuiceintheearly
morningwithemptystomachfollowedaftertwohours.Thismustbe
continuedregularlyatleastforaminimumperiodof3months.
FortheothersymptomspatientswereadvisedtotakeAyurvedic
medicinesasperrequirementaftertheirprakrutiexaminationandnadi
pariksha.Thefollowingherbswereadministeredalongwiththe
treatment.

Thesepatientswerealsoadvisedstrictdietrestrictionlike
(1)Tostopgheeandoilpreparationscompletely.(2)Toavoidspicyfood.
(3)Totakeboiledvegetables.(4)Totakegoatmilk.(5)Totakepapaya
andallkindsofcitrusfruit.
Theirclinicalstatusandbiochemicalparameterswerestudiedatthe
intervalof4to6weeks.Thecoursewascarriedoutforatleast3
months.Ifthejuicegivesgivespositiveresponseitcanbeincreasedup
to6months.After6months,radiologicalinvestigationswerealso
carriedout.
CRITERIAOFASSESMENT:
(1)Allsymptomsoflivercancerwerereportedatthetimeofpatient's
registration.Thesymptomswhenreassessedandifimprovedorscored
assymptomaticallyreliefandLFT(LiverFunctionTest)is
encouraging,isconsideredtobeas`Improved'.
(2)Patientwhoisobservedtobesymptomaticallynormalfromdisease
andLFTisnormalandradiologicalfindingsarealsonearabout
normal,isconsideredtobe'MarkedlyImproved'.
(3)Thosepatientswithallinvestigationslikeradiologicaland
hematologicalreportsnormal,areconsideredDiseaseFree.
(4)Patientswithsymptomsinstatusquoorincreasedareconsideredto
be`Uncured'.
RESULTS:
Totalnumbersofselectedprimarycancerpatientswere30.Total
numbersofselectedmetastasescancerpatientswere70.
