FOREWORD
Established in 1975, PT Avrist Assurance (Avrist) was the FIRST joint-venture life
insurance company in Indonesia, with a proud 36 years track record in providing life
insurance, accident and health insurance, credit life and pensions to both individuals and
group clients, including sharia insurance/takaful through multi-channel distribution.
Avrist is the only independent life insurer in Indonesia backed by an AAA-rated
international financial institution, namely its joint venture partner DEG (Deutsche
Investitions-und Entwicklungsgesellschaft), which is a member of KfW Bankengruppe, one
of the largest European Development finance institutions; as well as Meiji Yasuda Life (Meiji
Yasuda Life Insurance Company), one of the market leaders in life insurance in Japan with
more than 120 years experience.
With over 4,000 agents and 500 employees spread across the country in
approximately 28 sales offices and 15 independent agency offices, Avrist serves a large base

in Indonesia, established since 1975, has a proud experience for over 39 years.
Avrist provides life insurance program, accident and health insurance, credit life
insurance and pensions, both for individuals and groups, through various distribution
channels.
1975 AJIA (PT Asuransi Jiwa Ikrar Abadi).
1979 Opens first sales office outside the city of Jakarta.
1984 PT Asuransi AIA Indonesia.
First multinational life insurance companies in Indonesia.
1995 Headquarters moved to Panin Bank Building, Jl. Jend. Sudirman, Jakarta.
2000 Launched the first unit-link product.

Perry Mahyudin Diah

President Director

Armed with the belief that great potential of insurance in Indonesia, until
now Perry M. Diah has dedicated itself in for more than 20 years in the
world of life insurance. Currently, Perry M. Diah served as President Director of PT Avrist
Assurance.
Cultivate its totality in the insurance field is also supported by educational background, Perry
is a graduate of Bachelor of Business Administration from The College of Insurance and
holds the title of CLU and ChFC from the Singapore College of Insurance. In addition, Perry
also holds a license as an advisor of the Board of Investment of Indonesia.
Dedication to the life insurance world has led him to follow the transformations that occur in
Avrist. Surely these changes intended to encourage the development of the life insurance
industry in Indonesia, as well as presenting a better life for the people.
Adi Purnomo Wijaya
Vice President
PT Avrist Assurance
Adi Purnomo Wijaya are professionals who already have a track record of
experience for more than 34 years in the life insurance industry. Join the
PT Avrist Assurance in January 2011, who was born in Blitar, March 8, 1950 it now serves as
Vice President.
In Avrist, Adi together with the entire management team will continue to develop throughout
the company's distribution channels in marketing life insurance products to meet the needs of
the people of Indonesia will be wealth protection and wealth accumulation. He is also
committed to further build and promote the life insurance industry in view of the role of the
industry as one of the pillars of the welfare of Indonesian society.
UB Faculty of Economics graduate previously served as Vice President Director of PT
Asuransi Jiwa Manulife Indonesia (1984-2010) and as Senior Manager of PT Asuransi Jiwa
Central Asia Raya (1976-1984).
Ibnu Hasyim
Director of Operations and Actuarial
PT Avrist Assurance
Ibnu Hashim was born in Jakarta, June 15, 1979. The man graduated from
University of Indonesia majoring in Actuarial and Banking, is currently
serving as Director of Operations and Actuarial PT Avrist Assurance (2013-present). Before
serving in these positions, Ibn was Head of Actuary and ERM Avrist.
Before joining Avrist, Ibn have experience as an Actuarial Consultant at various multinational
companies, including PT Eldridge Gunaprima Solutions (2000-2007) and PT Watson Wyatt

Purbajaga (2003-2006). Ibnu also have the experience of a solid career as Actuary in various
insurance companies, both national and multinational, inter alia PT Mandiri Life (20002002), PT Sun Life Financial Indonesia (2002-2003), AIA Indonesia (2003), PT Asuransi
Jiwa Generali (2011-2012) and Head of Actuary & Risk Management Avrist (2007-2011) the.
Ibnu also has been certified as a Fellowship of the Society of Atuary Indonesia (FSAI) in
2003 and participated in various seminars or training both as participants and speakers.
Supardi Supaman
Director of Finance

Supardi Supaman have a Bachelor's degree in Economics and Accounting
from Trisakti University. Supardi currently serves as Director of Finance
at PT Avrist Assurance. Previously, he also served as VP Comptroller, VP Admin and Director
during his tenure in Avrist. For more than 21 year career in PT Avrist Assurance, Supardi has
an important role in the process of change and development of the company.
He began his career as an Audit Manager in KAP SGV - Utomo & Co. (now Ernst & Young),
he has participated in various training programs, including in the areas of auditing, taxation
and insurance such as Audit Management Conference, AIA Comptroller Conference, AIG
Leadership Breaktrough in Hongkong & China , the revised Code of Indonesian Insurance.

Gregory Robert Scott Crichton
President Commissioner (Chairman)
PT Avrist Assurance
Gregory Robert Scott Crichton was Regional General Counsel with
American International Group and General Counsel for American
International Assurance, Co., Ltd., for more than 25 years. During that time he held various
officer level positions including Executive Vice President and was promoted to the Board of
Directors of AIA in 1994 as well as serving on numerous other Boards of AIG and AIA. He
has served on the Hong Kong Inland Revenue Board of Review and other bodies. He is a
graduate in Law from the University of Sydney and holds a Bachelor of Arts from the
University of New South Wales. He was also admitted as a solicitor of the Supreme Court of
Hong Kong in 1995 and is also a solicitor of the Supreme Court of England and Wales. Mr.
Crichton was appointed as President Commissioner of PT Avrist Assurance in 2009.
Jannes Hutagalung

Jannes Hutagalung graduated from University of Indonesia and Yale

University. He currently serves as Independent Commissioner of PT Avrist
Assurance. Previously, he had served as a member of the Board of
Commissioners of PT Phillips-Ralin Electronics, PT Asuransi Jasa
Indonesia and PT Angkasapura II.
Jannes Hutagalung had also held several important posts at the Ministry of
Finance of Republic of Indonesia during his long service in the Government of Indonesia. He
had also worked at the Coordinating Ministry for Economic Affairs of the Republic of
Indonesia as Deputy Minister for International Economic and Financial Cooperation.
Moreover, he worked for 9 years at the World Bank, Washington DC, where he served as the
Executive Director representing Southeast Asian Group from 1996 to 2000.
Kazunori Yamauchi

Upon achieving Bachelor of Laws degree from Aoyama Gakuin
University, Tokyo, Japan in 1985, Kazunori Yamauchi joined Meiji Yasuda
Life Insurance Company. In line with the joint-venture partnership of Meiji Yasuda Life
Insurance Company and PT Avrist Assurance since November 2010, Yamauchi holds an
important position in PT Avrist Assurance as a member of the Board of Commissioners.
Yamauchis 29 years of experience with Meiji Yasuda Life Insurance Company involved
various positions, such as Securities Analyst (1985), Representative (1993) at Sydney Office,
Deputy Manager (1997), Investment Research of Investment Administration Dept., Manager
(2000) of Asset Management Dept., Senior Account Executive (2005) of 1st Corporate
Marketing Dept., General Manager (2007) of Planning Dept. Meiji Dresdner Asset
Management Company, Staff General Manager (2009) of Capital & Surplus Management in
Corporate Planning & Research Dept., Staff General Manager (2010) of New Business
Planning & Research in Corporate Planning & Research Dept., Staff General Manager (2011)
of International Business Dept., and General Manager (2013) of International Business
Dept.. Since July 2014, Yamauchi is Executive Officer and General Manager of International
Business Dept.
