Preface
Drill of evacuation and chlorine leak response has been conducted 1 day carried out on September, 21st 2011 at Gajah
Mungkur WTP, Semarang-Indonesia.
Drill held in one day and is divided into one session, good practices and technical skills associated with the response in
terms of evacuation, first aid and leakage of chlorine. The second session was the implementation of the chlorine leak
handling drill and followed by an evaluation that involves all participants.
The report is particularly concerned and aimed for:
Measuring the emergency preparedness level on site
Capitalizing some defectives and deficiencies aspect (and vice versa) due to the changes of procedures,
equipments or facilities adapted by site since the previous drill.
Preparing action plan required to continually improve the Emergency Response Plan adapted by site.
Measuring the effectiveness of on call procedures, directing the handling of leaks and evacuation from a
distance.
The drill conducted was the fifth for PT. Tirta Gajah Mungkur.

II.

SCENARIO

The following scenario was draw up during drill:


An Incident occured on the event unloading chlorine cylinder from the truck to storage.
This happen due to a clash between chlorine tank that will be weighed to the operation tank.The leakage come from
a crack of the flexible hose operation tank and unfortunatelly the auto shut off valve is not functioning well.
The alarm and neutralisation system are on and caused the truck driver panic and jump from the truck and fell down
then his head hit the wall and he unconsious.
The shift leader, supplier and operator left the location go to assembly point, and then report to production
coordinator and Emergency manager.
Emergency manager arrange the Emergency Response Team shall act the to respond the incident .

D. Roles and responsibilities
The following are roles and responsibilities of the ERT:
1. Emergency Manager
a. Through IC to ask all employees at site to gather at CLAP after receiving information from shift
leader/employee that has identified an emergency situation occurred.
b. To state that an emergency situation has occurred. To command all employees staying at CLAP
unless situation has been stated safe and secure.
c. Immediately to the plant and immediately activate ERT with the following sections: Incident
controller, Evacuation controller, Security controller and historian.
d. Call Safety Officer, production coordinator, and the HR Head in order to plant soon
e. To activate crisis and alert management.
f. To ask IC to ensure all employees and outsourcing can be safely evacuated.
g. To ask IC to prepare for evacuation and chlorine leak handling.
h. To give production operator in charge direction whether must stop production or not.
i. To ask SC guards to tighten access to site.
j. To coordinate with IC during incident. Monitor the progress of evacuation, chlorine leak and first
aid.
k. To state the emergency situation has been under control. Confirm employees may go back to work
normally.
2. Incident and evacuation Controller
a. To gather at CLAP and receive direction from EM.
b. To ask Gas Detection Officer to guide the limit of hot zone and the clear and safe area.
c. To ask equipment Team to stand-by with safety line and then assist gas detection officer.
d. To guide Responder Team wearing SCBA and PPA correctly and securely.
e. To brief Responder Team to draw up sequential actions safely before handling chlorine leak.
f. To give signal to Responder Team to move after Gas Detection Team finished applying safety line.
To monitor usage time of SCBA used by responder.
g. To monitor the progress of evacuation and chlorine leak handling during incident and coordinate
with EM at the safety line.
h. To ensure alarm and scrubber system shutting off after situation under control or normalized.

To ask equipments Team identifying all equipments or tools used during incidents.
To check and identify all employees gathering at CLAP.
Report immediately if personnel identified missing.
Report the progress of evacuation to EM.
To ensure the path of evacuation line is clearly visible and no obstacle during evacuation.
To keep all employees staying at CLAP unless situation has been stated safe and secure

3. Responder
a. To join at CLAP and receive IC directions regarding steps and sequential actions to be undertaken.
b. To wear SCBA and PPA properly and correctly.
c. To control clean air usage of SCBA is safe enough to supply during mission.
d. Change SCBA cylinder with a full one if the content has run out.
e. Prepare for mission to handle chlorine leak (tools and emergency kit) and cooperate with Gas
Detection Officer to measure chlorine content level.
f. To go back inside chlorine building handling chlorine leak. To confirm weather the leak problem
could be solved or not.
g. After mission completed, clean the contaminant under safety shower and gather at CLAP then
explain what they have fixed to EM.
4. Gas Detection Officer
a. To join at CLAP and receive IC instruction regarding determining safe zone.
b. To prepare protective equipments for entering the hot zone and prepare the equipments used for
pollutant detection.
c. To equip with full face mask and screen the area around chlorine building.
d. To search for the clear and safe area, make the boundary of the area by applying safety line.
e. To cooperate with responder to measure the pollutants level inside the chlorine building.
f. To keep monitoring the level of pollutant around chlorine building.
g. To confirm to IC whether the chlorine building and the environment has been in a safe and secure
level from pollutant.
5. Timer & Equipment Team
a. To join at CLAP preparing emergency equipment and identify PPEs needed for evacuation and
intervention on chlorine leak.
b. To prepare and check means of communication working properly.
c. To list in Table PPEs used and countdown the duration of SCBA used by responder.
d. To assist responders wearing PPA and SCBA properly and also to assist responders replacing
empty cylinder with full one.
e. To prepare, check and monitor equipment / tools needed by Evacuation and First Aid Team.
f. To distribute mask for employees at CLAP.
g. To collect and identify PPE and other equipment after incident is under control or situation is
normalized.
6. Security Controller
Public Relation
a. Immediately to the plant after receiving instructions from the emergency manager
b. To make coordination with EM preparing for communication with third parties when needed
assistance.
c. To contact management of external companies / outsourcing regarding to the emergency situation
occurred.
d. To make coordination with the Evacuation Team to persuade neighbours when urgently needed to
be evacuated.
Security
a. To guard and secure site preventing access from unauthorized persons more tightly.

b. To prepare data of employees, helping Evacuation Team in identification.


c. To arrange traffic of the vehicles in and out of site safely and smoothly.
d. To assist other teams when need more manpower assistance
7. Historian
a. To join at CLAP and prepare tools for recording / writing.
b. To make documentation and record steps or actions taken during incident.
c. To record time during carrying or handling tasks, interventions or process in the incident.
d. After incident controllable or situation normalized, assist EM to finish the report.

2. TRUSTWORTHY EFFORTS
A. Usage of SCBA and PPA relatively fast (2.5 minutes).
B. Openness of all participants involved during evaluation session has made identification of some defectives
and deficiencies aspects unbiased.
C. Chorine supplier was prepared for an emergency chlorine leak (Follow TGMs ERP Procedure)
D. The drill has been treated as a learning experience than a demonstration of capability.
