At 2137 hrs, patient came in from triage, with the complaint of vomiting ,Dr.

Hoffman assigned his name on this patient but did not see the patient
physically, as he just went outside the cubicle with Dr Rugya. I overheard him
telling DR Rugya to assign her name since he will go home at 2300h.
2258hrs seen by Dr. Rugya. 2335hrs endorsed patient to RN Evelyn. 0005hrs
Received patient back from RN Evelyn, awake, alert, cuddled by his father,
vomited. 0030hrs patient Midazolam 1mg IV given as ordered by Dr.
Tameem, then another dose of 1mg iv to be given at the radiology room, if
the patient is still awake, as endorsed by RN Evelyn, as ordered by Dr.
Tameem.
At 0035hrs patient is still awake, alert, so another dose of Midazolam 1mg
was given slowly through IV. 0045hrs informed Dr. Tameem, that the patient
is still awake, after 2 mg iv of Midazolam, and he told me that he will come
and see the patient. 0050hrs Dr. Tameem , came, seen and examined the
patient.
0055hrs Dr. Tameem ordered for Ketamine 5mg iv, and he told to call in ER
Peads and let somebody bring the medication. 0056hrs called ER Pediatric
Station, and RN Josephine was the one who answered the call, then I
informed her that I need Ketamine 5mg iv as ordered by DR. Tameem. 0058
RN Jeffrey came, and gave the Ketamine vial 50mg per ml. I was about to
prepare the dose with RN Jorge but Dr. Tameem told me and insisted that he
will give the dose, I also informed him that the dosage of the medicine
(KETAMINE) is 50mg/ml, and he even read it infront of me, then I go back to
the patient and hooked to our cardiac monitor, because previously we used
the cardiac monitor of the radiology department unit. 0103hrs Dr. Tameem

iv (0.5ml). 0103hrs patient deteriorated, saturating 85%, ambubagging
done,immediately sent patient to PEADIATRIC CRITICAL AREA.
