JOINT AFFIDAVIT
We, P02 SARAH GRACE MARCOJOS and P01 DOLE IBANEZ, both of legal
age, married, Filipino citizens and residents of Mercedes, Zamboanga City, Philippines
and after having been duly sworn to in accordance with law hereby depose and say
that;
1 We are a bonafide member of the Philippine National Police (PNP for brevity) and
presently assigned at Zamboanga City Police Office 9 (ZCPO9) with office address
at Barangay Ayala, Zamboanga City, detailed at Women and Children Protection
Desk (WCPD for brevity) as duty Investigator under office order no. 12345 dated
01 April 2015;
2 Sometime on 01 August 2015, on or about 2:30 in the afternoon, we were at the
WCPD, when the brother of the complainant/victim identified as JESUS
CONCEPCION (JESUS for brevity), 26 years old, a resident of Barangay La Paz,
this City, appeared and reported to our office that her sister ANA MARIA
CONCEPCION (ANA for brevity) was allegedly raped by an unknown perpetrator;
3 On or about 3:30 in the afternoon of the same date, after their complaint was
registered under IRF No.__________________, P01 DOLE IBANEZ immediately
informed the Scene of the Crime Operation (SOCO9) personnel through cellular
phone call;
4 Thus, on or about 4:00 in the afternoon of the same date after the initial interview
with reporter JESUS, we immediately proceeded at Barangay La Paz, this City, to
conduct ocular investigation where the rape incident transpired involving the
abovementioned complainant/victim ANA;
5 Upon arrival thereat, we cordoned said area by using crime-scene tape to preserve
the crime scene and/or to protect/prevent the evidence from being contaminated.
Subsequently, on or about 4:15 in the afternoon the SOCO9 personnel arrived at
the crime scene;
6 Thereafter, we took photographs of the scene, took several overview images of the
surrounding areas from multiple angles and point of view so that we can show the
scene preserved in an unaltered condition;
7 We execute this affidavit to attest to the truth of the foregoing facts based on our
personal knowledge and authentic records that we are the Police Investigators who
conducted an ocular investigation in the said location and to prove the veracity of
our statement.
