INSTRUCTIONS FOR 4S1.


4th Year Surgery Clinical Group 1
C11

One Week Penang Hospital Wards C9 C10

1. The ENTIRE 4S1 group will report to me at 0730 hours Monday at SOPD.
If I am away you will be notified. Alternate arrangements will be made for
bedside clinical sessions.
2. Attendance will be taken TWICE daily at SOPD. A log will be kept by Sister
Sandrah. You are required to sign your NAME IN FULL. Initials and other
scratchings are unacceptable.
3. You will log in at BEFORE 7.45 am and again at 12.45 to 1.00pm
EVERY DAY. Any attempt to falsify attendance records in any manner will
result in SEVERE PENALTIES. You should be in the hospital area between
these two periods, not elsewhere. You will be considered absent if any
ONE of the TWO entries per day are unrecorded OR if you were elsewhere.
80% attendance for the week is mandatory.
4. You are encouraged to be in the wards even on Public Holidays.
5. Do not consider that one hour bedside clinical session with your tutor the
most important part of your day. The time you share with your patients is.
Make every use of this time to cultivate your clinical skills. Failure to do so
will haunt you later on and you only have yourself to blame. Your choices
will determine your character.
6. Group Leaders are to have the weekly activity log with them from BEFORE
they start 4S1. Daily entries to be made and record your activities, any
absentees, as well as any comments or any unusual events. This activity
log should be with the group leader at ALL TIMES while at the hospital.
This should be submitted to Paulina by 3pm Friday on the last day of the
4S1 rotation. This is ENTIRELY CONFIDENTIAL.
7. Medical Certificates of Illnesses are only recognized only if issued by the
doctors in Outpatients or Emergency Departments of Penang Hospital, or
me. Any other medical certificate needs to be endorsed by me.
8. There is an Operation Theatre-Infection Control Module taken by the
Matron at the OT Complex at Tuesday mornings at 8.00am sharp. Do
not be late. Your clinical tutor for this day will have to work around this
time. This module is compulsory and needs certification by both the
Matron and me.
9. You will EACH submit case studies of 5 patients-history, physical
examination, differential diagnoses, investigations and management.
These are to be submitted to Paulina by 3pm Friday. You may collect them
from her on the following Monday they can be included in your portfolio.
They must be handwritten and legible. Short forms are discouraged.
10.
You are reminded that completed portfolios are to be submitted
within a week of your surgery module completion.
11.
Dress codes apply. You will be asked to leave if requirements are
not met. Punctuality is important.

12.
There may be abrupt changes in the clinical teaching sessions,
especially those taken by our hospital colleagues- this is to be expected,
service is their priority. Every attempt will be made for alternative
arrangements if possible.
