tinitus, sensacin de odo tapado, hiper o hipoacusia y otalgia. Mltiples posibilidades anatmicas o neurolgicas que, a partir de una
disfuncin muscular o articular, pueden generar
condiciones ticas, no parecieran corresponder
con los hallazgos clnicos al momento de la
valoracin1. Estas posibles vas ataen tanto el
enfoque puramente descriptivo de estructuras
anatmicas en comn para la articulacin temporomandibular (ATM) y el odo, as como tambin
vecindades entre estructuras musculares y el odo
medio2. Esta revisin pretende desarrollar bajo un
enfoque principalmente anatmico y neuroanat-

La actividad muscular normal del msculo tensor
del tmpano (msculo masticatorio) y el msculo
estapedial, est relacionada con un mecanismo
neurolgico reflejo conocido como la va centrfuga en los mecanismos nerviosos de proteccin al
trauma sonoro y la discriminacin auditiva (de
tonos bajos), adems de una compleja co-activacin durante los movimientos velo-farngeos. La
actividad anormal del msculo tensor del tmpano
(aparentemente durante los DTM) est asociada a
sntomas ticos expresados como sensacin de
odo tapado, tinitus, vrtigo e hipo-hiperacusia, sin
existir una enfermedad tica que se muestre con
uno a ms de estos sntomas, como en el sndrome
de Mnire, otitis, neurinoma del ngulo pontocerebeloso u otosclerosis, entre otros. Los cambios
en la actividad muscular intratimpnica producen
cambios en la impedancia area y lquida del odo
medio e interno, respectivamente, que pueden ser
medidos por impedanciometra y audiometra.
Los DTM son una sub-clasificacin de los
desrdenes msculo esquelticos e incluyen una
amplia serie de condiciones craneofaciales, con
etiologa multifactorial, tanto en adultos como en
nios3. Klockhoff y col. prueban la tensin simultnea de los msculos tensor del tmpano y
temporal electromiogrficamente, lo que trae una
relacin afn entre la tensin de los msculos
orofaciales, del odo y masticatorios por un mayor
tono simptico durante el estrs emocional y DTM
que comparten vas fisiopatolgicas comunes4.
La tensin emocional incrementada genera
actividad muscular masticatoria disfuncional y
consecuentemente DTM. Lam y col.5 afirman que
la prevalencia de otalgia y dems sntomas de
causas no otolgicas en pacientes con DTM vara
entre 3,5% y 42% y que la prevalencia de tinitus
en pacientes con DTM, parece ser mayor que la
encontrada en la poblacin general6. La frecuen-

te subsanada con especialidades como la otorrinolaringologa y el dolor craneofacial, que cada vez
cierran ms la brecha de esta temtica que ya iba
adquiriendo la aridez de un terreno de nadie.
CONCLUSIN
Este artculo fue desarrollado por la necesidad de
comprender la presencia de msculos masticatorios en el odo medio que puede complicar la
sintomatologa tica en presencia de un DTM,
entre otras causas estomatognticas de tipo neurolgico y mecnico. La anatoma, neuroanatoma y
fisiologa de estos msculos, adems de interesante
por s misma, se relaciona con una actividad intratimpnica de modulacin masticatoria externa que se
vera implicada en presencia de desrdenes msculo esquelticos del sistema estomatogntico.
