Responsibilities: RNAC, Case Management, Admission Team Liason for Acute Care Hospital referrals,
Restorative Program Coordinator.

RNAC responsibilities included Care Plan Development and revision as needed.

MDS scheduling with care conference coordination and transmission review to assure accuracy to Myers and
Stauffer, Case Mix monitoring to assure optimal accurate case mix relative to care delivered.

Case Management responsibilities included telephonic updates to managed care companies with associated
