State Bank of Mysore invites applications for empanelment of (I) Architects/


Consultant (II) Contractors (III) Contractors( Miscellaneous works) & (IV)
Suppliers/ Vendors under different category & grade such as (I) Architects/
Consultant : (1)Civil & Sanitary Works (2) Electrical (3) Interior Works & (4) Air
Conditioning (II) Contractors: (1) Civil & Sanitary Works, (2) Interior Works, (3)
Electrical Works (4) Water-proofing Works (5)Data networking (6)Telephone Works
& (7) A/C Works (III) Contractors for Miscellaneous Petty Repair & Maintenance
works: (1) Civil & Sanitary Works, (2) Interior Works, (3) Electrical Works (IV)
Suppliers/ Vendors: for (1)Air Conditioning, (2)Steel/ Wooden Furniture,
(3)Modular Work-Station, (4)Electrical fittings and fixtures, from reputed and
competent agencies fulfilling the prescribed eligibility criteria and other conditions.

The eligible agencies can obtain the empanelment document from our office, during
working days 10.30AM to 5.00 PM on payment of processing fee of Rs.500/- (nonrefundable) in the form of Demand Draft only drawn in favour of State Bank of Mysore
payable at Bangalore. The empanelment document may also be downloaded from our
website www.statebankofmysore.co.in. However, while submission of application the
specified fee should be enclosed.

The sealed envelop containing application duly completed in all respect superscripted as
Application for empanelment ___(category & grade to be mentioned)____ along
with fee of specified amount should be submitted to the below mentioned office on or
before 30/04/2011.

Those who are already in our panel should also apply afresh.Those who are
empanelled already but not applied to any of our tender notification for the past 2
years need not apply.The Bank reserves the right to accept or reject any application

without assigning any reasons whatsoever. Decision of Bank at all stages of empanelment
process will be final.

1. The average annual financial turnover of the applicant during the last 3 years, ending
31st March 2011, should be at least 30% of the grade limit applied for and also should
have successfully completed similar work (in the respective category applied for) as
per the following manner during the last 5 years (i.e. prior to 28/02/2011) for
Govt./Public Sector Undertakings/ Banks/ reputed public/ private sector / multinational
companies/institutions.

One work costing 80% of the value mentioned in the grade limit applied for
OR
Two works each costing 50% of the value mentioned in the grade limit applied for

Three works each costing 40% of the value mentioned in the grade limit applied for
2. (a) In respect of AC suppliers the agency should be an authorized dealer for
the make of AC dealing with.
(b) In respect of AC contractors the contractors should have dealership of
approved manufacturers as well as should have executed specified value of works
of approved makes.
3. In respect of modular workstation vendors, the agency should have their own
manufacturing/ factory unit as well as installation setup.
4. In respect of electrical fixture, the agency should be an authorized dealer for the make
of fixture dealing with.
5. In respect of network data cabling vendors, the agency should be an authorized dealer
and should have authorization for certification of the make of the data cable dealing
with.
6. The selection of the firms under different category will be based on fulfilling the
eligibility criteria mentioned above for the respective categories. However, the ability
and competence of the applicant to render required services within the specified time
frame will be a major factor while deciding the empanelment of the firms.

In respect of the Miscellaneous repair works, the applicant must have successfully
taken up and completed repair works as per the following manner during the last 3
years (i.e. prior to 28/02/2011) for Govt./Public Sector Undertakings/ Banks/ reputed
public/ private sector / multinational companies/ institutions.
All works put together for a year costing at least Rs. 2 Lacs ( for fresh applicants).
All the existing contractors who take up the repairs/ maintenance of our Bank can
apply without any financial turnover restrictions.

INSTRUCTIONS & GUIDELINES FOR SUBMISSION OF APPLICATION FORM


FOR EMPANELMENT
1. This application for empanelment is for a period of 3 years (2012 to 2014).
2. Application form for empanelment shall be submitted in sealed envelope super
scribing Application for empanelment _(category & grade to be mentioned)_
The application shall be submitted strictly as per the format enclosed (Annexure I
to V). In case of the miscellaneous repair works, the application shall be submitted

enclosing Annexure II, IV &V compulsorily. Each application shall be submitted
along with documentary proof with respect to the details furnished in the
application form regarding eligibility criteria. In this regard, copies of the work
order and completion certificate/ TDS certificates or any such documents which
supports the category , grade & eligibility applied for. Incomplete application or
application without proper proof for establishing their credentials will be
summarily rejected and no correspondence will be entertained in this regard.
3. The application shall be signed by the person/s on behalf of the organization
having necessary Authority/ Power of Attorney to do so. Copy of Power of
Attorney / Memorandum of association (Wherever applicable) shall be furnished
along with application.
4. If the space in proforma is insufficient for furnishing full details, such information
may be supplemented on a separate sheet stating therein the part of proforma and
serial number. Separate sheets may be used for each part of application. While
filling up the application with regard to list of important projects completed or on
hand the applicant shall only include major works handled by the firm.
5. Applications containing false and / or inadequate information are liable for
rejection, besides action for misrepresentation. Canvassing of any sort may also
disqualify the applicant.
6. The applicant is required to sign the empanelment document as token of
acceptance of the contents mentioned there at by himself or through an authorized
signatory duly enclosing the power of attorney / authorization letter.
7. The empanelment shall be made for a specific category and grade. However, the
firm empanelled for a specific category in higher grade shall be eligible for the
works in the lower grade of the same category.
8. The applicant should not have any discouraging/adverse report against their past

performance.
9. The applicant who intends to apply for more than one category, have to apply for
each category separately using the same application form. Photocopies of the
format can be used for various categories of work by the same applicant.
10. The Bank shall approach the applicants clients, corporations, organizations, etc. to
verify the applicants general reputation / competence.
11. The Bank reserves the rights to accept or reject any application without assigning
any reason.

13. The Bank reserves the right to amend the selection criteria depending upon the
applications received in response to the Advertisement and Banks other
requirements.
14. The Bank reserves right to accept or reject any or all the application without
assigning any reasons whatsoever and decision of the Bank in regard to selection
of the firm shall be final.
