LETTER OF MANDATE FOR INTERNET BANKING FACILITIES

Sir/Madam,
We refer to the account opened by us in the firm name and style of.

Bank Ltd. (Bank) Pursuant to and in partial modification of the authority given
by us to the partners authorized to operate the said account(s), in order to
facilitated operation of the said account(s), we hereby request the Bank to allow

partners to operate the said account through Internet Banking facility (doing so
singly) by using the login ID and PIN mailers, on Terms and Conditions detailed
hereunder which has been read and understood by us. We acknowledge that the
same are in addition to and not in the Terms and Conditions relating to the said
account(s) and Internet Banking facility.
(1) Mr. /Ms.
will be solely entitled to
receive the customer Login ID and PIN mailers to access the Internet Banking
facility and to acknowledge the same.
(2) The customer Login ID and PIN mailer shall be kept totally secret and
confidential by them.
(3) All transactions carried out in the said account through use of the Internet
Banking facility shall be binding on all the partners and we shall keep the Bank
saved and harmless from all actions, claims, demands, proceedings, losses,
damages, costs, charges and expenses whatsoever which Bank may a any time
incur, suffer, or sustain or to be put to as a consequence of or by reason of or
arising out of transactions carried out through the Internet Banking facility.
(4) Bank shall at all times be informed of any changes in the constitution of the
partnership by furnishing necessary documents and in writing.
(5) Bank shall not be responsible and liable to monitor any debits made in the
Account by the use of Internet Banking facility.
(6) This writing shall be enforceable against all the partners and shall not be
affected by any change in the constitution of the partnership and shall be binding
on all the partners.

I / We affirm, confirm and undertake that I / We have read and understood the
Terms and Conditions for usage of the Internet Banking service of Bank, as
displayed on the website www.suratnationalbank.com, and that I / We agree to
abide by them.

I / We hereby state that should I / We to revoke the above authorization, I / We

shall duly issue a letter of revocation (the revocation letter) to Bank in this
regard. I / We hereby agree that until ten days after receipt of such revocation
letter, the authorization as aforestated shall hold good.
