1) Operations and systems will be planned and the designed to eliminate or materially reduce
mining hazards.
2) Implement statutory rules and regulations and strenuous efforts made for achieving
superior standards of safety.
3) To bring about improvement in working conditions by suitable changes in Technology.
4) Provide all material and monetary resources needed.
5) Deploy safety personnel wholly for accident prevention work.
6) Organize appropriate forums with employees representative for joint consultations on
safety matters and secure their motivation and commitment in safety management.
7) Prepare Annual safety Plan and long term safety plan at the beginning of every calendar
year unit wise and for the Company to affect improved safety in operations as per
respective geo-mining needs to prepare the units for onset of monsoon, to fulfill
implementation of decisions by committee on Safety in Mines and the 5 th, 6th, 7th, 8th & 9th
Safety conference and to take measures for over-coming accident proneness as may be
reflected through study of accident analysis, keeping priority in sensitive areas of roof falls,
haulage, explosives, machinery etc.

8) Set up a frame work for execution of the safety policy and plans through the General
Manager of Areas, Agents, Managers and other safety personnel of the units.
9) Multi-level monitoring of the implementation of the safety plans through internal safety
organization at the company Hqtrs. And Area Safety Officers at the area level.
10) All Senior Executives, at all levels of Management, will continue to inculcate a safety
consciousness and develop involvement in practicing towards accident prevention in their
functioning.
11) Institute continuous education, training and retraining of all employees with the accident
placed on development of safety oriented skills.
12) Continue efforts to better the living conditions and health of all the employees in and
outside the mines.

BRIEF INTRODUCTION;

Chhal Opencast Mine is situated in the Raigarh of SECL.The mine is about 18


Kms away from the Kharsia railway station On the Bombay- Hawrah railway line. It can be
approached by Dharamjaygarh Kharsia National Highway. The mine is situated between
the longitudes- 83*0610 ___ 83*0910 and latitude- 22*0440___22*0621. The Mine
has two quarries namely Quarry No-1 and Quarry No-2. The extraction of coal from quarry
no-01 is completed, is being backfilled by the overburden of quarry no-02. At present the
extraction of coal is being done from the quarry no-02.

Arrangement of Communication
System.
The following communication systems are followed at Chhal OCM:
1. Exchange of information between shifts :a)
Shift In charge hand over charge to shift In charge of next shift,
Production In charge and Manager.
b)

Over man hand over charge to over man of next shift.

c)

Mining Sirdar hand over charge to Mining Sirdar of next shift.

2. Communication in the event of Risks :a)

Any person noticed any risk should inform it to any official i.e. over
man, mining sirdar or any other official present therein.

b)

Informed present official inform it to Manager or any principal officer


present in the mine.

c)

Informed risk shall also be confirmed by one or more messenger.

d)

Communicate to Attendance clerk then Attendance clerk shall


communicate it to the Manager or principal officer present therein.

Safety Performance Evaluation System.

1. Pit Safety Committee:- On the 5th day of every month all Pit Safety Committee members
inspect the mine and then after a meeting is held. In the course of meeting, new
circulars if any discussed, recommendations of the last meeting reviewed, findings
of the present inspection discussed and responsibility fixed.
2. Area Safety Board:- Generally on the 1st day of every month Area Safety Board
members inspect one of the mine in the area alternately and a meeting is held after
the inspection. In the course of meeting, new circulars if any discussed,
recommendations of the last meeting reviewed, findings of the present inspection
discussed and responsibility fixed.
3. Workman Inspector:- Workman Inspector of different discipline i.e. mining,
mechanical and electrical inspect the mine as per statute i.e. at least two days
in a week and record the results of inspections in form U. The manager
countersigns it at least once in every 15 days stating compliance.
4. I.S.O. Inspections:- I.S.O. Officials inspect the mine, recommendations and
circulars of I.S.O. are strictly complied.
5. D.G.M.S. Inspections:-

- D.G.M.S. Inspectors of different discipline i.e. Mining, Mechanical


and Electrical inspect the mine and give their good suggestions or recommendations to
improve the safety of the mine and in the field of related activities.
5. Annual Safety Inspections:-

Every year an annual safety inspection is conducted by an annual safety


inspection team constituted members from DGMS, ISO, Area and unit
level officers and officials and from different mining companies. The
team inspects the mine and evaluates the performance of the safety in all
respect.

The VTC trained & Initial Medical Examined workers are issued Identity
Cards, when they produce above said certificates and kept it for official

Their name and address along with all necessary informations to be entered
in the B form kept at managers office.
Then their name, fathers name and B form number is entered into D form
or daily attendance register maintained by MTK or a person provided by the
contractor and it is to be produced in front of the colliery manager at the end
of every month for its verification a list of daily deployment is produced in
front of the Shift I/C every day.

6. The deployment of machines as well as contractual workers and supervisors

are made as per the agreement or statute there in.


7. The contractors are bound to give Safe Operating Practices (SOP) to the
management and their supervisors and workers.
8. The contractors are bound to provide their employees all necessary Personal
Protective Equipment as per the statute.
9. The Shift I/C and the supervisors would fulfill the requirement of statutory
parts made in the SOP.
10. Colliery manager, Safety officer, all executives and supervisors give them
safety talk or safety awareness, operation or safe operation practices in the
mine. All kind of instructions given to the contractual workers to be kept in
bound paged book for its record.
11. As per the requirement of VT Rule & Mines Rules contractors are asked to
sent their workers once in every 5 years for Refresher Training, Special
training whenever required and medical re-examinations once in every five
years workers bellow 45 years and once in every three years workers above 45
years as per statute.
12. The effectiveness or the compliances of the agreement made there in is
checked by the shift I/C, shift supervisors and any executives or supervisor
any time any where if it is required without giving any information to the
contractor.

FORMATION OF RISK ASSESSMENT


TEAM
Under the guidance of Manager of Chhal Open Cast Mine and in consultation with General
Manager (Mining)/SAM of the Chhal Sub Area the strategy for Hazard Identification and Risk
Assessment was prepared, which is as follows:-

Strategy adopted

All the operations of the mine are to be divided into appropriate categories.
Activities in each category are to be listed out.
Different groups involving persons from all levels are to be formed.
Each group will be assigned a compatible category of works for hazard identification and
risk assessment.
These groups are to be taught the principles and processes of hazard identification and risk
assessment. Practical training has to be given to these groups in the processes of risk
assessment.
Each group, as per the instructions of their group leader will meet regularly, list out the
activities/operations involved in their category.

After the activities for which risk assessment has to be done been identified, the group will
formally conduct the risk assessment.
The methods to be followed for risk assessment are: Job Safety Analysis (JSA)
Workplace Risk Assessment and Control (WRAC)
Fault Tree method of Risk Assessment
The group will decide what method will be applied for each activity as per the suitability.
All the risk assessment processes shall be documented.

RULE - 1: PARKING & STANDING


(A) No dumper shall park or stand except at designated places ,which be flat, well
lighted, need not be reversed for taking out and have proper layout. Road layout shall
be demarcated.
(B) It is to be parked due to breakdown,
(a) A driver shall not park his vehicle in a position that will endanger other traffic in the area.
(b) A driver shall not park or stand his vehicle within 30 m. of the working areas of mobile
equipment.
(c) A driver shall not park or stand his vehicle within the area of swing of dragline without
obtaining prior permission from the dragline operator/
(d) A driver before leaving his parked vehicle, shall ensure that

designated place shall be well lighted, well demarcated and shall not have road

(c) a dumper can overtake a water tanker at a designated place
(d) Overtaking shall be done only after giving designated signal to the vehicle being
overtaken and also after receiving designated acceptance signal Before allowing
overtaking the operator shall ensure that the adequate clearance is there in the front
and on the side.

(a) Reversal of dumper, water tanker or any other vehicle shall be done only designated
places, which may be loading and unloading station of dumpers, Even to attend
nature calls he shall park at the designated parking places where provision of
drinking water, canteen facilities, toilets etc, shall be provided,

(b) Dumper water tanker or any other vehicle shall not be reversed under any
circumstances on a haul road.

RULE 4-SIGNS
(a) Clear and distinct signal shall be provided where a possibility or collision of a
dumper with other dumper or structure or even berm/divider etc.
(b) Every sign shall be clearly visible from a distance of more than 30 in the signal post
shall not be in the direct of the dumper.
(c) All signs to be, obeyed as read.
(d) Warning signs, Including the daily Blasting notice, are to be posted and noted at all
times.

(a) Small vehicles shall not be allowed in the haul roads, A separate road shall be made
to small vehicles. If no such road has been made, and till a separate road is made only

(b) Two wheeler and unauthorized small vehicle shall not be permitted on haul roads,

fitted.

d) A caution sign or hazard lights are to be placed a the front and rear in any vehicles
being tower or which broken and obstructed the roadway.
(e) All earth moving equipment, sound one blast or horn before loving off from parked
position.
(f) Every helmet shall have a red fluorescent band all around and without which no one

shall be allowed inside the mine.

operator for operation.

Fitter/Foreman.
4. Before starting the dumper, operator shall make physical inspection around the
dumper to ascertain the status of dumper and check for the efficiency of safety
provisions fitted with the dumper.
5. The operator shall ensure presence of persons and other equipment if any nearby
and shall blow horn/ AVA before moving out the dumper from parking yard.
6. The operator shall move the dumper on the specified haul road (separate road for
HV) provide for the purpose with the perimitted speed limit.
7. While placing the dumper for loading, the operator shall correctly place his
dumper at the cleaned/ and lvelledgraded loading point witrh clear visibility and as
indicated by the loading machine operator.
8. While reversing the dumper for loading, he shall place his dumper away from
active working place with use of rear view mirror, AVA and as per instruction of
supervisor.
9. During operation of the dumper if any defect in brakes, lights, AVA, RVM or
warning system is noticed, the operator shall stop the vehicle immediately and
report the same to the fitter, foreman or engineer.
10. The dumper operator shall strictly follow the provisions of Traffic Rules
applicable in the mine.
11. The operator shall not permit riding of his dumper by any unauthorized person.
12. While moving in down gradient, he shall move his dumper with low gear.
13. The dumper shall not be parked on gradient. But on unavoidable circumstances,
dumper shall be parked using gear in addition to parking brake & suitable safety
appliance must be used. It should be informed to supervisor immediately.
14. Befoe bladsting, the operator shall park his dumperat a safe place away from the
danger zone.
15. At the end of shift work while moving towards the contractors workshop, the
operator shall use the specified haul road and speed limit.
16. After reaching the parking yard, the operator shall park his dumper at a specified
level and stable ground with applying parking brake and other safety appliances.

1. The dozer operator shall be competent, properly trained and authorized by the
manager.
2. Preshift checking of dozer by Fitter & Foreman before dozer is deployed for
operation.
3. Record of such checking shall be maintained at workshop by Fitter/Foreman.
4. Before starting the dozer, operator shall make physical inspection around the dozer
to ascertain the status of dozer and check for the efficiency of safety provisions
fitted with the dozer.
5. The marching of dozer shall be carried out under supervision of a supervisor.
Separate road for movement of dozer shall be ensured.
6. The dozer operator shall not march the dozer in reverse gear for a long distance.
7. The dozer operator shall strictly follow the provisions of Traffic Rules applicable
in the mine.
8. While pushing/dozing of dump material is done, the working area shall be
inspected by a supervisor before deployment of dozer and only skilled operator to
be engaged and adequate berms should be provided.
9. While reversing of dozeroperator shall blow horn/AVA and follow the instruction
of supervisor.
10.During the dozing of faces, all activities are stopped at the faces proximity to the
dozer.
11. If the dozer is deployed to deal with fire, cleaning of oil and grease from the
dozer shall be ensured. Adequate fire quenching apparatus, pressurized water
tanker should be made available and the operation should be carried out under
supervison.
12. Incase of any defect noticed during operation, dozer operator shall stop the
operation immediately and informit to the supervisor/foreman/fitter.
13. At the enbd of shift, dozer operator shall park he dozer at specified place with
applying safety appliances.
14. The dozer operator shall ensure that the blade of dozer is placed on the firm
ground.

3. Standrd Operating Procedure for Blasting Operation.


1. Before starting any blasting operation, explosive carriers/blasting crew shall
consult with bslasting officer or blasting overman and perform their duties as per
instruction received from their officials.
2. Blasting overman shall deploy only those persons who has adequately trained and
authorized by the manager.
3. Transportation of explosive and accessories shall be done as per code of practice
and in explosive van only. Detonators and accessories shall be kept in locked
condition in a separate compartment under supervision of blasting overman.
4. Record of explosive and accessories used shall be maintained at work place as
well as at magazine.
5. Explosive van shall not be used for transportation of blasting crew or other means.
6. The speed of van carrying explosive and accessories shall not be more than
specified speed limit for the mine.
7. Charging of explosive shall be done under direct supervision of blasting overman.
8. Incase of presence of water in blast hole, slurry mixed explosive (SME) shall be
lowered in the hole cautiously and stemming should be made adequately under
supervison.
9. Blast hole parameters like burden, spacing, depth of hole, charge per hole,
stemming height, etc. shall be designed so as to improve blast performance and
reduce vibtration.
10. No coal dust shall be used as stemming material.
11. Blasting overman shall ensure that the shotfiring cable of at least 300m length
and proper delay is used.
12. All entries leading to the blast site are to be guarded before blasting. If required
manpower should be arranged by diverting from other heads.
13. Warning by siren/hooter, caution board and posting of guards at entry points to be
ensured before blasting.
14. There shall be used clear two way communication for final warning and giving
clearance before blasting.
15. Blasting overman shall ensure that all machineries, persons including blasting
crew have been taken shelter at safe place and adequately guarded all entry points
before blasting.
16. Blasting shall be done within the specified blasting time i.e. from 1.0pm 3.0pm
with controlled blasting technique.
17. Inspection of blasted site shall be done after 5 minutes of blasting by blasting
overman and misfire if any should be dealt with as specified in the statute.
18. The work shall be resumed only after giving clearance from the blasting
overman.
19. Balance explosive and accessories shall be deposited at the magazine.

1. The tipper operators/drivers shall be competent, properly trained and authorized
by the manager.
2. Preshift checking of tippers by Fitter & Foreman before tipper is handed over to
the operator for operation.

Fitter/Foreman.
4. Before starting the tipper, operator shall make physical inspection around the
tipper to ascertain the status of tipper and check for the efficiency of safety
provisions fitted with the tipper.
5. The tipper driver shall blow horn/AVA before moving out the tipper from parking
yard/bay.
6. The driver shall move the tipper on the specified haul road with the permitted
speed limit.
7. At the right turn/blind head if any provisions of speed limit and caution boards
shall be displaced at the curve.
8. The driver shall not permit riding of his tipper by any unauthorized person.
9. The tipper operator shall strictly follow the provisions of Traffic Rules applicable
in the mine.
10.While reversing the tipper for loading, he shall place his tipper away from active
working place with use of rear view mirror, AVA and as per instruction of
supervisor.
11. At the weighbridge, tipper drivers shall be prohibited to alight and provision of
trucks in queue system for weighment shall be followed.
12. The tipper driver shall not dump coal where loading operation is going
on.Dumping and loading of coal shall be done separately under supervision of a
supervisor.
13. At the place where crossing of vehicles is unavoidable, provision of check gate
with security guards, caution boards shall be provided effectively.
14. During operation of the tipper if any defect in brakes, lights, AVA, RVM or
warning system is noticed, the tipper driver shall stop the vehicle immediately and
report the same to the fitter, foreman or engineer.
15. The driver shall not park the tipper on gradient. But on unavoidable
circumstances, dumper shall be parked using gear in addition to parking brake &
suitable safety appliance shall be used. It should be informed to supervisor
immediately.
16. At the end of the shift, the tipper driver shall park his vehicle at specified place
and on stable ground with applying parking brakes and other safety appliances.

5. Standrd Operating Procedure for Pay Loader Operation.


1. Only competent persons, properly trained and authorized by the manager shall be
appointed as Pay Loader Operator.
2. Preshift checking of pay loaders by Fitter & Foreman shall be done before handed
over to the operator for operation.
3. Record of such checking and list of ok pay loaders shall be maintained at
workshop by Fitter/Foreman.
4. Before starting the pay loader, operator shall make physical inspection around the
pay loader to ascertain presence of any persons or equipment /object and check for
the effectiveness of safety provisions fitted with the pay loader.
5. Before starting the pay loader for loadingoperation, operator shall blow a long
horn.
6. Pay loader operator and supervisor hsall notallowpersons or any equipment within
the danger zone i.e. 50m in radius from its position.
7. The operator shall strictly follow the provisions of Traffic Rules applicable in the
mine.
8. Placement of pay loader w.r.to tippers at loading point, the loadersshall be so
placed that the truck/tipper operator shall always able to see the loaders cabin.
9. The operation of pay loader shall be restricted to the height of the less than the
maximum digging height of the loader.
10. The loading operation shall be carried out under supervision of a supervisor.
Truck drivers shall not be permitted to get down from the vehicle.
11. Marching of pa loader shall be done under supervision of a supervisor.
12. At the end of shift, the operator shall park his vehicle at specified level and on
stable ground with applying parking brakes and other safety appliances.
13. The operator shall keep with him the ignition key and handed over it to the
security guard.

6. Standrd Operating Procedure for Errection of Power Supply Line.

1. A team consists of Engineer, Foreman and Survyor shall inspect the


appropriate site for errrection of electric pole.
2. Digging operation shall be done by skilled personnels under supervision of
Foreman/Engineer with proper tools.
3. Suitable equipment or vehicle ashall be used for transportation of pole and
conductors under the guidance of foreman/Engineer.
4. The errection of electrical pole shall be performed by competent adequate
personnel and equipment/tools undr supervision.
5. While working at height ( coupling brackets and insulators), mobile ladder
with cabin of adequate boom length, adequate personnel and equipment, use
of personal protective equipment and safety belts shall be ensured.
6. Working in proximity of another live line, proper shut down procedure, safe
distance from other line shall be maintained.
7. While working on pole during rainy day is performed, special maintenance
of OH lines before onset of monsoon for avoiding work on poles during
heavy wind, rain and storm and lightening shall be made.
8. If any break down occurs in rainy season, maintenance work during heavy
wind, rain, storm and lightening should be stopped.
9. At the end of work detailed information shall be given to the foreman
/Engneer and record in a bound paged book for ready reference.

objects and blow horn/ AVA to avoid any mishappenings.
6. Incase of any stoppage of tanker, driver should get switch off the tanker with
applying parking brake.
7. The driver shall not permit riding of his tanker by any unauthorized person.
8. The tanker driver shall strictly follow the provisions of Traffic Rules applicable in
the mine.
9. The AVA system and rear view mirrors should be maintained in proper working
order.
10. During the working ofwater tanker for opening spray pipe vale, the driver shall
park the tanker at level and sable ground with applying parking brakes.
11. While tanker is engaged for dealing with fire fighting/quenching at coal face or
coal dump, the tanker shall be placed at safe place. Athe operation should be under
supervision of a supervisor and followed COP for fire fighting.
12. At the end of shift, tanker shall be parked at specified level and on stable ground
with applying parking brakes and other safety appliances.
13. The driver shall keep with him the ignition key and handed over it to the security
guard.

8. SOP for Human Attributes/Improving human Behaviour.


1. No persons shall be deployed /engaged in any work within the
mine premises unless he has undergone vocational training and
medicaly examined by a qualified medical practitioner and found
medically fit.
2. For increasing safety awareness, improving alertness & human
behaviuor and effective use of personal protective equipment,
regular training programmes should be organized.
3. Persons shall not be allowed to mark attendance in the drunken
state. Proper mechanism to be provided at the MTK office to test
any suspect in drunken state.
4. No persons shall be allowed to use mobile/ cell phones while
driving a vehicle or operating equipment.
5. Over speed of vehicles should not be permitted in the mine
premises.
