Urinary incontinence is loss of bladder control. Symptoms can range from mild leaking to
uncontrollable wetting. It can happen to anyone, but it becomes more common with age.
Most bladder control problems happen when muscles are too weak or too active. If the muscles that
keep your bladder closed are weak, you may have accidents when you sneeze, laugh or lift a heavy
object. This is stress incontinence. If bladder muscles become too active, you may feel a strong urge
to go to the bathroom when you have little urine in your bladder. This is urge incontinence or
overactive bladder. There are other causes of incontinence, such as prostate problems and nerve
damage.

Urinary incontinence is the inability to control the release of urine from your bladder. Some people
experience occasional, minor leaks or dribbles of urine. Others wet their clothes frequently.
Types of urinary incontinence include:
Stress incontinence. This is loss of urine when you exert pressure stress on your
bladder by coughing, sneezing, laughing, exercising or lifting something heavy. Stress incontinence
occurs when the sphincter muscle of the bladder is weakened. In women, physical changes resulting
from pregnancy, childbirth and menopause can cause stress incontinence. In men, removal of the
prostate gland can lead to stress incontinence.
Urge incontinence. This is a sudden, intense urge to urinate, followed by an involuntary loss
of urine. Your bladder muscle contracts and may give you a warning of only a few seconds to a minute
to reach a toilet. With urge incontinence, you may need to urinate often, including throughout the
night. Urge incontinence may be caused by urinary tract infections, bladder irritants, bowel problems,
Parkinson's disease, Alzheimer's disease, stroke, injury or nervous system damage associated with
multiple sclerosis. If there's no known cause, urge incontinence is also called overactive bladder.
Overflow incontinence. If you frequently or constantly dribble urine, you may have overflow
incontinence, which is an inability to empty your bladder. Sometimes you may feel as if you never
completely empty your bladder. When you try to urinate, you may produce only a weak stream of
urine. This type of incontinence may occur in people with a damaged bladder, blocked urethra or
nerve damage from diabetes, multiple sclerosis or spinal cord injury. In men, overflow incontinence
can also be associated with prostate gland problems.
Mixed incontinence. If you experience symptoms of more than one type of urinary
incontinence, such as stress incontinence and urge incontinence, you have mixed incontinence.
Functional incontinence. Many older adults, especially people in nursing homes, experience
incontinence simply because a physical or mental impairment keeps them from making it to the toilet
in time. For example, a person with severe arthritis may not be able to unbutton his or her pants
quickly enough. This is called functional incontinence.
Total incontinence. This term is sometimes used to describe continuous leaking of urine, day
stress incontinence, which is the involuntary loss of urine during actions-- such as
coughing, sneezing, and lifting -- that put abdominal pressure on the bladder
urge incontinence, which is the involuntary loss of urine following an overwhelming urge
to urinate that cannot be halted
overflow incontinence, which is the constant dribbling of urine usually associated with
urinating frequently and in small amounts

Nerve Problems
Any disease, condition, or injury that damages nerves can lead to urination problems. Nerve
problems can occur at any age.
Men who have had diabetes for many years may develop nerve damage that affects their
bladder control.
Stroke, Parkinson's disease, and multiple sclerosis all affect the brain and nervous
system, so they can also cause bladder emptying problems.
Overactive bladder is a condition in which the bladder squeezes at the wrong time. The
condition may be caused by nerve problems, or it may occur without any clear cause. A
person with overactive bladder may have any two or all three of the following symptoms:
urinary frequency -- urination eight or more times a day or two or more times at
night
urge incontinence -- urine leakage that follows a sudden, strong urge to urinate
Spinal cord injury may affect bladder emptying by interrupting the nerve signals required
for bladder control.
Prostate Problems
The prostate is a male gland about the size and shape of a walnut. It surrounds the urethra just
below the bladder, where it adds fluid to semen before ejaculation.
BPH: The prostate gland commonly becomes enlarged as a man ages. This condition is
called benign prostatic hyperplasia (BPH) or benign prostatic hypertrophy. As the prostate
enlarges, it may squeeze the urethra and affect the flow of the urinary stream. The lower
urinary tract symptoms (LUTS) associated with the development of BPH rarely occur before
age 40, but more than half of men in their sixties and up to 90 percent in their seventies and
eighties have some LUTS. The symptoms vary, but the most common ones involve changes
or problems with urination, such as a hesitant, interrupted, weak stream; urgency and leaking
or dribbling; more frequent urination, especially at night; and urge incontinence. Problems
with urination do not necessarily signal blockage caused by an enlarged prostate. Women
don't usually have urinary hesitancy and a weak stream or dribbling.
Radical prostatectomy: The surgical removal of the entire prostate gland -- called
radical prostatectomy -- is one treatment for prostate cancer. In some cases, the surgery may
lead to erection problems and UI.
External beam radiation: This procedure is another treatment method for prostate
cancer. The treatment may result in either temporary or permanent bladder problems.

Over the past month or so, how often have you had a sensation of not emptying your
bladder completely after you finished urinating?

Your answers to these questions may help identify the problem or determine which tests are
needed. Your symptom score evaluation can be used as a baseline to see how effective later
treatments are at relieving those symptoms.

If changing medicines is not an option, your doctor may choose from the following types of drugs
for incontinence:
Surgical Treatments
Surgical treatments can help men with incontinence that results from nerve-damaging events,
such as spinal cord injury or radical prostatectomy.
Artificial sphincter: Some men may eliminate urine leakage with an artificial sphincter,
an implanted device that keeps the urethra closed until you are ready to urinate. This device
can help people who have incontinence because of weak sphincter muscles or because of
nerve damage that interferes with sphincter muscle function. It does not solve incontinence
caused by uncontrolled bladder contractions.
Surgery to place the artificial sphincter requires general or spinal anesthesia. The device has
three parts: a cuff that fits around the urethra, a small balloon reservoir placed in the
abdomen, and a pump placed in the scrotum. The cuff is filled with liquid that makes it fit
tightly around the urethra to prevent urine from leaking. When it is time to urinate, you
squeeze the pump with your fingers to deflate the cuff so that the liquid moves to the balloon
reservoir and urine can flow through the urethra. When your bladder is empty, the cuff
automatically refills in the next 2 to 5 minutes to keep the urethra tightly closed.

Functional Incontinence
Urinary incontinence, the involuntary loss of urine, affects an estimated 25 million Americans, mostly
women. For most, incontinence is the result of problems controlling the bladder. For people with a
type called functional incontinence, however, the problem lies in getting to and using the toilet when
the need arises.
Causes and Symptoms of Functional Incontinence
There are many possible causes of functional incontinence. Often, the cause is a problem that keeps
the person from moving quickly enough to get to the bathroom, remove clothing to use the toilet, or
transfer from a wheelchair to a toilet. This includes musculoskeletal problems such as back
pain or arthritis, or neurological problems such as Parkinson's disease or multiple sclerosis (MS).
Further compounding the problem, restroom facilities aren't always easy to get to or may not be set
up for people with disabilities.
In other cases, functional incontinence may result from problems with thinking or communicating. A
person with Alzheimer's disease or other forms of dementia, for example, may not think clearly
enough to plan trips to the restroom, recognize the need to use the restroom, or find the restroom.
People with severe depression may lose all desire to care for themselves, including using the
restroom.
Sometimes medications can cause functional incontinence. For example, if strong sedatives cause
grogginess, the person may not recognize the need to use the restroom until it's too late.
Although there are several types of incontinence, doctors suspect a diagnosis of functional
incontinence when there are other medical conditions that make getting to the bathroom
difficult. However, a physical exam, medical history, and medical tests may be necessary to confirm
the diagnosis or determine if other forms of incontinence are present that require treatment. For
example, Parkinson's incontinence and multiple sclerosis incontinence may be a combination of
functional incontinence and another form of incontinence called urge incontinencethat occurs because
damage to the nerves supplying the bladder, the nervous system, or bladder muscles make urine
control difficult.
Diagnosis and Treatment of Functional Incontinence
Treating functional incontinence requires treating the medical conditions that cause or contribute to
the problem. For example, appropriate treatment for arthritis may make it easier to get to the
bathroom quickly. Treatment also requires addressing factors in the environment to improve
accessibility.
If you suffer from functional incontinence, there are things you can do to reduce your risk of accidents.
At home, make sure your bathroom is accessible and the route from your bathroom is uncluttered,
which could help you avoid delays or falls. When out and about, know where the restrooms are so you
will not have to take time to ask directions or locate one when you need to go. Wear clothing that is
easy to remove. For example, if arthritis in your fingers makes it difficult to work zippers, wear pants
with elastic waists. If you have trouble transferring from a wheelchair to toilet, try to have someone
with you who can help.
Often, simple behavioral treatments that help other forms of incontinence can be helpful for reducing
accidents caused by functional incontinence as well. These treatments include:
Bladder training: This technique involves scheduling the amount of time between bathroom trips.
You'll initially start by going every two hours. If you feel the need to go between trips, you should stand
or sit still, contract the pelvic muscles, and concentrate on making the urge to urinate go away. Once
the urge is under control, you can go to the bathroom and urinate. After you have stayed dry for two
days, you should slowly increase intervals until you are able to go three to four hours without using
the bathroom.
Pelvic muscle exercises: Pelvic muscle exercises, also called Kegel exercises, strengthen the
muscles that support the bladder and urethra to prevent leakage. To do Kegel exercises you should
focus on isolating your pelvic muscles, so that contractions are in these muscles. To learn how to do
Kegel exercises, go to the bathroom and urinate. Halfway through, try to stop the stream of urine. This
will help you identify the muscles you need to contract. Once you identify the muscles, do not practice
while urinating. Do the exercises for about five minutes at a time as you go about your day. After a few
weeks to a month you should start to notice some improvement. Practicing Kegel exercises may be
helpful in preventing accidents as you try to get to the bathroom.
In addition to doing specific exercises to strengthen the pelvic floor muscles,walking can be helpful
because it improves your sense that the bladder is filling. This may enable you to recognize the need
to urinate and head toward the bathroom before it's too late.
Scheduled, timed, or prompted voiding: This involves setting a schedule for bathroom trips. If you
make regular trips to the bathroom, you may be less likely to have to go in a hurry. In nursing homes,
scheduled or prompted voiding can be helpful for dementia patients, who may not recognize when
they need to use the bathroom. Bathroom trips can be planned for times when the patient's bladder is
likely to be full.
Although these methods require motivation and effort, for many people with functional incontinence,
they can make a difference between having accidents and staying dry.
