Abstract
Antibioticshavebeenintensivelyusedoverthelastdecadesinhumanandanimaltherapyand
livestock,resultinginseriousenvironmentalandpublichealthproblems,namelyduetothe
antibioticresiduesconcentrationinwastewatersandtothedevelopmentofantibioticresistant
bacteria.Thisstudyaimedtoaccessthecontributionofsomeanthropologicalactivities,namely
urbanhousehold,hospitalandawastewatertreatmentplant,tothespreadofantibiotic
resistancesinthetreatedwastewaterreleasedintotheMondegoRiver,Coimbra,Portugal.Six
samplingsiteswereselectedinthewastewaternetworkandintheriver.Theampicillinresistant
Enterobacteriaceaeofthewatersampleswereenumerated,isolatedandphenotypically
characterizedinrelationtotheirresistanceprofileto13antibiotics.Someisolateswereidentified
intospecieslevelandinvestigatedforthepresenceofclassAandclassClactamases.Results
revealedhighfrequencyofresistancetothelactamgroup,cefoxitin(53.5%),
amoxicillin/clavulanicacidcombination(43.5%),cefotaxime(22.7%),aztreonam(21.3)cefpirome
(19.2%),ceftazidime(16.2%)andtothenonlactamgroup,trimethoprim/sulfamethoxazol
(21.1%),tetracycline(18.2%),followedbyciprofloxacin(14.1%).Thehospitaleffluentshowedthe
higherratesofresistancetoallantibiotic,excepttwo(chloramphenicolandgentamicin).Similarly,
higherresistanceratesweredetectedinthewastewatertreatmentplant(WWTP)effluent
comparedwiththeuntreatedaffluent.Regardingthemultidrugresistance,thehighestincidence
wasrecordedinthehospitalsewageandthelowestintheurbanwaste.Themajorityofthe
isolatesaltogetherarepotentiallyextendedspectrumlactamasespositive(ESBL(+))(51.9%),
followedbyAmpC(+)(44.4%)andESBL(+)/AmpC(+)(35.2%).Themostprevalentgenesamong
thepotentialESBLproducerswereblaOXA(33.3%),blaTEM(24.1%)andblaCTXM(5.6%)and
amongtheAmpCproducerswereblaEBC(38.9%),blaFOX(1.9%)andblaCIT(1.9%).In
conclusion,thehospitalandtheWWTPactivitiesrevealedtohavethehighestcontributiontothe
spreadofmultidrugresistantbacteriainthestudyarea.Suchdataisimportantforfuture
managementoftheenvironmentalandpublichealthriskofthesecontaminants.Thisisthefirst
embracingstudyinthewaternetworkofCoimbraregiononthedisseminationofantibiotic
resistancedeterminants.Moreover,itisalsothefirstreportwiththesimultaneousdetectionof
multiresistantbacteriaproducersofAmpCandESBLslactamasesinaquaticsystemsin
Portugal.
