At Interent department, in St.Mariam hospital, there are Nursing of
manager, three nurses primary and five nurses associate. The
applicable of document system in this hospital is written system. This
thing make nurses activities is hampared. One day, St.Mariam hospital
will do accriditation. The Accriditation team visit the all of department
and one of them is Interent department. Looking at the documentation
system applicable accreditation team then entered the renewal of the
system is written into the computerized system.

PA 1 : Good morning, Interent department with nurse Tasya, may i help you?

: Morning, if there were empty bed?
PA 1 : Yes, we have one bed in second class.

: Ok, please prepare the facilities needed for 09.00 oclock, because there
will be a
new patient.

Narrator :
At 09:00 am patient delivered by emergency nurses into the internal
room. Patients received by the NUM, second of nurse primary, and
second of nurse associate.
NUM : Morning Ms.Arisca, im nurse uchi as a NUM in this department, this is
Dyan my
nurse primary and next Melan my nurse associate. Well take care you.

Patient
: Ok nurse, thankyou.
PP 2 : please nurse, you do assessment and ill do handover with emergency
nurse.
PA 2 : Alright, nurse. Excuse me, Ms. Arisca. Ill take you into the treatment
room.
In treatment room....

: Ok, Ms.Arisca. what do you feel now?

: I feel weak and my body is tingle.

: Please, tell me whats happend with you, so you go to the hospital

Patient
: Last night, i ate rambutan. A few minutes later, i felt weak and
tingle in all my body
and than i took a rest. But, at 05.00 oclock, i felt weaken and all my
body was
puffy. Finally, my family took me to here.

: when u felt that reaction, did you consum some medicine?

Pasien: didnt, because i thougt its normal reaction.

PA2 : alright, now i wanna check your vitals sign and do physical examination
in you.
Pasien: Ok nurse.
A few minutes later....
PA 2 : i have finished. If u need some help, you can press bell button and the
nurse will
came to help you.
Patient

: Yes, nurse.

Narrator :
In nurse station, Ns.Melan do documentation and nursing care about
her assement in writting. And Ns.Nurma infom about the result of
Ns.Melan assement to the doctor by phone.
PA3 : Good morning doctor, im Ns.Nurma from Interent depertement, will be
report the new
patients condition at BO3 room, bed two. Her condition are the patient
feel weak and all
of her body are looked redness and puffy.

: Alright nurse, one hour again, ill visit to the room.

: Ok doctor, thankyou.

One hour later, the doctor is coming and he direct examination in
Ms.Arisca.

: Good morning doctor.

Dokter

: Morning too, please, bring me to the new patients room.

In patients room....
Doctor
: Good morning Ms.Arisca. Im doctor Andrew, ill take care you
during you stay in here and
now ill do examination in you.
Pasien: Ok, doctor.
The doctor direct examination....
Doctor

: What do you feel?

Patient

: I still feel weak and tingle in all of my body.

Doctor

: Why can this happen?

Patient
: Last night, i ate rambutan. A few minutes later, i felt weak and
tingle in all my body
and than i took a rest. But, at 05.00 oclock, i felt weaken and all my
body was puffy.
Doctor

: Is this your first time get it?

Patient

: Isnt, doctor. Usually, i eat rambutan but not until like this.

Doctor

: How many you consum it?

Patient

: Maybe, one stalk.

Doctor
: Ok, i think this is only body reaction and ill give you medicine
therapy to overcome
your puffy and tingle.
Pasien: Ok doctor.
Dokter

: Ok, excuse me.

Narrator :
After that, the doctor make medical prescription. And than, Ns.Nurma
back to the patients room to give it to the patients family. The
patients family go to the pharmacy to get some medicine. After that,
the patients family back to ward and give it to the Ns.Melan. Ns.Melan
ask help to Ns.Nurma to injection in Ms.Arisca.

In patients room....
PA 5 : Excuse me Ms.Arisca, ill injection, the function of this medicine is to
reduce your
tingle and puffy.
Patient

: Yes, nurse.

Narrator :
Three hours later, Ns.Melan back to the patients room to ask about
patients condition.

PA 2 : Excuse me, Ms.Arisca. I want check your condition after i injection to you.
What do
you feel now?
Patient
: I feel better than before, My tingle is reduce but the puffy dont
decline yet.
PA 2

: Ok, never mind, you still get injection until you already feel better.

: Thankyou, nurse.

: Your welcome.

Narrator :
Ns. Melan back to nurse station and begun write evaluation about
Ms.Ariscas respon.
Two days later, the accreditation team is coming again and do valuation
in nurse perfomance and facilities at all of department in St.Mariam
Hospital, consist Interent department. At this department, they do
valuation in documentation method that the ward apply.
NUM : Good morning, Ms.Okta and Ms.Afli, My name is Ns.Uchi, im a NUM in
this
department, thankyou for your coming in here.
TA 1 : Your welcome, Ns.Uchi. Ok, maybe now, can you company us to see this
ward
condition?
NUM : Alright Ms.

: Maybe, we can start from the patients room.

NUM : Ok, Ms.

After the accredition team do valuation in the patients room, they back
to nurse station to look the nurse perfomance and nurse documentation
system in this ward.

process.

evaluation

NUM : In my view, this system make my staff feel hard. Because, they must
write in a long
time and sometimes, they often forget to give treatment in the patient.

writtent system must changed into computerized documentation system,

so the
time is shorter and the nurse easier to perform interventions in patient.

After that, a few weeks later, the accredition team
teach about
computerized documentation system in NUM and nurse primary. And
than, nurse primary continue teach to nurse associate. Two days later,
the accredition team back to St.Mariam hospital to evaluation about
nurses skill in conducting the system.
