JOINT AFFIDAVIT OF EXPLANATION
WE, WILBUR S. CHAN and BEVERLY M. KAMID both of legal age, Filipinos,
married and married with residence address at Unit 901 Royal View Condominium 242 Lt.
Ariala St. Pasadenia, San Juan Metro Manila City and #5081 Modesto St. Mapulang Lupa
Valenzuela City respectively, in our capacity as General Manager and Inventory Head
respectively of Constant Packaging Corporation created and established under the existing rules
and regulations of the Republic of the Philippines, and after having been duly sworn to in
accordance with law, do hereby depose and say the following:
1. That due to mounting job orders from our customers for the last few months, we
overlooked and forgot to have our license renewed with PDEA prior to its expiration
last June 08, 2014;
2. That we never made any transaction nor purchase any precursor or controlled
chemicals from our supplier during the expiration of our said license;
3. That from now on, we will ensure to be careful and responsible enough to monitor
our recording CPECS of my PDEA so us not to commit the same mistake and see to it
that we will renew our license prior to its expiration.
4. That we never intend to delay its renewal, and we promise and undertake that it will
never happen again; and
5. That we are executing this affidavit to attest to the truth of the foregoing and for the
purpose of renewing our P3-01513001-R036 license with PDEA and for whatever
legal purposes it may serve.

3. That the company by virtue of this affidavit hereby binds itself to all the Provisions of
Republic Act 9165, otherwise known as Comprehensive Dangerous Drug Act of 2002, and
other implementing rules and regulations and assuring that such law and regulations shall be
faithfully complied with ; and
