Contribucin de la hipoxia en la remodelacin del tejido pulmonar en
procesos asmticos

Resumen
El asma es una enfermedad inflamatoria crnica de las vas areas que inevitablemente se asocia con remodelacin pulmonar
como mecanismo de reparacin tisular. Los cambios estructurales por la remodelacin junto con el proceso inflamatorio persistente
ocasionan obstruccin del flujo de aire, lo que paradjicamente produce hipoxia en el tejido pulmonar afectado. Uno de los cambios
ms importantes que sufre el pulmn durante la remodelacin es la angiognesis, sta es necesaria por el incremento en la masa
pulmonar. El factor de crecimiento del endotelio vascular (VEGF) es el principal mediador de la angiognesis y se sabe que puede
ser regulado por el factor inducible en hipoxia (HIF-1), provocando un rpido incremento en la expresin de VEGF en condiciones de
hipoxia. El factor de transcripcin HIF-1 es un heterodmero constituido por dos subunidades: HIF-1 (inducible en hipoxia) y HIF-1
(constitutivo). HIF-1 se activa principalmente en la hipoxia y media la transcripcin de genes blanco en respuesta a la disminucin
de oxgeno por unin a elementos que responden en hipoxia. Estudios recientes revelan la asociacin de la expresin de HIF en la
angiognesis inducida por los eventos de remodelacin que ocurren en los procesos de asma. Se cree que la angiognesis puede
estar modulada por el factor de transcripcin HIF, como se ha descrito para otras enfermedades en donde existen eventos de hipoxia
y angiognesis. El propsito de esta revisin es mostrar un panorama general de los trabajos publicados en los ltimos aos acerca
de los mecanismos de hipoxia y remodelacin del tejido pulmonar a fin de comprender mejor la fisiopatognesis del asma.

Abstract
Asthma is a chronic airway inflammatory disease that is associated with pulmonary remodelling of respiratory tissue. During asthma,
lung structure changes due to persistent inflammation and causes obstruction of the air flow, which leads to a paradoxical hypoxic
condition in the affected pulmonary tissue. Angiogenesis, which is one of the main components of the remodelling process, is mainly
regulated by the vascular endothelial growth factor (VEGF), although other less influential factors are also involved. It is known that
VEGF is up-regulated by hypoxia inducible factor 1 (HIF-1) during hypoxia. The transcription factor HIF-1 is a dimeric protein composed
of two subunits: HIF-1 (inducible by hypoxia) and HIF-1 (constitutive). HIF-1 activates the transcription of genes during hypoxia by
translocating to the nucleus and binding to hypoxia response elements (HREs) on the promoter regions of target genes. Recently the
expression of HIF has been documented during angiogenesis in lung remodelling during asthma, thus suggesting that it may play a
role in this process. The objective of this review is to give to the reader an overview of the literature in hypoxia and its role in remodelling and the pathogenesis of asthma.

Para poder entender la patogenia del asma es necesario identificar los factores que inician, intensifican
y modulan la respuesta inflamatoria en las vas
areas y saber cmo estos procesos inmunolgicos
y biolgicos producen los cambios caractersticos
de la enfermedad. El desarrollo clnico del asma es
un proceso complejo, en el que participan la predisposicin gentica del individuo a la sensibilidad
al alergeno y el desequilibrio entre la inmunidad
celular (Th1) y la inmunidad humoral (Th2). Este
desequilibrio entre Th1 y Th2 est dado por el
microambiente generado por estas clulas, as por
ejemplo IFN- regula positivamente hacia una respuesta inmunitaria celular del tipo Th1 y desregula
la respuesta Th2 y cuando existe un microambiente
rico en IL-4 se regula hacia Th2 y se suprime Th1.6-8
En las enfermedades alrgicas predomina la IL-4,
favoreciendo la respuesta Th2.9
Se ha reportado que las personas atpicas tienen
concentraciones anormales de la inmunoglobulina
IgE circulante y eosinofilia; adems de mutaciones
en el brazo largo del cromosoma 5. El cromosoma
5q contiene los genes que codifican para las citocinas
IL-3, IL-4, IL-5, IL-9 e IL-13 y GM-CSF, as como el
cromosoma 11q con un locus que codifica la cadena
del receptor de IgE de alta afinidad. Otros estudios
demuestran que la concentracin del antgeno tambin se involucra en la respuesta Th1/Th2, ya que a
bajas concentraciones se produce una respuesta de

La respuesta generada por hipoxia puede sintetizarse en tres etapas: 1) la deteccin de la hipoxia
o sensor a cambios de oxgeno, 2) la regulacin de
genes, y 3) una etapa efectora en donde se da la
expresin de genes y los cambios funcionales que
stos involucren.74 Dicha respuesta es modulada por
el factor inducible en hipoxia 1 (HIF-1), este factor de
transcripcin activa genes que facilitan la adaptacin
y supervivencia de las clulas y el mantenimiento de
estados de normoxia o concentraciones normales de
oxgeno (-21% O2) ante la hipoxia (1% O2).75,76
Hasta la fecha se ha reportado que este factor regula la transcripcin de ms de 100 genes implicados
en procesos de angiognesis, eritropoyesis, metabolismo energtico, gluclisis, apoptosis, crecimiento
celular, supervivencia y movilidad celular. El HIF-1
es fundamental en la angiognesis embrionaria y
para la formacin de la placenta.77,78 El HIF-1 fue descrito en 1991 por el grupo de Semenza al identificarse
los elementos que responden a hipoxia en el gen de
la eritropoyetina, responsable de la eritropoyesis en
el que observaron la secuencia consenso de unin al
ADN 5-(A/G) CGTG-3.79,80

de genes. Los ratones floxiados se tratan posteriormente para inducir la expresin de Cre dando como
resultado la interrupcin del gen ARNT y perdiendo
la expresin del ARNm. Estos ratones se han usado para el estudio de la participacin de HIF-1 en
diferentes cnceres, lo que abre la posibilidad de su
estudio en asma.
Estos estudios ayudarn a entender mejor la
participacin del factor de transcripcin HIF-1 en
la patogenia del asma.
