2014 - Begin: Construction is steel sliding plate type with bronze sheet. All bearings are heavily
rusted and appear frozen in contracted position. G-7 bearing is most contracted to about 1/2"
past edge of masonry plate. (Photo 2) The G-5 & G-6 bearings have their left anchor bolt
sheared. (Photo 1) The other bearings are only in slightly better condition. All bearings warrant a
3 Rating.

2014 - Asphalt concrete pavement at begin and end of bridge has alligator cracking progressing
to breaking up of pavement. (Full width x 2' long) Scattered small potholes were noted in these
locations. In addition a few spots near right side at end has mud pumping out of pavement. There
was also moderate alligator cracking along lane line on begin & end approach. A high 4 is
warranted.

Construction is galvanized box beam on weak posts with transition to 2 tube bridge rail.
Approach box beam rail is damaged or deteriorated at the following locations:
(1) Begin Right: Posts 1 through 4 from bridge rail transition are bent over & detached from box
beam. Box beam is kinked in this area with rust holes near rail splice. (Photos 8 & 45)
(2) Begin Left: Most of rail run is affected by impact damage. Box beam is torn near bridge. Post
5 from bridge is bent & disconnected from box beam. Posts 23 through 26 & post 32 are bent &
disconnected from box beam leaving about 30' of rail unsupported. (Photos 11, 12 & 13)
(3) End Right: Box beam adjacent to bridge is heavily rusted with holes in bottom for 10' to 15'
length. There are also rust holes in vertical faces at rail splices. (Photos 14 & 15)
(4) End Left: Box beam adjacent to bridge is heavily rusted with holes in bottom for about 4'
length. Box beam terminus section also has bottom rust hole about 1' length.(Photo 46)

with efflorescence along its length. There is a 2' long by 6" high spall at Pier 1 joint. (Photo 47)
Pier joint is compromised by spall. Left & right fascia both warrant a 4 Rating.

consisting of 3" by 5" steel tubes are welded in place between the flanges on both sides of the

(1) Pier 1, span 1, girder 1 has section loss over the height of the web and is bowed slightly to
the left. The temporary repairs are tight to the flanges. (Photo 60)
(2) Pier 1, span 1, girder 7 has section loss over the height of the web. The temporary repairs are

(3) Pier 1, span 2, girder 1 has section loss over the height of the web. The temporary repairs are

No D-Meters possible due to location of temporary supports.
Balance of girders have areas of rusting with some pitting on bottom flange and lower web but
are otherwise in fair condition.
Deck supporting diaphragms at piers are also rated here. Heavy joint leakage has resulted in
rust, delamination and section loss on diaphragms adjacent to joints. Worst locations are at Pier
3 in Bay 1 (Photo 25) & Bay 6 (Photo 26) where very heavy rusting, delamination & section loss
was noted. Pier 2 in Bay 1 also had fairly heavy botttom flange & lower web section loss.
Diaphragm at Pier 3 in Bay 6 rises to level of a Yellow Flag due to large hole at connection to G7. (See Separate tab for YF4K140037 details) Interior diaphragms over pier in somewhat better
condition.

Seven rusted girder ends are red structural flag conditions due to heavy section loss &
deformation on web over supports. (No D-Meters possible due to location of temporary supports)

about 30% of the total area. Condition is worse under and around abutment and pier joints and
also along both flanges where moderate to heavy rust & section loss has developed. Much of
remaining paint has multiple small rust spots.

potholes. (Photo 51) Sealer is completely compromised at right side due to deep spall in
sidewalk & fascia. (Photo 47) Damp dirt & debris on pier below is evidence of leakage through
joint. Rusting of girders, diaphragms & bearings below is also evidence of long term leakage.
During wet snowy day at bridge joint was leaking heavily especially at left & right sides. Rating is
lowered to 2.

Rusting of girders, diaphragms & bearings below is also evidence of long term leakage. During
wet snowy day at bridge joint was leaking heavily especially at left & right sides. Previously
reported sink hole in asphalt at joint was repaired in 2013. Timber was wedged under joint,
braced to top of pier and holes were patched. (Photo 30) A Removal report for Safety Flag
4B130121 is included. No holes through joint were noted at time of inspection. Only small
surface potholes were noted. Rating is lowered to 2.

Span 2 (over abandoned RR bed) is overgrown and needs UBIU to inspect. This also allows safe

Pier 3: Columns 1 and 4 are spalled up to 4" deep for a 2'- to 3'-wide strip for full height.
Spalls are surrounded by a 1' to 2' wide strip of cracked and hollow or delaminated
concrete. Rusty reinforcing bars are exposed in spall areas. Hoop bars have an average
of 50% section loss. Vertical bars appear to have 10% to 20% section loss in spall
areas.
(1) Condition located on left of column 1.
(2) Condition located at begin & right on column 4. Two consecutive hoop bars near
mid-height on column 4 are severed.
NOTE: Columns are taller than normal because bridge originally spanned a RR track.
Plan shows columns are almost 30' in height from footing to bottom of capbeam.

Item 58 Approach Rail

Construction is galvanized box beam on weak posts with transition to 2 tube bridge rail.
Approach box beam rail is damaged or deteriorated at the following locations:
(1) Begin Right: Posts 1 through 4 from bridge rail transition are bent over & detached
from box beam. Box beam is kinked in this area with rust holes near rail splice.
(2) Begin Left: Most of rail run is affected by impact damage. Box beam is torn near
bridge. Post 5 from bridge is bent & disconnected from box beam. Posts 23 through 26
& post 32 are bent & disconnected from box beam leaving about 30' of rail unsupported.
(3) End Right: Box beam adjacent to bridge is heavily rusted with holes in bottom for
10' to 15' length. There are also rust holes in vertical faces at rail splices.
(4) End Left: Box beam adjacent to bridge is heavily rusted with holes in bottom for
about 4' length. Box beam terminus section also has bottom rust hole about 1' length.
Note: Different photo dates due to access problems to topside on 390.
Orientation is North.
