HSE ALERT

FINGER INJURY : CAUGHT IN BETWEEN A MOVING AND BODY PART


Incident description
On 29th May 2014, at 1400 hrs a sub-con workers suffered serious finger injury while in contact with steel
bar bending machine. It was observed that the IP unintentionally place his left hand in between the top
bender table and the end of the rotating roller. As a result, his left forefinger caught in between the
moving and body parts of the machine. He suffered injury to his left forefinger.

Note: Photos no. 1 and no. 2 Demonstrating how incident was happened. Photo no. 3 IP received first aid treatment before send to the nearest
hospital for further check up.

Immediate action taken


Victim sought immediate first aid treatment on site. He was then referred to hospital for further medical
treatment.

What went wrong?

Carelessness about the hand placement on the bender table


No awareness highlighted on the specific activity during tool box meeting.
Failure to identify potential hazards (Job Safety Analysis, Risk Assessment & Work Procedures)

Recommendation/ Lesson Learnt

Tool box meeting to be conducted before start work.


Each and every affected personnel to be briefed on potential hazards of the activity accordingly.
Pay attention to specific hazards at machines on top of Job Safety Analysis and Work Procedures.

When possible, the most preferred option should be selected and implemented (in other words,
PPE, training, and awareness devices are not suitable as substitutes for guarding when guarding is
feasible).
Elimination or substitution through inherently safer design. Examples include: automated
material handling, reduced mechanical force/energy, elimination of pinch points by increasing
clearances, etc.
Guards or safeguarding devices. Examples include: barriers, presence sensing devices & etc.
Training and safe work practices/procedures. Examples include: written operating, maintenance,
employee training, employee demonstration of competency, on-going evaluation of employee
operating performance, etc.
Personal Protective Equipment (PPE). Examples include: safety gloves, ear plugs, protective
footwear, helmets, etc.

Example guards or safeguarding devices:-

STEEL
PLATE
WOOD

Note: Photos above shows an example of safeguarding method i.e. Installation of plywood OR steel plate on top of the bender table without
interfering/affecting with the use of machines) that can be apply on bar bending machine related to the incident.

NO JOB IS SO IMPORTANT THAT WE CAN'T TAKE THE TIME TO PERFORM IT SAFELY
