Novoa Domino's mother stated to Officer Ladwig that Cody James Novoa Domino assaulted her by punching her in the face and
slamming her hand in the door.

I, the undersigned, being duly sworn and under oath, state that the following facts known by me or told to me by the other
reliable persons form my belief that Cody James Novoa Domino the defendant committed the crime charged.
On Sunday March 15, 2015 Officer Ladwig was called to 303 2nd Street in Lake View at 5:04am. Law enforcement has been called
to this residence 4 times this night. Upon arrival. Officer Ladwig was told by Cody James Novoa Domino's mother that she had been
assaulted by Cody. Cody's mother stated thai she had been punched in the face by Cody and Cody slammed her hand in the door.
While arresting Cody James Novoa Domino he wrestled with Officer Ladwig in an attempt to avoid arrest while threatening Officer
Ladwig with physical violence.
