GUIDELINES ON HOW TO APPLY FOR A MACHINE READABLE (MRP)

PASSPORT
FIRST TIME APPLICANT

1. Dial the Telephone Passport Express Hotline at (02) 567-1111 to learn the
requirements.
2. Prepare the duly accomplished Telephone Passport Express Application form
with the required documents.
3. Personally submit documents to the nearest LBC branch or Telephone
Passport Express office for screening and evaluation.
4. Pay the necessary fees.
5. Wait for confirmation of schedule for fingerprinting and signing.
6. Proceed to the DFA Telephone Passport Express Center on the assigned day
and time.
7. Your new passport will be delivered to you by Telephone Passport
Express/LBC Door to Door Delivery Service within 24 hours upon release.

REQUIREMENTS FOR FIRST TIME PASSPORT APPLICANT
* PERSONAL APPEARANCE IS REQUIRED *

• Duly accomplished application form with Telephone Passport Express/LBC

logo

• Birth Certificate on NSO security paper
• Recently acquired NBI clearance (for 18 years old and above) for travel
abroad
• Three (3) copies of passport sized pictures (1.77” x 1.37”) with plain royal
blue background
• Any two (2) of the following valid IDs

SSS ID, Computerized Voter’s ID, PRC ID, Driver’s License with receipt, GSIS
ID, School ID with original registration card (for student), Any old
documents with date and place of birth

Warning: Submission of false information and falsified documents are

punishable under Philippine Laws. It is up to the discretion of the DFA to
accept, reject or request for additional supporting documents or personal
appearance from the applicant.

REQUIREMENTS FOR PASSPORT RENEWAL APPLICANT

* PERSONAL APPEARANCE IS REQUIRED *
• Duly accomplished application form with Telephone Passport Express/ LBC
logo

• Old passport for cancellation

• Photocopy of 1st, 2nd, 3rd and last pages of the old passport including the
latest arrival/departure stamp, (if used passport)
• Three (3) copies of passport sized pictures (1.77” x 1.37”) with plain royal
blue background

ADDITIONAL REQUIREMENTS
a. For Brown passport
• Any documents with complete and full middle name, such as any of the
following:

• NBI Clearance
• Baptismal Certificate
• Birth Certificate
• Driver’s License with receipt
• Form 137 / Transcript of Records and etc.
• Any old documents with date and place of birth
