steps completed) must be submitted to Drilling Engineer by person completing surface abandonment for well to be considered abandoned.

Guidance: Please provide a summary of drilling problems, geological description of interzonal communications (actual/potential) if porous
zones left open and brief description of downhole completion assemblies, if applicable. i.e. packer/frac port assemblies. Detailed description
of complicated wells should also be included.

REPORTING CHANGES OF SURFACE WELLHEAD COORDINATES AND BOTTOM HOLE LOCATION:


A change in surface wellhead coordinates or bottom hole location (BHL) must be reported as listed in the detailed in the
REQUIREMENTS below.
However, a Well Permit Amendment Application Form is required if:
a.

A change in surface wellhead coordinates results in additional land disturbance beyond the boundary of the original surface
lease;

b.

A change in surface wellhead coordinates results in a new well permit name (Example the pad is located on a boundary and
causes the well permit name to change from 13 to 3-3).

c.

A change in BHL is for a different objective formation than the original well permit objective formation;

d.

A change in BHL with attendant change in well profile such that well name adds or deletes HZ also requires an amendment.

REQUIREMENTS :

Change of Wellhead Coordinates:

If surface wellhead coordinates at time of rig release or suspension of drilling differ from those stipulated on the original well permit
application form, they must be entered below in both UTM NAD 83 coordinates and offset coordinates formats:

Change of Bottom Hole Location:

If BHL location(s) as drilled differ from those specified on the original well permit application form, the revised actual bottom hole
location(s) must be entered below. Multiple BHLs are to be entered in the order they occurred. Holes abandoned as a result of drilling
problems, plugged back strat holes or ghost holes are also to be included. The following formats are to be used, depending on the
area land map system in place:

Revised Wellsite Survery Plan:

A revised Wellsite Survey Plan must be submitted if the as drilled surface coordinates or the BHL is outside the boundaries
of the original Wellsite Survey Plan filed with the well permit application,

A revised Wellsite Survey Plan is also required if a change in surface wellhead coordinates necessitates the submission of a
well permit amendment, as described in (a) or (b) above.
