The following items, as a minimum, shall be confirmed by the Production/ Subcontractor before submittal of the RFI to QA Dept. after filling,
this form shall be attached by Prod./ Subcon. to the Requisition for Welder Qualification Test. The QA Insepctor shall physically check allitems
and submit the RFI to CLASS/ OWNER (if required) to arrange for official Welder Qualification Test.

1. Are you prepared to perform the test/ inspection?
2. Have you checked all the requirements of the welding spec./ code regarding to this
specific WQT? (PMI, mechanical test, etc.)
3. Any issues not yet closed? (clarification request, site querry etc.)
4. Is it confirmed the availability of CLASS to witness the test?
B. Documents
1. WPS approved?
2. NDT procedure approved?
3. Base material certificates available and correspond with the proposed WPS?
4. Filler metal certificates available and correspond with the approved WPS?
5. Shielding/ Backing gas/ Flux certificate available and comply with the applicable
requirements?
6. Inspection + measuring tools calibration certificates available and valid?
7. Additional requirements for Stainless Steel, Ni-alloy, alloy 400, Cr-Mo etc.
a. Grinding discs/ wire brushes/ etc. certificates are avaialble and valid?
b. PMI procedure approved? (for SS, alloy 400, Ni-alloy, Cr-Mo Steel, Low Alloy, etc.)
c. PMI report availale?
C. Material and Welding Consumables
1. Base materials (pipes, plates, etc.) properly prepared and identified for performance of the
test? (marked as per certificate, beveled as per the applicable WPS, etc.)
2. Filler Metal available and correspond to the applicable WPS?
3. Shielding/ backing gas/ flux available?
D. Working Tools/ Equipments and QC Tools?
1. Welding machine available, calibrated and in proper working conditions?
2. Grinding machines, grinding discs, brushes etc. avaialbe (correct types)?
3. Calibrated clamp meter, pyrometer etc available?
4. Welding gauge, gap, misalignment gauges available?
5. Additional requirements for stailness steel, Cro-Mo, low alloy, Ni Steel, alloy 400, etc.
a. Stainless steel tools (wire brushes, grinding/ cutting discs etc.) available including special
markers (markers shall be free of Chloride/ Sulphide)
b. PMI equipments available (correct type)?
E. Subcontractor Personnel Required
1. Subcontractor qualified welding inspector/ engineer available?
2. Adequate welders, fitters, helpers, etc.?
3. Confirmation that the Subcontractor welding team know what to do?
