A citation was issued to you or your employee for violating SF Traffic Code section 194.3. Please notify your
Employer or Managing Officer of this citation, so that your Injury & Illness Prevention Program may implement the
necessary corrective safety measures.

To avoid additional penalties/fines, pay the Citation or request a Protest Hearing within 21 days

TO PAY THE CITATION AND CLOSE YOUR CASE:

1) Fill out the enclosed “Citation Disposition Form” – left side, and all contact information
2) Make your payment check payable to “DPT – 194.3 TC” in the amount of $622.81
3) Be sure to note your citation number on your check
4) Mail your completed “Citation Disposition Form” and check to the following address:

After DPT receives the $622.81 your citation will be closed unless you request a Protest Hearing.

TO REQUEST A PROTEST HEARING, YOU MUST:

1) Fill out the enclosed “Citation Disposition Form” – right side, and all contact information
2) Make your deposit check payable to “DPT – 194.3 TC” in the amount of $622.81, to be held for 30 days
3) Mail your completed “Citation Disposition Form” and check to the address listed above
4) You will be contacted by California Community Dispute Services (CaCDS) to schedule a hearing.
5) You or your representative must appear at the hearing within 30 days of citation or forfeit the payment. If
your payment is forfeited, the citation case will be closed.

Appearing at the Protest Hearing:

All Protest Hearings will be held at California Community Dispute Services, located at 502 Seventh St,
San Francisco. You must bring two separate checks to the hearing:
1) One business check, cashiers check, or money order made payable to CaCDS in the amount of $100.
2) One check made payable to DPT-194.3TC, the fine amount to be determined at the Hearing.

A check of $622.81 must be deposited with DPT within 21 days of the date of the citation or the citation
will be referred to the State of California Superior Court. Failure to appear in Superior Court will result in
a warrant for your arrest. Fines increase by $250 for each subsequent violation within the SAME YEAR.
