ASSIGNMENT
Objective:
Evaluate the understanding of Occupational Safety and Health Management System
(OSHMS) concept and its application in the company
Task 1
a) Legal requirement
1. Occupational Safety And Health Act (OSHA) 1994 Act 514
Section 4 (objective of act)

Secure safety, health and welfare of employee against risk cause by the
activity at work

To protect other people besides employee at workplace from risk cause by


work activity

To promote an occupational environment for employee considering


physiological and psychological needs

Section 15 (General Duties of Employers)

Maintain and provide safety, health and welfare of the employees at working
place.

Provide and maintain the system at work are safe and without risks to health
as far as practicable

Ensure safety and absence of risks to health on the use or operation, handling,
storage and transport.

Provide training, supervision and enough information and instruction for


employees

Provide and maintain adequate facilities for employees to ensure their welfare.

Section 16 (Duty to formulate safety and health policy)

Revise safety policy of the company from time to time to make sure the
information regarding safety is updated.

Section 18 (duty of occupier other than employees)


Section 21 (General Duties of Manufacturers)

Ensure substance is safe and without risks as far as practicable

Carry out testing and examination necessary

Take reasonable care of safety and health of himself

Established because has more than 40 workers

The employer and committee join hands to improve safety and health at
workplace

Section 32 (Notification of Accident, dangerous occurrence, occupational


poisoning and occupational diseases and inquiry)

Has to notify nearest Department of Safety and Health (DOSH) office any
accident, dangerous occurrence, occupational poisoning and occupational
disease which has happen or has high possibility to happen at workplace

Section 37 (Industry Code Of Practice)

Any industry code of practice must be approved by Director General (DG)

Section 67 (Duty to Keep Secret)

Any manufacturing and commercial secret that is known by a person


cannot be disclose

2. Occupational Safety And Health (Safety And Health Committee) Regulations 1996
Regulation 12

Inspect workplace at least once in every 3 months to ensure safety and


health of employees.

Further inspections can be made at any time to check whether the


measures taken to ensure safety and health of persons at work are
effective.

The discussion of the observation will be made as soon as possible to


solve any problems that arise.

Regulation 13

When any incidents occur, the company will inspect as soon as it safe to
do so.

The report of the finding shall be submitted to the chairman by safety and
health officer

Regulation 14

Based on the report action should be taken and recommendation will be


made by Safety and Health Officer.

3. Occupational Safety And Health(Use Of Standard Of Exposure Of Chemicals Hazardous


To Health) Regulations 2000
Regulation 5 (Identification of Chemical Hazardous to Health)

All chemicals hazardous to health use in the place of work should be


identify and record in register.

The register should be accessible by all workers at the place of work that
has possibilities to be exposed to the chemicals.

Regulation 6 (Permissible Exposure Limit)

The exposure of the chemicals should only be allow within permissible


limit

Make sure CSDS is provided by each supplier for each chemical


hazardous to health.

Regulation 29(Warning Signs)

Warning signs should be installed in area that is hazardous to health due to


the presence of chemicals hazardous to health.

Documented procedure

Task 2
Safety procedure for working in confined space.
Before starting work
1. Have system of permit to enter and work in confined space
2. Prepare safe operating procedures
3. Conduct training of the SOP for related workers
4. Conduct a measurement test of the oxygen and gas content in the confined space by an
Authorised Gas Tester (AGT)
5. Purge hazardous gas/pollutants and ensure sufficient ventilation
6. Display warning signages and fencing around work area
7. Ensure availability of fire prevention equipments within close proximity of the confined
space entrance
8. Establish an emergency response team, first aiders and emergency equipments
9. Appoint a supervisor and assistant to monitor activities and record name and job
10. .Prepare and provide suitable personal protective equipment for the use of worker and
emergency team
11. Isolate/close all valves and connections to the tank using the lock out/tag out system
12. Prepare an exhaust system, if applicable for the suction of dust and welding activities
13. Ensure the suitability of hand and electric tools with the task at hand
14. Ensure safety of all electric equipments
15. Ensure the safety of entrance and exit points of the tank

16. Select workers who are physically and mentally fit, and are properly trained in confined
space works and record their names

During working
1. The supervisor or assistant continuously monitor the works being done
2. Direct measurement of the oxygen/gas content in the vessel/confined space
3. Continuously suck out dust that is generated from works
4. Ensure a good communication system between worker and supervisor
5. Ensure sufficient and continuous ventilation
6. Immediately take the necessary emergency responses in cases of accidents/emergency
situations (explosion, fire, air pollution etc)

After completion of work


1. Ensure and record that all workers have exited the confined space
2. Inspect the work area to ensure that all work equipments have been removed (mobile
exhaust system, lights, fire extinguishers etc)
3. Stop the electricity supply
4. Cancel/destroy the work permit issued by the supervisor on duty
5. Report problems, if any, related to the works carried out
6. Handover the responsibility of handling the confined space to the respective persons and
record the handover details

Task 3
Safety procedure for working at height
When deciding on the appropriate fall protection, consider:
In what situations is fall protection required?
What are the advantages or disadvantages of each type?
What is the best. specification for the installation of the fall
protection?
What degree of training and supervision is required to ensure
the correct use?
How can the system be safely installed?
What maintenance schedules or skills are needed to ensure
satisfactory performance?
How will the correct work procedures be implemented?

CHECKLIST FOR CONTRACTORS


To comply with the Act, principal employers and their agents should
ensure that the following issues are carefully considered during the
designing or planning stages, and the selection and co-ordination of
contractors and work schedules:
Is the contract designed and being carried out in a manner
that will reduce hazards to a minimum?
Can safety be improved by `building in' features, e.g. guardrails,
safety mesh, etc?
Can future maintenance work be made safer by building in
systems, e.g. fall arrest anchors?
Does the principal employer have supervisory systems in
place to monitor contractor safety performance?
Does the contractor understand the correct sequence of critical
operations?
Does the contractor have enough information to carry out the
work safely?
Do contractors have the expertise and ability to carry out the
work safely?
Will the operations of one contractor create a hazard for other
contractors?
Can fall-arrest Systems for future maintenance be built into
the structure?

Operational Planning
Operational planning involves developing timetables and systems that
include safe work practices during the erection, maintenance, repair and
demolition of any plant or building. A co-operative approach between
all parties involved is needed. Planning includes:
Designing building programmes so that walkways, guardrails, and
fixing points are installed as work progresses, allowing for safe
work methods during construction and future maintenance.
Planning for work to be carried out at ground level or in areas
where falls or hazards are not present.

Fitting guardrails or brackets to take guardrails and other


safety features to formwork and falsework as they are built
and before lifting into place.
Ensuring the structure will take the forces that will be required
for the attachment of fall-arrest systems, falsework, scaffold
ties, etc.
Building in safety by using such components as roof safety
mesh to provide for the safety of the roofing contractor and
future maintenance staff.
Providing leadership and regular inspections to ensure that
safety systems are in place and operating to identify and
control hazards.
WHAT EMPLOYEES NEED TO KNOW
When fully trained, an employee should at least know:
Safe and practical methods of carrying out the work;
Correct ways to use tools, plant and equipment safely;
How tools and equipment should be stored or secured;
Systems in use to protect employees and others from
injury;
Emergency procedures
