Notice is hereby given that the registration for the new teachers who passed the August 2014 BLEPT will start
on November 5, 2014 and onwards.
Bring the following:
a) Printed OATHFORMdownloaded from (http://www.prc.gov.ph/online/registration)
b) Registration/Annual Fee of Php 1,050.00 and;
c) Current Community Tax Certificate (Cedula)

IMPORTANT REMINDERS:
Teacher registrants are advised to strictly observe the schedule to avoid any inconvenience.

ONLY PASSERS SCHEDULED FOR THE DAY WILL BE ENTERTAINED.

Mindoro passers may opt to register during Mobile Services
STEPS IN REGISTRATION:
1. Download and print OATH FORM from http://www.prc.gov.ph/online/registration
2. Submit the electronic generated OATH FORM (printed in paper size 8.5 x 13) at Window 8 for
verification and assessment of fees.
3. Pay registration/annual fee at Window 17.
4. Secure documentary stamp at the Photo Booth (in front of Window 17).
5. Pay OATH TAKING TICKET at the PRB Representative (Photo Booth).
6. Submit OATH FORM after payment at Windows 11 or 12 (to enter payment in the LERIS).
7. Submit your OATHFORM in any of the following Windows 2, 3, 4 or 5 and wait to be called to sign the
Registry Sheet.
8. After registration, receive your claim slip; come on your scheduled date of release.
For those who want to apply for CERTIFICATION of PASSING, RATING and GOOD STANDING:
9. Secure ACTION SHEET from the Public Assistance and Complaints Desk
10. Pay certification fee at Window 17(PhP75.00 per copy)
11. Secure documentary stamp at the Photo Booth
12. Submit Action Sheet
Official Receipt at Window 7
13. Wait to be called at Window 5 to receive your Certifications
Important: ** Failure to submit AS/OR will mean that you will not have your certification
