This is what most current doctors offices look like in the way of record keeping.
Many doctors are making the switch from physical records to digital but its a slow process because of the abundance of labor it requires.
Already, one third of a physicians work day is taken up by paperwork.
Whats in your records:
height, weight, temperature, blood pressure, symptoms you bring up in appt., referrals, test orders/results, prescriptions, hospitalization recommendations, previous visits, docs diagnosis/treatment

So basically anything you want to know about your physical health is stored in that file but you dont have easy access to it.

When it comes to current medical technology, theres a disconnect
between the doctor and the patient. CHART bridges the
communication gap and gets people more informed and proactive
about their own health.

ashley is 30 years old and pregnant with her second child. She is currently in her 2nd trimester and anxious to make sure her pregnancy is moving along smoothly. She tries to ensure that
everything she does benefits her unborn child, and is very health conscious because of the risk of diabetes that runs in her family.

Jacob is Ashleys first child. Hes 7 years old with type 1 diabetes. He wants to able to be more independent and track his own health, especially when hes away from home. Ashley (mom) wants to be sure that all his

Dr. John Posey, age 65, is a practicing OBGYN at KU Med. Hes been in the medical field for a while and is looking for ways to connect with his patients and encourages them to be more involved with their own
healthcare. Ashley is one of his current patients and she connects with him about her pregnancy needs.

Ashley begins on the desktop version of the app
Since its important to keep her medical records confidential, CHART uses an eye scanning system to log in.
She uses the webcam on her computer to match her eye up to the scanner.
Once logged in, Ashley is brought to the main page of her account. From there, she can view her records (as well as those of her direct family), her doctors, insurance information, and the community section (which
would include support groups and recommendations).

Ashley selects the doctors tab. She wants to contact her OBGYN about some possible issues in her pregnancy.
Once shes selected Dr. Posey, the program opens up to his profile and contact information. From there she can call, email or video chat with her doctor.
She wants to talk face to face with her doctor so she selects the video chat option. She waits while it connects to her doctor.
During the video chat Dr. Posey suggests she come in to the office for a check up
Ashley leaves the computer and heads to her appointment.
When she arrives at the office, her phone alerts her that shes been checked in via near-able technology.
It then gives her the option to update any information in her file, such as insurance information, updates in symptoms, etc.
She selects the notication and unlocks her phone, which automatically brings up the CHART mobile app.
The same eye scan technology is present on the mobile version to keep all information private
She uses the front facing camera on her phone to scan her eye
This brings her to mobile version of her account with the two most recent notications at the top.
She dismisses one of the notications with a swipe and then selects the other to make updates to her le.
A form unfolds for her to ll in.
Once shes completed the form by lling in the reason for her appointment she closes the form.
Shes then returned to the main page of her account
Ashley enters the exam room.
Ashleys phone syncs with the exam rooms interactive wall.
Once synced, the wall loads the updates done by the nurse/technician, such as weight, height, temperature, blood pressure
Enter Dr. Posey. He has Ashleys files pulled up on his iPad.
He wants to show Ashley her sonogram so he holds down on the Sonogram Reading title
When he releases an icon pops up for him to send all the images to the interactive wall in the exam room.
Sonogram images appear on the wall so both Ashley and Dr. Posey can view them at large scale. In the corner of the wall, an audio recording is taking notes on the dialog of the appointment.
Dr. Posey selects one of the sonogram images to look at larger and in more detail.
Dr. Posey sees something slightly out of the ordinary and circles it and makes notes directly on the wall.
He then moves on to talk about Ashleys weight. He clicks on the weight in the side bar and it unfolds to show information graphs regarding her pregnancy
weight gain, organized by trimesters.
He notices that he weight is about 3 pounds over the healthy average. He circles and makes a note so she can be sure to keep an eye on it in the future. At the end of the appointment he drags the screen up to the
CHART logo which saves the whole appointment and sends it into Ashleys records.
Ashley leaves the appointment.
Her phone alerts her to the updates made to her records.

Jacob at school.
Jacobs phone alerts him every time he needs to check his blood sugar. Right now its lunch time!
She selects the notification and unlocks her phone, which automatically brings up the CHART mobile app.
Jacob also has to sign in using the secure eye scanner
Signing in is quick and easy for him.
CHART then brings up his account, a simpler version of Ashleys. The notification reminding him to check his glucose levels is at the top.
He clicks the notification and it brings him to his daily log, which show the week so far.
He selects today and it unfolds for him to start checking his levels.
Jacob is using a new diabetic meter that syncs directly with his smartphone
CHART receives and plugs in the readings without any effort from Jacob
It then displays his current glucose levels which then helps the school nurse to calculate the amount of insulin Jacob will need before lunch as well as what
his levels have been for the past week.
Once thats complete the pre-lunch bar is filled in on the Wednesday tab as complete. Jacobs entries then sync with CHART and can be seen from the desktop version.
Ashley back at home.
once Jacob has completed his lunch time log, Ashley is alerted via pop up.
She selects his profile, which includes his health as well as info graphs showing his growth. She sees that the update thats been made is lower down.
She scrolls down to the graph showing Jacobs glucose levels from this week and sees that the most recent pre-meal entry has been made. This way she feels comfortable knowing that Jacob is taking care of his health
even when shes not around.
Heres to a healthier and more engaged tomorrow.
