On 9-28-14 at approx. 0955 hrs. I was dispatched to a one vehicle accident on Jacksonville Cato
Road near Ison Rd. Upon my arrival MEMS was treating the driver, Aaron Lewis, for shoulder/neck
injuries.
I observed the Lewis vehicle (V-1) positioned on top of a concrete culvert. The Lewis vehicle was
traveling west on Jacksonville Cato Rd. (8700 block) where the front of the vehicle came into contact
with the concrete barrier on the north side of the roadway.
Mr. Lewis stated that an unknown vehicle (no description) ran him off the roadway.
Witness Swagerty stated that he was behind the Lewis vehicle and that the Lewis was traveling at a
high rate of speed prior to the crash.
Witness Martin stated that she was on Jacksonville Cato Rd. going toward Hwy. 107 (eastbound)
and observed that the Lewis vehicle was going "so fast" prior to the curve and she further stated that
she observed the Lewis vehicle "fishtail" around the curve, going into the ditch.
Mr. Lewis was transported to Baptist Hospital ER (Little Rock) by MEMS. Per owner request, Ryno
Wrecker arrived at the scene and towed the vehicle.
I requested that Deputy T. Stevens (P-77) go to the hospital and issued Mr. Lewis a traffic citation
(154148) for Careless Driving and No Seat Belt. (The front driver side windshield indicated that the
driver came into contact with it).
The approx. POI was 85 ft. W of W curb line of the Driveway at 8727 Jacksonville Cato Rd. and 2 ft.
N of N curb of Jacksonville Cato Rd. No visible marks on the roadway at or near the impact.
