Matric or O' Level or American High School - 9th and 10th grade complete results;
Attested/Certified Copies of Complete Academic Transcripts and Certificates of:


FA/ICOM (if applicable)/FSc/ICS complete result or FA/ICOM (if applicable)/FSc/ICS
first year result (in case the FA/ICOM/FSc/ICS final exam has not been taken yet)

or
A' Level complete result or grades for any A' Level subject(s) completed or A' Level
first year results (in case the A' Level final exam of three principal subjects has not
been taken yet)

American High School complete result or up to 12th grade first semesters result (in
case High School Diploma 12th Grades final exam has not been taken as yet).
Applicants who have completed their High School Diploma before the application
submission deadline should submit an Equivalence Certificate issued by the IBCC,
Pakistan in addition to the final result of HSD;

SAT score report (s) (required for overseas applicants exempted from the LCAT/
SBASSE Admission Test);


International Baccalaureate complete result or International Baccalaureate first year
complete results(in case the IB final exam has not been taken yet);

SAT Score reports


Students who are taking LUMS Admission Test(s) and have also taken/are planning
to take the SAT are encouraged to submit a copy of their SAT score report(s);

Four passport size photographs with white background;


Proof of payment;

Applicants taking both LCAT and SBASSE Admission Test will pay a total of Rs. 3300/- as Test Registration Fee in
addition to the application processing fee of Rs. 4400/-.

Transcripts and certificates should be attested from Schools administration.

Applicants taking their A levels or AS level exam in November 2012 are required to submit the final result within
three days of its announcement; however, they must provide this information through online application form and by
submitting a copy of the Statement of entry along with all other supporting documents.

You will need to provide photocopies of the following documents with the Financial Aid form. Check off
each item as you attach it to your form.
Income certificates of all earning members of the family This includes salary slips of last three
months for salaried persons, pension books for retired, financial statements for self employed or
businessman and a certificate/proof in case of any other income
Retirement Order/ Final Settlement Letter (If Applicable)
Income Tax Returns of all earning members for 2012
Wealth Statement for 2012
Bank Statements (for the last year) of all bank accounts in the names of all the family members
and in the name of business in case of businessmen
Utility Bills (last six months) Electricity, Gas, Phone, Water
Rental Lease Agreement (if the residence or any other property is being leased for residence or
business purposes or excess property is leased to any other person or party for income
purposes)
Copies of Saving Certificates/bonds/shares
Loan Document(s) (Loan outstanding as a liability and loan receivable from any relative, agency
or organization)
Medical bills/ expenditure related documents (if applicable)
Credit Card Statements
Property ownership documents (house, plots, agricultural land etc)
Vehicle(s) Registration documents
Fee Concession (Scholarship/Loan) document(s) of sibling(s) - from any institution including
LUMS
Fee document from the last institution that applicant attended
Domicile Certificate of the applicant only
Copies of CNIC/ B Form of Applicant and Parents/Guardian
Complete Photocopy of Passport of Applicant and Parents/Guardian (All 36 pages)
Any other document(s) demonstrating financial need

* The Application Processing fee and the LUMS Admission Test Registration Fee can be paid:

Through a bank draft/pay order drawn in favour of Lahore University of Management
Sciences. All applicants are required to submit original bank draft/ pay order;

By depositing in cash at the MCB Booth located within the LUMS campus.

Applicants are required to send a proof of payment along with all other required supporting documents as
ONE package by on or before

valid if an attested official proof/document is missing.
