above-named Defendant:

3. The Plaintiff provided goods, merchandise, and/or services at an agreed upon price to
the Defendant and received the original of the check(s) which is/are attached hereto in payment
for same. Said check(s) was/were not honored by the bank, and there remains an unpaid
balance due to the Plaintiff for said check(s) and bank/handling charge(s) as hereinafter
itemized.
4. In addition to the unpaid amount of said check(s) and bank/handling charge(s), the
Defendant is indebted to the Plaintiff up to $500/per check for exemplary damages and
attorney's fees pursuant to Wisconsin Statute Section 943.245. Advance warning of the
implementation of this statute was provided as evidenced by the attached exhibits.
5: Thus, the total amount due to the Plaintiff from the Defendant is itemized as follows:
