COMPLAINT-AFFIDAVIT

I, Max Alvarado, Filipino, of legal age, single, and a resident of Las Pinas, Philippines,
after being sworn to in accordance with law, depose and state that:
1. I am a first year law student of University of Perpetual Help of Las Pinas;
2. I know Elmer Pereyno is a senior student in college of Law, but I dont know him
personally since we do not have any subjects in common;
3. Sometime around eight in the evening of August 28, 2014 after my class, while I was
walking towards my car at the parking lot of University of Perpetual Help Las Pinas,
that said Elmer came running towards me and without any warning punched me
thrice in the face.
4. The other students within the vicinity approached us and restrained Elmer;
5. I proceeded immediately to the University clinic for medical check-up, and Doctor
Edgar Zarate who personally attended to me diagnosed that I suffered a broken
cartilage on my nose and it would take nine days for the injury to heal; attached is a
medical certificate issued by Dr. Edgar Zarate.
In view of the foregoing, I am executing this affidavit to support the filing of a criminal
case for slight physical injuries against Elmer Pereyno.
