Introduction:
In what could be one of Indias most catastrophic air disaster in over a decade (and third most
horrible in the past fifty years), the Mangalore air crash tragedy turned out to be a life altering
experience for all of the 166 persons that were on board
.n22 Ma* 2010/ the Air India 01press Flight '12/ a scheduled
passenger service (ro! ,u2ai to Mangalore overshot the run3a* on
landingat around 01400 5TC/ (ell over a cli6 and caught 7re/ spreading
3rec8age across the surrounding hillside. .(the 1%0 passengers and % cre3
!e!2ers on 2oard/ onl* eight passengers survived.
9ith its 1$' (atalities/ the accident 3as the third deadliest aviation
disaster in India :a(ter the 1++% Char8hi ,adri !id;air collision 3hich 8illed
34+ and the 1+&' crash o( Air India Flight '$$/ 3hich 8illed 213<. The crash
caused the highest nu!2er o( aviation (atalities in 2010 and 3as the second
o( the *ear to involve a &3&='00. It 3as also the second ti!e that an aircra(t
had overshot the run3a* at Mangalore.

Ahlu3alia/ the 7rst oFcer/ 3as also a (or!er e!plo*ee o( Cet Air3a*s 3ho
Ioined Air India in April 200+. The* 3ere 2oth 2ased in Mangalore and 2oth
3ere 8illed in the incident.
Follo3ing its scheduled departure ti!e o( 2243$ 5TC (ro! ,u2ai
International Airport in ,u2ai/ the plane crashed upon landing on the '/033
(eet :2/44' !< run3a* nu!2er 0%J24 at Mangalore International Airport at
around 01400 5TC. #ituated in a hill* area/ the airport is one o( the seven
Indian airports designated as a Kcritical air7eldK 2* the ,irectorate Aeneral o(
Civil Aviation :,ACA</ 3herein such air7elds prohi2it Ksupervised ta8e o6s
and landingsK/ so that onl* the captain :not the 7rst oFcer< !a* pilot an
aircra(t during ta8e;o6 and landing. The airport is one o( three airports in
India having ta2le top run3a*sthat re)uire a ver* precise landing approach.
The Crash:
A(ter touching do3n/ Air India 01press Flight '12overran the run3a*
and crashed do3n the hill at its (ar end. The 7nal conversations 2et3een Air

TraFc Control :ATC< and the pilot prior to landing sho3ed no indications o(
distress.

The then Civil Aviation Minister/ -ra(ul -atel said that the aircra(t 3as
(ollo3ing an Instru!ent Landing #*ste! :IL#< approach (or landing on the
ne3er run3a*/ 3hich 3as co!!issioned in 200%. #e)uence o( speci7cs

The pilot reported to ATC that an IL# approach 3as Besta2lishedB
a2out 4.3 !iles :%.+ 8!< (ro! touchdo3n
Landingclearance 3as then given at 2/000 (eet :%10 !< (ro!

The airplane concluded its IL# approach on run3a* 24/ touching
do3n 300 (eet :+1 !< past the touch do3n Mone
It then overran the run3a* and ploughed through a +0 !eters
:300 (t< sand arrestor 2ed 3hich did not stop it
As the aircra(t passed the arrestor 2ed/ its star2oard 3ing collided
3ith the concrete soc8et o( the IL# localiMer antenna
It eventuall* plunged over the cli6 and on to the hillside/ co!ing to
a stop %%0 to +'0 (eet:200 to 300! < past the top o( the slope.

The !inister also stated that 3eather conditions 3ere nor!al 3ith a
visi2ilit* o( 3.& !iles :%.0 8!< and said 3ind conditions 3ere cal! and there
3as no rain at the ti!e o( the crash. A driMMle started onl* a(ter the accident.
The Victis!The Survivors:
Apart (ro! the si1 cre3 !e!2ers/ a total o( 1%0 passengers 3ere on
2oard at the ti!e o( the crash. Although there 3ere 1%+ na!es on the
original passenger list/ + did not 2oard the Hight. All the 2odies 3ere
recovered (ro! the 3rec8age.
Initial reports (ro! survivors suggested a t*re 2urst as the aircra(t
atte!pted a go;around. #o!e other recollections (ro! the survivors also
indicated the plane 2rea8ing into t3o parts/ (ollo3ed 2* dense 2lac8 s!o8e
invading the ca2in. #o!e passengers 3ho 3ere a2le to Iu!p out through an
opening in the 3indo3 3ere essentiall* the survivors. 9iththe help o( the

The Investigation:
Initial investigations revealed that the plane landed a2out 2/000 (eet
:%10 !< 2e*ond the usual touch do3n point on MangaloreBs ne3 '/040 (eet
:2/4$0 !< run3a* 24. A tea! o( airline oFcials and sta6 (ro! the Airports
Authorit* o( India 3ere rushed to the scene to investigate the incident and
assist 3ith rescue e6orts. ?oeing also announced that a tea! 3ould 2e sent
to provide technical assistance (ollo3ing a re)uest (ro! Indian authorities.

The ,irectorate Aeneral o( Civil Aviation ordered an in)uir* into the
crash/ 3hich 2egan the sa!e da*. The @ational Transportation #a(et* ?oard
:@T#?< also assisted the investigation 2* sending a tea! o( specialists
including a senior air sa(et* investigator/ a Hight operations specialist/ an
aircra(t s*ste!s specialist and technical advisers (or ?oeing and the Federal
Aviation Ad!inistration.
In direct response to the accident/ the Aovern!ent o( India decided to
set up an independent air accident en)uir* 2oard called the Civil Aviation
Authorit* that 3ould (unction independentl* o( the ,ACA. 06ectivel* this
!eant that the ,ACA 3ould 2e the regulator and the CAA the investigator.
The ,irector Aeneral o( the ,ACA said that it 3ould 2e set up through
legislation and 3ould co!pl* 3ith the reco!!endations o( the International
Civil Aviation .rganiMation.
The Investigation "e#ort:
According to audio transcripts o2tained (ro! ATC/ #er2ian pilot
Dlat8oAluEica/ aged $$/ 3as given clearance to land ho3ever/he suddenl*
a2orted the atte!pted landing. The aircra(tBs throttle handle 3as reportedl*
(ound in the (or3ard position/ suggesting that the pilot had atte!pted to
a2ort the landing and ta8e;o6 again. According to unna!ed ATC sources at
Mangalore/ the co;pilot Ahlu3alia 3as said to have 3arned his co!!ander
!ore than once to go around instead o( landing/ and that this 3arning had
co!e at a height o( '00 (eet :240 !</ 3ell 2e(ore the aircra(t !ade a
touchdo3n.
The Coc8pit oice Recorder :CR< 3as recovered on 23 Ma*/ and the
Flight ,ata Recorder :F,R< t3o da*s later. The recorders 3ere sent to @e3
,elhi 2* the ,irectorate Aeneral o( Civil Aviation (or data ac)uisition and

?oard(or investigation.
The en)uir* report su2!itted 2* the Civil Aviation Ministr* clai!ed that
Alusica slept (or over +0 !inutes during the Hight. The A!erican @ational
Transportation #a(et* ?oard sa*s it 3as the 7rst instance o( snoring recorded
on a Coc8pit oice Recorder :CR<. Anal*sis o( the accident revealed that
had the pilot Kdeplo*ed dOtente reverse thrust and applied !a1i!u!
!anual 2ra8ing at touchdo3nK/ the aircra(t could have 2een stopped 3ithin
the paved overrun area o( the run3a*. The captain had aggravated the long
landing 2* atte!pting a go;around (ollo3ing deplo*!ent o( the thrust
reversers.
Court of In$uir%:
.n 1& August 2010/ the Court o( In)uir* 2egan a three da* pu2lic
hearing in Mangalore to intervie3 airport oFcials and 3itnesses. .n da* one/
airport and airline oFcials overthre3 that the aircra(t had approached at an
altitude higher than usual/ and that it had landed 2e*ond the landing Mone
:L,D<. The* also !entioned that the airportBs radar 3as operational (ro!
20 Ma* 2010. The airport chie( 7re oFcer testi7ed that crash tenders had
ta8en (our !inutes to reach the aircra(t2ecause the road leading a3a* (ro!
the airport peri!eter to the crash site 3as ver* narro3 and undulating. This
3as (ollo3ed 2* da* t3o transcript o( the coc8pit conversation to ATC.
,octors 3ho conducted post !orte!s on the 2odies recovered
recorded that !ost victi!s had died o( 2urns. .n da* (our/ Air IndiaBs Hight
sa(et* oFcer in(or!ed the in)uir* that the aircra(tBs thrust lever and thrust
reverse levers 3ere 2oth in the (or3ard position/ possi2l* indicating that the
pilot intended to go around.

.n ' #epte!2er 2010/ details (ro! the Coc8pit oice Recorder :CR<
and Flight ,ata Recorder :F,R< 3ere presented to the Court o( In)uir*. The
CR anal*sis revealed that one o( the pilots 3as asleep in the coc8pit. For
110 !inutes the CR had pic8ed up no conversation (ro! the pilots/ 3ith the
report adding that the sound o( nasal snoring and deep 2reathing could 2e
heard during this recording. The F,R anal*sis indicated that the Hight started
its descent at an altitude o( 4/400 (eet :1/300 !</ instead o( the nor!al
2/000 (eet :%10 !<. The aircra(t also touched do3n at the 4/%3' (eet
:1/414 !< !ar8 on the run3a* instead o( the 1/000 (eet :300 !< !ar8/
3hereupon the pilot then tried to ta8e o6 3ith Iust '00 (eet :240 !< o( the
run3a* re!aining 3hich resulted in the crash.
?oth pilots had 2een a3are o( the 3rong Hight path since the* are 2oth
heard sa*ing KFlight is ta8ing 3rong path and 3rong sideK/ 3hile the
aircra(tBs instru!ents gave repeated 3arnings o( this.
.n 1% @ove!2er 2010/ 7ve !onths a(ter the Court o( In)uir* 3as
constituted/ it su2!itted its report 3ith input (ro! the @T#? and ?oeing/ and
stated that pilot error 3as the cause o( the accident since the Hight path 3as
incorrect.
The Fire De#artent:
The investigation report stated that the 7re services reached the site
3ithin 4 !inutes.Go3ever/ the truth 3as disclosed 2* the Chie( o( Fire
services at Mangalore/ Mr.G.#.aradaraIan. Ge )uotedP QA(ter ever* 7re/
people convenientl* (orget the 7re!en 3ho rescued the!/ 2* ris8ing their
o3n nec8s. The heroic e6orts o( 7re!en in the Air India 01press I> '12 crash
too have gone unsung. A(ter nearl* a *ear/ their e6orts 3ere la2eled as
Rnothing e1traordinar*S. The 7re!anSs !anual on aircra(t disasters and 7res
depicts a 2urning aircra(t as a 2o!2 3aiting to e1plode. The o1*gen tu2es/

co!2usti2le !aterial 3hich gives 7re!en onl* 1%0 seconds to carr* out an*
rescue operationT. It is called R2.$ !inute 3indo3S. 9ithin this ti!e/ the 7re
3ill travel through the tu2ular structure o( the plane engul7ng the entire
passenger area. Atte!pts to save lives 3ill have to 2e !ade 3ithin that
ti!e/ and the I> '12 crash happened in a valle* 3here approach 3as diFcult
2ut our vehicle reached there in eight to nine !inutes o( the crash. The 7rst
gush o( a)ua 7l! (or!ing (oa! 3as ad!inistered 3ithin 13 to 1$ !inutes o(
the crash. ?ut 2* that ti!e/ 7re had engul(ed the entire plane and the 2ro8en
parts o( the 2ell* had stre3n around in three di6erent places and had turned
into !ounds o( 7re.T
The 7re depart!ent 3as also criticised (or using the AFFF. ?ut that 3as
the onl* !aterial that could have e1tinguished the high intensit* 7re ignited
2* highl* volatile !aterial li8e aviation tur2ine (uel :ATF<. An* 3ater spra*ed
on the 2urning plane 3ould have Iust evaporated even 2e(ore it reached the
target area.
Co#ensation:
The -ri!e Minister o( India/ Man!ohan #ingh announced 2 la8h (or
the (a!ilies o( the dead and $0/000 (or the inIured to 2e allocated (ro! the
-ri!e MinisterBs @ational Relie( Fund. Narnata8a Chie( Minister Uedd*urappa
has also announced co!pensation o( 2la8h to the (a!ilies o( the dead. In
addition to this/ the Civil Aviation Ministr* advised that the Airline 3ill
provide up to &2 la8h to (a!il* !e!2ers o( each victi! as per the
provisions o( the Indian Carriage 2* Air :A!end!ent< Act 3hich (ollo3s the
Montreal Convention.

The Airline announced interi! co!pensation o( 10 la8h (or
passengers a2ove 12 *ears o( age/ $ la8h (or passengers 2elo3 12 *ears o(
age and 2 la8h (or ever* inIured passenger. This co!pensation 3as over
and a2ove the e1;gratia pa*!ent announced 2* the -ri!e Minister.
Additionall*/ Air India had said it 3ould o6er Io2s to the survivors.
As o( 11 Cune 2010/ an a!ount o( 1&0 !illion had 2een distri2uted as
co!pensation to the (a!ilies o( the victi!s and to the eight survivors.
icti!sB (a!ilies had 2eco!e increasingl* vocal as to the ine)uita2le nature
o( co!pensation paid out 2* Air India and also o( the alleged hostile attitude
o( the airlineBs counsel.
Violationof &andator% Procedures:
9hen Air IndiaSs Cu!2o Cet 0!peror Nanish8a e1ploded !id;Hight and
got scattered in Atlantic near Ireland coast on Cune 23 1+'$/ the
investigators had a gigantic tas8 at hand. The Ro*al Canadian Mounted
-olice o( Canada organiMed dives in e1cess o( &000 (eet not onl* i!!ediatel*
a(ter the crash/ 2ut in 1+'+ and 1++1 also to collect 3rec8age (ro! the
ocean Hoor and to pic8 up the aircra(t de2ris scattered across the ocean
Hoor.The nu!erous parts recovered (ro! the thousands o( s)uare !eters
2eneath the sea 3ere all cleaned/ nu!2ered and shipped to a (acilit* in
Ireland 3here the* 3ere all 8ept (or !ore than t3o decades. The recovered
parts 3ere later arranged to re;create the shape o( the aircra(t as a part o(
the investigation and also to 7nd out ho3 the e1plosion happened and 3hat
e1actl* caused it.
In case o( -an A!erican 9orld Air3a*sS -an A! Flight 103 that 3as
disintegrated in an e1plosion !an* thousands o( (eet a2ove southern
#cotland/ on 21 ,ece!2er 1+''/ the sa!e procedure repeated. More than

10/000 pieces o( de2ris 3ere retrieved/ tagged and entered into a co!puter
trac8ing s*ste!. The (uselage o( the aircra(t 3as reconstructed 2* air
accident investigators/ revealing a 20;inch :$10 !!< hole consistent 3ith an
e1plosion in the (or3ard cargo hold.

Gere in India too/ the air crash investigators are o2liged to conduct the
sa!e e1ercise. As per the -rocedure Manualo( Accident J Incident
investigation pu2lished 2* ,ACA :Issue I rev 2 dated $.10.200%</ the
reconstruction o( the aircra(t 3ith all the de2ris collected care(ull* (ro! the
crash is !andator*/ irrespective o( the circu!stances in 3hich the crash
occurred.
Go3ever/ in case o( the Mangalore crash/ (or (ort* da*s on a stretch
the de2ris had re!ained in the crash site soa8ed in dust and !ud enduring
heav* rain and sun. This negligence on the part o( Air India/ the o3ner/ and
the Court o( In)uir* that investigated the crash 3as 2ound to 2e su2Iected to
a great deal o( criticis!.
The clearing process 3as initiated 2* FiMa/ a local construction 7r!/
3hich 3as hired to do the Io2 and the* heaped the pic8ed up parts in Lorries
and then du!ped the! on an open plat(or! near the ne3 ter!inal o(
Mangalore airport. According to an oFcial o( FiMa/ the total 3eight o( the
de2ris recovered (ro! the crash site 3as Iust 1% tons/ as co!pared to 41
tons o( !etal and 7re resistant co!posites 3hich !a8e up the total e!pt*
3eight o( a ?oeing &3&;'00.
The un(ortunate episode continued even a(ter the re!oval o( de2ris
3as oFciall* co!plete and the police !en 3ere 3ithdra3n (ro! the site.
Goardso( scrap !etal collectors descended on the crash site and (or three

continuous da*s that the R!etal scavengersS looted the site 3ith the !etal
re!ains shipped to various scrap dealers in Mangalore cit*.

As per the -rocedure Manualo( Accident J Incident investigation
pu2lished 2* ,ACA :Issue I rev 2 dated $.10.200%</ 3henever an accident
occurs/ the .3ner/ .perator/ -ilot;in;Co!!and/ Co;pilot o( the aircra(t shall
ta8e all reasona2le !easures to protect the evidence and to !aintain sa(e
custod* o( the aircra(t and its contents (or such a period as !a* 2e
necessar* (or the purposes o( an investigation su2Iect to the Indian Aircra(t
Rules 1+3&. #a(e custod* shall include protection against (urther da!age/
access 2* unauthoriMed persons.
The Court o( In)uir* involved in the Mangalore crash cala!it* had
done a scienti7c e1a!ination o( the RreconstructedS aircra(t. It 3as 3hile
e1a!ining these 1% tons o( salvaged aircra(t pieces that a !e!2er o( the
Court o( In)uir* tea! noticed the do3n3ard position o( the Hap locator/ a
7nger siMed !etallic s3itch in coc8pit used to !ove the Haps in the 3ings.
The reason (or the aircra(t to generate not enough li(t to ta8e o6 in the last
!o!ent 3as 2eco!ing clear then.
The panic8ed pilots !ust have (orgotten to push up the s3itch.I( a 7nger
siMed !etallic part could have spo8en so !uch a2out the crash/ the act o(
sha!e 3as the loss in precise investigation due to the sheer volu!e o( the
precious evidential scrap !etal that 3as sold in nu!erous shops scattered
across the cit*.
To carr* out e6ective rescue and recover* operations in an e!ergenc*
situation/ it is necessar* to cordon o6 $00 !eters around the crash area 3ith
no unauthoriMed person 2eing allo3ed inside. In the case o( Mangalore crash/

ever*2od* and an*2od* entered the site. Indeed/ so!e o( the! tried to help
the victi!s/ 2ut !ost o( the! onl* added to the con(usion. The narro3 road

including private cars and t3o 3heelers/ not allo3ing the e!ergenc*
vehicles to operate (reel*. The victi!sS 2odies 3ere da!aged so 2adl* that it
3as hard to identi(* the!. The disaster !anage!ent !achiner* had no clue
a2out a procedure called RTriage Area :TA<S 3here the 7re!an on dut* 3ill
deposit the recovered 2od*. This (acilit* 3as !issing at the crash site.
The Politics:
The post investigation indications 3ere clear that the o2Iective 3as not
to 7nd out the truth and co!e out 3ith procedures to prevent another
traged*/ 2ut it 3as a single !inded o2Iective to 2la!e the captain and give
a (air* tale ending to all the others 3ho are e)uall* responsi2le (or the (atal
traged*.
In @ove!2er 200&/ the ,ACA sa(et* oversight audit on Air India
01press had (ound several de7ciencies and the !anage!ent 3as noti7ed to
!a8e the corrections. The 7ndings included the (act that the airline did not
have a Gead o( #a(et* and Chie( o( Training as per the ,ACA regulations. In
Cune 2010/ 3hen a (resh audit 3as carried out on the airline/ the sa!e
de7ciencies 3ere (ound. Uet/ the conclusion in the report neither 2la!es the
airline nor the ,ACA (or per!itting the continued operation 2* an airline
3hich did not con7r! to 2asic sa(et* nor!s.
The Gead o( sa(et* should have 2een held responsi2le (or the 3rong
circular that 3as issued a2out hard landings. The captain had 2een
counseled earlier (or a Qhard landingT 3hich (ell into that categor* 2ut 3hich
3as 3ell 3ithin the !anu(acturerSs li!it. The (atal Hight 3hich 3as

descending at a ver* high rate 3as corrected to !a8e a s!ooth landing. The
circular 3ould have 2een at the 2ac8 o( the captainSs !ind. Uet/ this 3as
covered up 2* the Court o( In)uir*.
The !ost serious aspect is the (ailure o( the C.I in not indicting the Airports
Authorit* o( India. The report covers up the dangerous rigid concrete
structure that holds the Instru!ent Landing #*ste! Localiser antenna. This
is co!pletel* against the ICA. #tandards 3hich prohi2its an*thing other
than a (rangi2le structure in that area. The report clearl* states that the right
3ing 2ro8e 3hen it struc8 the concrete structure and the post;accident 7re is
evident in the slope 2e*ond the 2oundar* 3all. The illegal structure 3as
re2uilt and operations continue 3ith this dangerous structure re!aining.
Current Scenario:
T3ent* t3o !onths a(ter the horri7c crash o( Air India ?oeing I> '12/
3hich resulted in the death o( 1$' passengers/ the legal counsel (or Air India
= Mulla " Mulla ; Mu!2ai/ has settled 114 cases/ eight o( the! partiall*. Napi
lAseri/ chie( 7nance oFcer/ Air India/ said a total o( +% cases have 2een
