Case Diary Dated: 20-06-2013
This is a Case of Suicidal Death that occurred today i.e.
On 20-06-2013 at about 10-30 AM at Gandhi Nagar, Gunupudi of
Bhimavaram and reported the same at 11-15 AM wherein, one
Mandela Rama Krishna S/o Venkata Swamy, aged about 46 years who
herein-after be referred as deceased in this case having come to know
that his wife Smt. Mandela Vara Lakshmi Devi reported against him
including his younger sister and brother in-law alleging that they
harassing her, rushed to home by consuming some pesticide poison,
demanded his wife to withdraw the case against him, and turned to I
Town Police Station under Drowzy condition and was shifted to
Community health Centre, Bhimavaram for medical aid. Mandela Sai
Sandeep who is no other than the elder son of the deceased reported
the same for necessary action. While undergoing treatment the
deceased succumbed to the said poison.
Today i.e. On 20-06-2013 at 11-15 AM while I am present
in the Police Station in the temporary absence of the S.I. of Police
engaged otherwise at APPA Hyderabad for training in mandatory
courses, the marginally noted complainant followed by his mother and
younger brother turned to the Police station and presented an English

scribed report with regard to the ofence and the following is the
Verbatim of the said report.
I am hereby inform that my father M. Rama Krishna
harassing my mother at the instigation of his younger sister Chamana
Ratna Kumari and her husband Chamana Purna Venkata Sesha Rao.
They visited our house and settled here. My father through away my
mother from my house to that efect. My mother lodged a complaint in
your Police Station today morning. While I am at my house at about
10-30 AM my father came and said that he consumed poison and may
die. And went away by a motor cycle. Therefore requesting you to take
necessary action.
Basing on the said report, I registered a Case in Cr. No.
148/2013 U/s. 309 IPC of Bhimavaram I Town P.S. at 11-15 AM today
i.e. On 20-06-2013, submitted the original FIR together with the
complainants original report to the Honble Court of II AJFCM
Bhimavaram and copies to all the ofcers concerned and took up
investigation in this case.
Then I secured the presence of marginally noted
complainant, his mother and younger brother, examined them and
recorded their statements U/s. 161(3) Cr.P.C. in Part II of my case diary
appended herewith for perusal. They stated as follows.
LW-1 Mandela Sai Sandeep:He is resident of 1

year B.Tech. in
Hits College, ECIL, Keesaragutta of Hyderabad by residing at his
uncles house. His younger brother Sai Mandeep passed SSC and has
to join for intermediate. He studied up to the end of 6
th
Class and his
younger brother Sai Mandeep studied up to end of 4 th class in Sri
Vidhya School, Bhimavaram. His father harassed his mother since a
long time and thus his mother took him and his younger brother to his
uncles house at Hyderabad and stayed therein. There after he was

joined for 7

class at Hyderabad and his younger brother was joined for

th
class. About 1 year back his father visited Hyderabad and brought
back his mother stating that he will not harass her in future and on
that his mother came down Bhimavaram and living together with his
father in a private rented house at Gandhi Nagar, Gunupudi of
Bhimavaram as his aunt Chamana Ratna Kumari occupied their house
and residing therein along with her husband. Though, his father used
to harass his mother often and while caused her mental agony, today
i.e. On 20-06-2013 morning his mother lodged a complaint against his
father and two others who are no other than the younger sister and
brother in-law of his father. To that efect Police summoned his mother
and two others. Having come to know the said fact, today i.e. On 20-
06-2013 at about 10-30 AM when he along with his mother and
younger brother returned home from Police Station, his father came
there and asked his mother to withdraw the case against them
otherwise he will die because he already consumed pesticide poison.
They noticed foul smell emitting from the mouth of his father. On that
he along with his mother and younger brother turned to the Police
Station and lodged a complaint to that efect. Meanwhile, his father
rushed to Police Station on his Hero Honda CBZ motor cycle bearing
Regd No. AP 37 AU 995 in Drowsy condition. Then the Police shifted
his father to Govt. Hospital, Bhimavaram for medical aid and etc. facts.
LW-2 Smt. Mandela Vara Lakshmi : She is no other
than the wife of deceased. On examination she stated that she got
marriage with the deceased in the Year 1992 and after the marriage
she was sent along with her husband for their marital life. During the
wed-lock they were blessed with two sons namely Sai Sandeep and Sai
Mandeep. Both of them lived together happily for a period of 11 years
and later her husband used to harass her both physically and mentally.
As unable to bare the tortures of her husband about 7 years back when
children completed 6

class and 4

class respectively she left
Bhimavaram along with her children and stayed at the house of her
elder brother at Hyderabad and got admitted her children for their

further studies. While the matter stand thus, about 1 year back her
husband visited Hyderabad and brought her to Bhimavaram promising
that he will not harass her in future, occupied a private rented house at
Gandhi Nagar, Gunupudi of Bhimavaram, since their own house
occupied by her sister in-law Chamana Ratna Kumari who is residing
therein along with her husband. Thereafter her husband continued his
harassment towards her with the instigation of his sister Chamana
Ratna Kumari and her husband Purna Sesha Rao. On that today i.e.
On 20-06-2013 morning she lodged a complaint against her husband
include his younger Sister Ratna Kumari and her husband Purna Sesha
Rao. To that efect Police summoned them. Having come to know the
said fact, today i.e. On 20-06-2013 at about 10-30 AM when she along
her sons returned home from Police Station, her husband came there
and asked her to withdraw the case against them otherwise he will die
because he already consumed pesticide poison. They noticed foul smell
emitting from the mouth of her husband. On that she along with her
children turned to the Police Station and her eldest son Sai Sandeep
lodged a complaint to that efect. Meanwhile, her husband rushed to
Police Station on his Hero Honda CBZ motor cycle in Drowsy condition.
Then the Police shifted him to Govt. Hospital, Bhimavaram for medical
aid and etc. facts.
LW-3 Mandela Sai Mandeep:He is no other than the
youngest son of the deceased, Passed SSC recently. On examination he
stated the same facts as stated by LWs. 1 and 2.
While I was at my Police Station at 1 PM, received
intimation of death of the deceased from Community Health Centre,
Bhimavaram that the deceased died at 11-50 AM.
Basing on the death intimation, I altered the Sec. of Law
309 IPC in this case into 174 Cr.P.C. at 1 PM On 20-06-2013, submitted
the original altered FIR to the Mandal Executive Magistrate,

Bhimavaram Mandal and copies to all the ofcers concerned and
continued further investigation in this case.
Then I left the police Station, visited the residential house
of the deceased situated at Gandhi Nagar, Gunupudi of Bhimavaram,
examined the scene of ofence with the help of LW-1. The following is
the topography of the residential portion of the deceased.
There is black tar road running from West to East and
known as Gandhi Nagar road. To the North of the said road there is
up-stair building of Manne Hari Gopal Rao and to its East another
building belonging to Manne Ravindranath Tagoor in a premises as
Group of houses. The up-stair building faces East and the other
building faces East and West. There is vast compound wall covered
these houses with entrance at the Southern side with an iron grill gate.
The residential portion of the deceased in question is the ground foor
of the up-stair building and the up-stair portion is kept vacant. Manne
Rani who is the wife of Manne Ravindranath Tagoor is residing in the
Eastern side building. The said house in question the deceased is
residing is having bearing Door No. 9-7-29/3. To the North of the said
buildings, building of Sarabaiah which is kept locked, to the South
Gandhi Nagar road and further South Vacant site are situated. At the
West and East of the said houses, vacant sites are situated. Nothing
incriminating evidence is found at the residential portion of the
deceased. A rough sketch was drawn and the same is appended
herewith for perusal.
Then I rushed to Community Health Centre, Bhimavaram
along with my available staf, got photographed the dead body of the
deceased by private photographer. Then I summoned the marginally
noted Panchayatdars as well as relatives of the deceased and other
witnesses and in their presence I held inquest over the dead body of
the deceased at 3-30 PM onwards and got drafted inquest report in
detail.

It is a male dead body and named as Mandela Rama
Krishna S/o Venkata Swamy, A/46 Years, C/Kapu, R/o Gandhi Nagar,
Gunupudi, Bhimavaram. The deceased was frst seen by duty medical
ofcer when the deceased was found dead at 11-50 AM On 20-06-2013.
The deceased was last seen by the duty medical ofcer at 11-45 AM
when the deceased was alive and joined in the hospital for medical aid.
The dead body of the deceased is about 5 feet and 8 inches height with
white complexion. A black mole on right side of abdomen and another
black mole on below the left arm pit appears as identifcation marks.
The deceased is a male person. Black colour Jean Pant White polyster
shirt with black stripes desizn and two rows black waist thread found
as raiment over the dead body of the deceased. No external injuries
found over the dead body of the deceased. The dead body of the
deceased was found lying on its back with head towards North and legs
towards South in the Freezer in Mortuary shed, Community health
Centre, Bhimavaram.
During the inquest, I secured the following witnesses,
examined them and recorded their statements U/s. 161(3) Cr.P.C. in
part II of my case diary appended herewith for perusal. They stated as
follows.
LW-4 Bandela Sundara Sunil Kumar : He is resident of
Bethanipet, Bhimavaram and working as clerk in Luthern High School,
Bhimavaram. On examination he stated that today i.e. On 20-06-2013
at about 11-30 AM he visited I Town Police Station upon the
instructions of his Head master, requesting the Police to cause vigil on
the young folk causing disturbance by doing Cricket in the school
ground during the classes are going. Meanwhile, a male person aged
about 45 years rushed to Police Station on Hero Honda CBZ motor cycle
in drowsy condition due to consumption of some pesticide poison. Then
the police immediately referred the said person to Govt. Hospital,
Bhimavaram for medical aid. He learnt that the name of the said

person is Mandela Rama Krishna and he consumed pesticide poison as
his wife reported against him and others, in I Town Police Station. So
saying he stated that the said person succumbed to the said poison
while undergoing treatment.
LW-5 Mandela Krishna Murthy : He is no other than the
junior paternal uncle of the deceased, resident of Pasupuletivari Street,
Gunupudi of Bhimavaram and does vegetable business. On
examination he stated about his coming to know the death of the
deceased and etc., facts.
LW-6 Mandela Nageswara Rao : He is cousin of the
deceased resident of Nachuvari Centre, Bhimavaram and does vegetable
business. On examination he stated about his coming to know the
death of the deceased and etc., facts.
LW-7 Gurujukota Suryanarayana, HC 460 : He is Had
Constable, working in Bhimavaram I Town P.S. On examination he
stated that today i.e. On 20-06-2013 at about 11-45 AM when he was
in the police Station as Section in-charge the deceased rushed to I Town
Police Station on his Hero Honda CBZ motor cycle bearing Regd No. AP
37 AU 995 by consuming some poison. Immediately he shifted the
deceased to the Community Health Centre, Bhimavaram by Auto for
medical aid. But the deceased succumbed to the said poison while
undergoing treatment and etc., facts.
During the inquest, on hearing the contents of FIR in this
case and besides the contents of FIR in Cr. No. 147/2013 U/s. 498-A,
109 IPC and Sec. 4 of D.P. Act. 1961 of Bhimavaram I Town P.S.,
statements of the witnesses LWs. 1 to 7 and state of the dead body of
the deceased, the Panchayatdars gave their verdict that During the
year 1992 the deceased got marriage with Mandela Vara Lakshmi who
is no other than the lonely daughter of Manepalli Bhima Raju resident
of Gunupudi, Bhimavaram according to Hindu tradition and they were

blessed with two sons namely Sai Sandeep and Sai Mandeep and since
a long time the deceased used to harass his wife Vara Lakshmi and
while the matter stand thus about 1 Year back one Chamana Ratna
Kumari who is no other than the younger sister of the deceased
occupied the house of the deceased which is kept vacant on a plea that
their house is not suitable for them and residing in the house of the
deceased along with her husband, and since then they including the
deceased while harassing the said Vara Lakshmi both physically and
mentally and unable to bear the tortures the said Vara Lakshmi lodged
a complaint against her husband including his younger sister and her
husband on 20-06-2013 at 8 AM which was registered by HC 702 as a
Case in Cr. No. 147/2013 U/s. 498-A, 109 IPC and Sec. 4 of D.P. Act.
1961 of Bhimavaram I Town P.S. and while summoned the accused in
this case, having come to know the said fact, at 10-30 AM today i.e. On
20-06-2013 the deceased rushed to house and demanded his wife Vara
Lakshmi to with draw the case against them otherwise he will die
because he already consumed pesticide poison. In this regard, Mandela
Sai Sandeep who is no other than the eldest son of the deceased while
reported the same was registered as a Case in Cr. No. 148/2013 U/s.
309 IPC of Bhimavaram I Town P.S. and in the meanwhile the
deceased turned to the Police Station in Drowsy condition. Then HC
460 shifted him to Govt. Hospital, Bhimavaram for medical aid while
undergoing treatment the deceased succumbed to the said poison at
11-50 AM on 20-06-2013 and thus the deceased himself committed
suicide by consuming some pesticide poison.
I referred the dead body of the deceased for P.M.
Examination through PC 2328 to elicit exact cause of death of the
deceased.
Then I secured the presence of marginally noted LW-8 who
is private professional photographer, examined him and recorded his
statement U/s. 161(3) Cr.P.C. in Part II of my case diary appended
herewith for Perusal. He stated as follows.

LW-8 Maddala Philip :He is resident of Gutlapadu village
of Bhimavaram Mandal and he is private professional photographer. On
examination he stated about his made photographs of the dead body of
the deceased and handed over them to Police.
As seen from the evidence collected hitherto and on my
confdential enquiries, investigation in this case revealed that the
deceased in this case by name Mandela Rama Krishna S/o Venkata
Swamy, A/46 years, C/Kapu was resident of Gandhi Nagar,
Bhimavaram. The deceased got marriage with LW-2 Smt. Mandela Vara
Lakshmi in the Year 1992 and after the marriage they were lived
together happily for a period of 11 years and they were blessed with two
sons namely Sai Sandeep and Sai Mandeep. Thereafter, the deceased
used to harass his wife Vara Lakshmi subjected her to cruel and due
to unable to bare the tortures the said Vara Lakshmi left Bhimavaram
about 7 years back when her children completed 6

class and 4

class
respectively and gone to Hyderabad and stayed at the house of her
elder brother and got admitted her children for their further studies.
While the matter stand thus, about 1 year back the deceased visited
Hyderabad and brought her to Bhimavaram along with their youngest
son promising that he will not harass her in future, occupied a private
rented house at Gandhi Nagar, Gunupudi of Bhimavaram, since their
own house occupied by her sister in-law Chamana Ratna Kumari who
is residing therein along with her husband. Thereafter the deceased
continued his harassment towards his wife Vara Lakshmi and to that
efect she lodged a complaint with HC 702 of Bhimavaram I Town P.S. at
8 AM today i.e. On 20-06-2013 against the deceased including his
younger Sister Ratna Kumari and her husband Purna Sesha Rao which
is subjected matter in Cr. No. 147/2013 U/s. 498-A, 109 IPC and Sec.
4 of D.P. Act. 1961 of Bhimavaram I Town P.S them. Having come to
know the said fact, at about 10-30 AM today i.e. On 20-06-2013 the
deceased turned to his house by consuming some pesticide poison
when his wife and children returned home from Police Station, asked
her to withdraw the case against them otherwise he will die because he

already consumed pesticide poison. They noticed foul smell emitting
from the mouth of the deceased. On that immediately, LW-1 followed by
his mother and younger brother turned to the Police Station and lodged
a complaint to that efect. Basing on the said report, I registered this
case as Cr. No. 148/2013 U/s. 309 IPC of Bhimavaram I Town P.S. at
11-45 AM on 20-06-2013 and took up investigation in this case.
Meanwhile, the deceased rushed to Police Station on his Hero Honda
CBZ motor cycle in Drowsy condition. Then HC 460 shifted him to
Govt. Hospital, Bhimavaram for medical but while undergoing
treatment the deceased succumbed to the said Poison.
Basing on the death intimation, Sec. 309 IPC in this case
was altered into Sec. 174 Cr.P.C. No foul play is suspected in this case.
However, the exact cause of death of the deceased is to be elicited soon
after receipt of P.M. Certifcate.
