Assembly (NAPC-NCSA). Please be reminded of the following

Each applicant-organization must submit:
LETTER OF INTENT to apply for the NCSA addressed to:

Filled-out ORGANIZATIONAL PROFILE SHEET
Filled-out REPRESENTATIVE PROFILE SHEET
PROOF OF LEGAL EXISTENCE from any credible institution such as Securities and
Exchance Commission (SEC) registration or LGU or NGA Accreditation (e.g. DSWD, CWC,
NYC, NAPC) stating at least one (1) year of existence AND a good track record; and
BOARD RESOLUTION or CERTIFICATION that the participant is the duly authorized
representative of the organization

The deadline of all applications is on August 20, 2014. Applications must be sent to one of the
following:
1. E-MAIL APPLICATION: Download APPLICATION KIT <>, fill out all forms and send to
<napc.childrep@gmail.com> with the header NCSA APPLICATION <Name of Organization>.
2. BY-MAIL APPLICATION: Download APPLICATION KIT, print and fill-out all forms. Mail
forms with necessary requirements to:
