En los ltimos aos la ciruga de la inestabilidad glenohu-
meral ha sufrido un portentoso avance debido fundamental-
mente a un mejor conocimiento de la fisiologa de la estabili-
dad y de la implicacin de las estructuras anatmicas en su
fisiopatologa, sin olvidar la aportacin que ha hechola artros-
copia al mejor conocimiento de la articulacin. El xito de esta
ciruga pasa por un correcto diagnstico en relacin al tipo de
paciente, la direccin de la inestabilidad y IGS mecanismos ana-
tmicos y funcionales involucrados, as como por la correcta
reconstruccin de las estructuras lesionadas o alteradas con la
mnima agresin posible.
La valoracin del resultado final no ha de realizarse a corto
ni medio plazo, sino cuando se considere que el paciente est
totalmente reincorporado a su vida laboral y deportiva.
An existen multitud de aspectos desconocidos y otros sobre
los que resulta imprescindible profundizar, entre los que caben
destacar las causas genticas que determinan las propiedades
materiales y estructurales de algunos de los hombros inesta-
bles, la relacin entre control neuromotor y las estructuras
anatmicas que garantizan la estabilidad glenohumeral, la
mejora de tcnicas y de instrumentacin artroscpicas para
imitar en lo posible tcnicas abiertas con menor agresin tisu-
lar y la elaboracin de protocolos de evaluacin, tanto subje-
tivos como objetivos, que permitan el estudio racional de los
pacientes, de los resultados y de la comparacin de las dife-
rentes tcnicas.
