pyrexial. He is noticed to desaturate on minimal exertion. On examination there is a few crackles bibasally. A chest X
ray reveal perihilar bilateral diffuse infiltrates. What is the most likely diagnosis?

Explanation:
This patient presents with a pneumonia. The clue to the underlying diagnosis is the desaturation on exertion,
chest xray findings and minimal findings on chest which are all indicative of pneumocystis jiroveci. Klebsiella
tens to lead to upper lobe cavitating lesions and staphylococcus aureus lead to multilobar consolidation.

A 28 year old female presents with acute onset of shortness of breath and
pleuritic chest pain. She has just recently flew back from Australia . She has
no past medical history although she is on the contraceptive pill. On
examination there is evidence of a swollen tender left calf. On auscultation of
the chest it is clear however she is tachycardic. A CXR reveal nil focal. What
