CHAPTER I

INTRODUCTION
A. BACKGROUND
Community demands for the quality of nursing care perceived as a phenomenon that
must receive immediate response from the nurse . Responses are intended primarily deductive to
always improve themselves in terms of learning more about the concept of nursing service
management and concrete steps in its implementation . Concrete steps that can be a model
structuring nursing care , nursing personnel arrangement and improvement of nursing
documentation system .
The current nursing management require attention and top priority in the future
development of nursing . It is closely related to the demands of the profession as well as global
demands on the quality of nursing care , requiring professional management , in particular the
ability of the professional nurse in performing managerial role as a nurse manager . Now this , as
we know that the health care system , especially nursing care systems undergo rapid changes .
The changes are in addition to the increasing nursing science and technology , as well as the
impact of the political situation and social systems , economic shifts . Of the three aspects of
change that has implications for nursing care system changes as well as a challenge for
professional nurses ( Nursalam , 2002) .
Nursing management must be applied in order real service , both in hospitals and in the
community , so that nurses need to understand the concept of nursing service management ,
particularly on the application of the role of the nurse manager . Concepts that must be mastered
is the concept of change , the concept of nursing management , preparation of planning (
strategic plan ) and problem solving steps .

DISCUSSION
I. Definition of Nurses
The nurse is someone who has the knowledge , skills and authority to provide nursing
care to others based on its knowledge and tips within the limits of its authority . ( PPNI , 1999;
Chitty , 1997) .
The nurse is someone who has graduated nurse education both at home and abroad in
accordance with the legislation in force . ( Decree of the Minister of Health No.
1239/MenKes/SK/XI/2001 of Registration and Practice Nurses in article 1 , paragraph 1 )
II. Role of Nurses
The role is a set of behavior expected by others against a person , the corresponding
position in the system . The role is influenced by social circumstances both within and outside
the profession of nursing and is constant .
1 ) Care Giver
The nurse should :
a) Taking into account the appropriate individual in the context of the client 's life , nurses should
pay attention to the client based on the client's needs significant .
b ) Nurses use Nursing Process to identify nursing diagnoses , ranging from physical (
physiological ) to psychological problems
c ) Its main role is to provide nursing care to individuals , families , groups or communities
according diagnoses problems that occur from problems that are simple to complex .
2 ) Client Advocate
As a client advocate , the nurse is responsible for helping clients and families in interpreting
information from various service providers and to provide other information necessary to take
approval ( informed concent ) for nursing actions that are given to him .
Besides nurses must maintain and protect the rights of clients . This must be done because the
client is ill and hospitalized will interact with many health workers . Nurse is a member of the
health team the longest contact with the client , therefore the nurse must defend the rights of
clients .

3 ) Counselor
a) The main task of the nurse is to identify the client to change the pattern of interaction
healthy state of pain .
b) A change in the pattern of this interaction is the " Basic " plan methods to improve
adaptability .
c) counseling given to depressed individuals / families in integrating health experience with
past experience .
d) is focused on solving problems ; nursing problems , health behavior change ( changing
patterns of interaction )

4 ) Educator
a) This role can be carried out to clients , families , other health care team , either
spontaneously ( sat interactions ) and formal ( prepared ) .
b) The task of the nurse is to help clients enhance knowledge in an effort to improve health
, the symptoms of the disease according to the conditions and specific actions .
c) Basic implementation is the role of intervention in the NCP .

5 ) Managerial
The role of the nurse is to direct , plan , organize service of all members of the health
team . Because the client receives the services of many professionals , for example ; of nutrition .
Aspects to be considered are ; the type, quantity , composition , preparation , management ,
providing , monitoring , motivation and so on .

6 ) Collaborator
In this case the nurse with clients , families , other health team seeks to identify the
necessary health services including brainstorm the necessary services clients , providing support ,
alloy expertise and skills from various professional health care provider .

7 ) Consultant
This element is not directly related to the client's request to information about the purpose
of nursing is given . With this role can be said treatment is ang resources related to specific
conditions of the client .

8 ) Change Agent
This element includes planning , cooperation , systematic changes in the client and how
to relate primarily to the provision of nursing to clients .


III . Role of Nurse Managerials
Nurses have a role and responsibility in managing nursing services in order sema health
services (hospitals , health centers , and so on ) as well as educational settings that are in
accordance with the concept of the responsibility of nursing management . Nursing management
can be defined as the process of implementing nursing services through the efforts of the nursing
staff members of nursing care , treatment , and safety to patients / families / communities ( Gilies
, 1985) .
The role of the manager in the health care environment is currently experiencing
significant change , in which the health care organization to decentralize the functions of
management and organization of each unit by a care manager .
As for some of the responsibilities given to the nurse manager ( Potter & Perry , 2005 ) ,
among others, the responsibility to :
1 ) Employ , develop and evaluate staff .
2 ) Development of the annual budget and led a unit that holds the authority to set the
corresponding units of the fund.
3 ) Monitor the quality of care , facing labor problems , and do these things cost- effective .

Furthermore Potter & Perry ( 2005 ) , states that to be done in his role as a professional
nurse manager nursing care are :
1 ) Planning / goal setting
Helping patients and their families in formulating a picture of health
after returning from hospital care
2 ) Teaching / orientation
Understanding the function and information to encourage health patient / family
3 ) Coordinating with service
Helping families in utilization of support services ( religious leaders , home care ) and patient
care scheduling .
4 ) Development of a support system
Stressing on the patient and family to think of a greater responsibility in maintaining health
5 ) Trust working group or profession
Actively participate in group work or participate in activities in the community

Competencies Nurse Manager
According to Potter & Perry (2005 ) , says there are some competencies required of a
nursing manager by increasing the effectiveness of work :
1 ) Leadership
Communicate about the organization , the organization's activities and the implementation
of changes
Delegating tasks and accept responsibility
Creating a conducive organizational culture and effective
Implement effective leadership style
Involving staff in the development of the organization
Flexibility in the implementation of regulations



2 ) Decision-making and planning
Re-think and reorganize organizational priorities
Quick response to unexpected changes
Anticipate changes in budget planning
Provide guidance on organizational decision
Interpreting economic change staff

3 ) Relationship / Communication
Empathy , listening and responsive staff statement
Creating a conducive situation in communication
Demonstrate self-confidence through the ability to communicate
Develop a good relationship processes within the organization

4 ) Budget
Controlling budget
Interpreting the use of the budget as needed
Plan annual budgets ( 5 years )
Consult on financial issues

5 ) Development
Develop effective working team
Develop professional relationships between staff

Role and Function of Nursing Management .
As well as general management approaches , roles and functions of nursing management
consists of planning, organizing , staffing , directing ,and controlling .
1) Planning
In the planning process , define the mission , vision , goals , policies , procedures , and
regulations in nursing services , and then make an estimate of the projected short-term and long-
term as well as determining the amount of fees and set the change of plans .
2) Organizing
Includes several activities including organizational structure is set , the model determines the
assignment of nursing according to the circumstances and the client's workforce , grouping
activities to achieve the objectives of the unit , working within established organizational
structure and to understand and use the appropriate power and authority .
3) Staffing
Includes activities related to employment including the recruitment , interviewing , orienting
staff , schedule and socialize new employees as well as staff development .
4) Directing
Includes motivation , supervision , resolve the conflict , delegation , how to communicate
and facilitation for collaboration .
5) Controlling
Includes the implementation of performance appraisal of staff , financial accountability ,
quality control , legal and ethical aspects of control and the control of professional nursing care.

A. Conclusion
The nurse is someone who has the knowledge , skills and authority to provide nursing
care to others based on its knowledge and tips within the limits of its authority . ( PPNI , 1999;
